All items listed in punch list of February 8, 1984 must be completed to the satisfaction of the Engineer. 2. Make all necessary repairs to the sanitary sewer in N.E. 20th Street and Jores Ave. K.E., including re-TV of repaired lines. 3. Box 5036 Kent WA 98031 Subject: CAG 047-83 Kennydale Sanitary Sewers and 1983 Sanitary Sewer Rehabilitation Project Inspection Gentlemen: An inspection was performed for the referenced rroject on February 6 and 7, 1984. The following items are required before i final inspection of the project is performed. 1. All spoil sites will require a fill permit from the City Building Dept. All spoil sites must be araded and hydroseeded in accordance with permit requirements. A certified letter from the affected property owner is required stating that the property has been restored to his satisfaction. 2. The wearing course of the trench patch in Wells Ave. S. will require removal and replacement. This includes all side sewer patches as well as the main line trench. The pavement will require re-sawcutting to provide neat straight edges. 3. Seal pavement patches at South 4th Street and South 6th Street. 4. Provide as-built information for entire -oject. All manholes require location by street stationing relative to the contract drawings. Sevmr stub locations may be referenced from the manholes, indicate depth of sewer stubs at property line. As-built information must be stamped by a registered land surveyor. 5. Dress up,stabilize, and restore vegetation on roadside banks, north side of N.E. 20th Street at Blaine Ave. N.E. and South side of N.E. 20th Street at Dayton Ave. N.E. 6. Clean and stabilize all roadway drainage ditches that have been disturbed. 7. Clean all drainage pipes and ditches silted up as a result of project. February 8, 1984 Page 2. 8. A letter is required from property owners on the sewer rehabilitation work whose property was used for storage of materials stating the pro- perty has be*" restored to their satisfaction. 9. Complete tie-ins of water line in Jones Ave. N.E. Please notify this office when all the above items are completed in order that a final inspection of the project may be scheduled. Please be advised that future violations may result in impoundment of materials or equipment. In accordance with Section 4-1.12 of the Standard Specifications, a permit must be obtained by the contractor for all waste sites. A copy of the permit must be submitted to the Department of Public Works. Before the project is accepted by the City, a letter is required from the property owner stating that the site has been left in an acceptable condition. All waste sites will require grading and hydroseeding. All properties that have been used for storage of materials or equipment or otherwise damaqed by trespassing will require a letter from the owner stating that the property has been restored to their satisfaction. The Kennydale projects are: 1. N.E. 10th St. - Jones to Edmonds Ave. N.E. This has been budgeted $57,000 from our May Creek Reserve Fund. (All that was available). Bob Bray's estimate for construction only is: $103,934.00 2. Jones Ave. N.E. - N.E. 20th St. to N.E. 24th St. This is new, part of the future trunk suer under tte street over- lay project, 620 feet of 8" sanitary sewer. Not budgeted, but at a minimum included in the Sever work, this is a key segment of the Kennydale Interceptor. Estimated cost: $ 32,425.00 3. Future Steps: Grant priorities set late April, 1983. City will know If we are in further running. Plan could be completed May let +. Easements can almost all be obtained by dedication at no cost. All property owners I hove talked to are in strong support. If the grant is obtained, City would have to come up with $560,000 as our local share, with State at $400,000. Not all of local share would have to be provided in 1983, but could be spread into 1984. tf the grant is not obtained, City should start to build the gravity line in the creak in this year's dry spell in phases, with work in the upper segment first coming from Sunset and Union.
