NARRATIVE
On 11/25/2025 at approximately 1648 hours, I was dispatched to a two-vehicle collision with reports
of injury at the intersection at Maple Valley Hwy and 154th PL SE within the City Limits of Renton,
County of King, State of Washington.
Upon arrival, I contacted the involved parties and determined that the driver of Unit#2 was
complaining of lower back pain. The driver stated that due to this collision she was experiencing
minor back spasms. She explained that she had long lasting back injuries from a previous collision
with a DUI driver. The driver stated she would visit their medical provider after the collision is
investigated.
While on scene, I collected the involved parties driving documents and their independent recollection
of events leading up to the collision.
The driver of Unit#1, identified as Preston Coleman, said he was the sole occupant of his vehicle and
was traveling eastbound on Maple Valley Hwy and approaching the intersection of 154th PI SE, in
Renton. Preston changed lanes from lane 2 of 2 into the left turn lane as he intended to travel north
on 154th PI SE. Preston traveled directly behind Unit#2 and saw that Unit#2 had come to a stop for
queued traffic at the intersection. Preston stated that he simply was unable to stop in time and
subsequently collided with the rear of Unit#2 causing minor damage to the front of Unit#1.
The driver of Unit#2, identified as Heather Bushaw, said she was the sole occupant of her vehicle
and was stopped at the intersection of 154th PI SE and Maple Valley Hwy. Heather was in the left
turn lane of Maple Valley Hwy, facing east. While stopped for queued traffic, Unit#1 collided with the
rear of Unit#2 causing minor damage to the rear of Unit#2.
Due to the possible injury, this collision was recorded on Sector.
Preston was not paying attention to traffic in front of him. He violated Renton Municipal Code (RMC)
10-12-25-Inattentive Driving which is defined as any person to drive a motor vehicle on any public
highway, street, alley, or road in an inattentive manner in the City of Renton. "Inattentive manner" in
this section means a lack of: 1) attentiveness required to safely operate the vehicle under the
prevailing conditions, including, but not limited to, the nature and condition of the roadway, the
weather conditions, the presence of pedestrians, the presence of other traffic, or by the driver
focusing or directing the driver's attention to something other than driving the motor vehicle; or 2)
attentiveness that would permit the driver of a motor vehicle to observe anything resting on, or
traveling on, or entering the roadway in time to take appropriate action as circumstances require.

Based on the above statements, I determined that Preston is the proximate cause for the collision
due to inattention causing a collision.
I gave Preston a verbal warning for Inattentive driving. Both involved vehicles were able to be driven
away without further incident. An exchange of information was provided to all involved parties.
