EG62443 CASE# 25-80803 DATE OF COLLI r�510NN + 12/17/25 17:29 L1 NARRATIVE On December 17, 2025, 1 was assigned to District 22 as Unit 1 R22. At approximately 1719 hours, I was dispatched to a two-vehicle collision at NE 4th St. and Rosario Ave. NE, within the City of Renton, King County, Washington. Per dispatch notes: Two-vehicle accident, no injuries reported, blocking eastbound lanes. Vehicles involved were a black Lexus and a black Jeep Cherokee. Upon arrival, I contacted the driver of Unit 1, WA license plate CUU6204, and positively identified him as Aniken Kwak via his Washington State intermediate driver's license. Kwak stated he was traveling eastbound in Lane 1 on NE 4th St. when Unit 2 pulled out and stopped in front of him, causing the collision. David Macvicar, age 16, stated he was the passenger of Unit 1. I observed Unit 1 to have damage to the front right side. Both occupants of Unit 1 denied any injuries. Unit 1 was privately towed from the scene. I then contacted the driver of Unit 2, WA license plate BBZ5371, and positively identified him as Michael Appleby via his Washington State driver's license. Appleby stated he was attempting to make a left turn from Rosario Ave. NE onto NE 4th St. He stated he did not see Unit 1 because its headlights were not on, the vehicle was black, and it was dark outside. Serena Tran, the passenger in Unit 2, reported neck pain but declined medical evaluation by Renton Fire. Unit 2 had damage to the front left side. I asked Kwak if his headlights were on at the time of the collision. He stated they were on automatic mode and were illuminated. Based on the statements provided and evidence observed at the scene, it appears Unit 1 was traveling eastbound on NE 4th St. when Unit 2 attempted to make a left turn from Rosario Ave. NE, resulting in the collision between the two vehicles. I provided both drivers with copies of the exchange of information form. issued Kwak a warning for violation of RCW 46.20.267 (Intermediate Driver's License Restrictions), as he was operating a vehicle with a passenger under the age of 20, specifically David Macvicar, age 16.
