DL State DLExp Address Primary Phone&Type AdditionalContact Number&Type Additional Contact Number&Type Race Sex DOB- Height Weight Even Hair Collision 2025-00081187 Paqe 2 OF 4 Case Number 2025-00081187 Officer Narrative Narrative ASSIGNMENT/ARRIVAL On 12/31/2025 at approximately 1439 hours, I was dispatched to a two vehicle collision in the 1300 block of N Southport Drive in the city of Renton, King County,Washington. Dispatch advised both parties drove their vehicles into the Lowe's parking lot at 1000 Garden Avenue N for contact. I arrived at Lowe's at approximately 1450 hours. UNIT 1:ACURA INTEGRA Upon arrival I observed a silver Acura Integra (WA/CNG3261, Unit 1) in the Lowe's parking lot. Unit 1 had moderate front end damage (scratches, dents and deformation)to the front bumper and the leading edge of the hood. UNIT 2: SUBARU IMPREZA I observed a blue Subaru Impreza (WA/CNL1849, Unit 2) parked nearby. Unit 2 had minor damage (scratches)to the rear bumper. DRIVER 1: SMITH JAYLYN D. I contacted Jaylyn D. Smith (DOB: 04/27/2009) standing near Unit 1. 1 positively identified Smith by his valid Washington Driver License and established him as the driver of Unit 1 by his admission. Smith was not injured in the collision. Smith said he drove Unit 1 west on N Southport Drive to turn left (south) onto southbound 1-405. The traffic signal changed from green to red and Unit 2,who was driving in front of Unit 1, suddenly stopped before the intersection. Smith was unable to stop Unit 1 in time and collided This incident occurred in the sCity of Renton,King County,Washington State. !declare underpenalty of perjury under the laws of the State of Washington that the foregoing is true and correct. DRIVER 2: GARCIA CHRISTIAN I contacted Christian Garcia (DOB: 05/20/1992) sitting in Unit 2's driver seat. I positively identified Garcia by his valid Washington Driver License and established him as the driver of Unit 2 by his admission. Garcia was not injured in the collision. Garcia's statement aligned with Smith's statement. PASSENGER: SCULLIN, CHLOE S. Chloe S. Scullin (DOB: 01/02/1990)was the uninjured front passenger of Unit 2. DISPOSITION All parties exchanged information. This report is for documentation only.
