EG65610 CASE# 25-80665 DATE OF COLLI r�510NN + 12/13/25 11:37 L1 NARRATIVE On 12/13/2025 at about 1456 hours Diana C. Sefi-Cyr called 911 to report an earlier hit and run collision that occurred in the 4600 block of NE 4th St within the city limits of Renton, King County, Washington. I spoke to Diana over the phone, and she explained what happened. Driver 1-Unknown. Unit 1-white colored Dodge Ram pickup truck with Washington state license plate D46614F. Driver 2-Thomas E. Cyr. Unit 2-light blue colored 2012 Toyota Camry with Washington state license plate APK4074. On 12/13/2025 at about 1137 hours, Unit 2 was traveling in lane 2 headed westbound on NE 4th St. After passing Duvall Ave NE, the rear end of Unit 2 was struck by the front end of Unit 1. Driver 2 pulled his vehicle into a nearby parking lot, but Unit 1 continued westbound on NE 4th St. Unit 1 then turned right (north) into a nearby apartment complex (possibly the Windsor Apartments). Driver 2 attempted to locate Unit 1, but Unit 1 was no longer seen in the area. Driver 1 did not appear to make any attempt to contact Driver 2 before leaving the scene of the collision. Unit 2 had damage to the rear end of the vehicle, but it was able to be driven from the scene. Diana C. Sefi-Cyr was seated in the front passenger seat of Unit 2 at the time of the collision. Driver 2 complained of neck pain, and Diana complained of back pain because of the collision. Unit 1 may have damage to the front end of the vehicle from the collision, and it is unknown if Driver 1 suffered any injuries. Driver 2 and Diana did not get a look at the driver of Unit 1, and they were only able to describe Unit 1 as a white colored Dodge pickup. On 12/13/2025 at about 1137 hours, a witness (Rebekah Donnelly) called 911 and reported just seeing a hit and run collision near the intersection of NE 4th St/Duvall Ave NE. At that time, 1 drove through the area of the intersection, but 1 did not locate the collision or anyone involved. Diana had not yet called 911 to report the collision. After I spoke to Diana, I referenced the earlier call and spoke to Rebekah over the phone. Rebekah was driving westbound on NE 4th St when she saw Unit 1 turn right (west) onto NE 4th St from Duvall Ave NE. Unit 1 was traveling in front of Rebekah when Rebekah saw Unit 1 collide into Unit 2 just west of Duvall Ave NE. Rebekah saw Unit 1 drive off and then turn right (north) into the area of a nearby apartment complex just east of the Safeway grocery store. Rebekah was able to use her cellphone to take photos of the back side of Unit 1 before losing site of the truck. I sent Rebekah an Axon link to upload the photos, and the photos showed a clear image of Unit 1's rear license plate (Washington state license plate D46614F). The truck in the image was white in color and appeared to be a Dodge make. Rebekah did not see the driver of Unit 1. The license plate to Unit 1 was registered to Diane L. Shepard (DOB: 12/11/1950) who resides at 19126 SE 133rd PI, Renton, Washington; however, the driver of Unit 1 at the time of the collision was unknown. I went to Diane's home and informed Diane that her pickup truck was involved in a recent hit and run collision. 1 saw Diane's Washington state DOL photo and recognized the person I spoke to as Diane Shepard. Diane said she did not remember where her vehicle may have been on 12/13/2025, but she also said she has not given anyone permission to drive her truck. I noticed a young adult male (name unknown) was inside the house and may be living there. 1 also saw an unidentified female walk out of a camper parked in the driveway at Diane's residence. The unidentified male and Diane both said the truck had existing damage to the front end. Unit 1 was parked on the street in front of Diane's house, and I noticed what appeared to be newer damage to the front driver side fender of the truck; however, I did not notice any paint transfer to the truck fender. The front chrome bumper had minor dings, but unknown if any damage was new. I took photos of Unit 1 and uploaded those photos to Axon. 1 gave Diane a Renton PD business card with the case number and urged her to contact me if she has any additional information regarding the collision.
