ASSIGNMENT/ARRIVAL
On 01/23/2026 at approximately 1042 hours, I was dispatched to a two-vehicle collision near
the intersection of S 2nd Street and Wells Avenue S in the city of Renton, King County,
Washington.
I arrived at approximately 1045 hours.
OBSERVATIONS
Upon arrival I observed a white Toyota Corolla (WA/ BKG4527, Unit 1) stopped facing southwest
in the intersection of S 2nd Street and Wells Avenue S in the westbound number one (closest to
north curb) lane prolongation of S 2nd Street, which was a one-way, westbound street.
The front, driver door of Unit 1 was engaged with the front-passenger bumper of Unit 2, a 2018
Gillig bus (WA Exempt/C2856C) operated by King County Metro. Unit 2 was stopped facing west
in the westbound number two (closest to south curb) lane prolongation of S 2nd Street just in
the intersection after the white stop bar.
DRIVER 1:JOHNSTON DEBORAH A
I contacted Deborah A. Johnston (DOB: 12/14/1950) sitting in the driver seat of Unit 1. 1 positively
identified Johnston by her valid Washington Driver License and established her as the driver of
Unit 1 by her admission.Johnston was not injured in the collision.
DRIVER 2: LESTER,JEAN M.
I contacted Jean M. Lester(DOB: 01/29/1969) standing in Unit 2's passenger area. I positively
identified Lester by her valid Washington Driver License and established her as the driver of Unit
2 by her admission. Lester was not injured in the collision.

Lester was a King County Metro bus driver. Unit 2 did not have passengers aboard at the time of
the collision. A King County Metro supervisor responded to the scene.
STATEMENT: LESTER JEAN M.
I spoke with Lester about the collision.The following is a summary of our conversation:
Lester drove Unit 2 west on S 2nd Street toward Wells Avenue S on a green light. Unit 1 drove
next to Unit 2 in the number one lane. Unit 1 suddenly accelerated and started turning left
(toward the south)to drive into the southbound lane of Wells Avenue S.
Lester turned Unit 2 left (southwest)to attempt to avoid the collision. Unit 2's front-passenger
bumper collided with Unit 1's front, driver door.
This concluded my conversation with Lester.
STATEMENT:JOHNSTON, DEBORAH A.
I spoke with Johnston about the collision.The following is a summary of our conversation:
Johnston drove Unit 1 west on S 2nd Street and tried to turn left(toward the south) into the
southbound lane of Wells Avenue S.Johnston admitted she turned from the number one lane,
not the number two lane.
This concluded my conversation with Johnston.
PROXIMATE CAUSE
The proximate cause of the collision was determined by my observations of Unit 1 and Unit 2's

Johnston's action was the proximate cause of the collision, namely her violation of RCW
46.61.290(2), which states:
The driver of a vehicle intending to turn shall do so as follows: Left turns. The driver of a
vehicle intending to turn left shall approach the turn in the extreme left-hand lane
Lawfully available to traffic moving in the direction of travel of the vehicle.Whenever
practicable the left turn shall be made to the left of the center of the intersection and so
as to leave the intersection or other location in the extreme left-hand lane lawfully
available to traffic moving in the same direction as the vehicle on the roadway being
entered.
CITATION
I authored Johnston a citation (#6A0128828)for a violation of RCW 46.61.290(2) and filed it with
the Renton Municipal Court.
DISPOSITION
Both drivers exchanged information. I provided both drivers the case number on my business
card. I photographed both vehicles.
This concluded my involvement in this case.
