EG72210 CASE# 25-80978 OATS OF COLLI r�510NN + 12/23/25 12:46 L1 NARRATIVE On December 23, 2025, at approximately 1246 hours, I was dispatched to a hit and run vehicle collision at the intersection of Rainier Ave N and Renton Ave Ext, within the City Limits of Renton, County of King, State of Washington. Per a witness, one of two vehicles was seen fleeing the scene. Upon my arrival, I confirmed there were no complaints of injury requiring immediate medical response at the time of report. There, I was able to collect the involved party's information and independent summary of the events leading up to the collision. The driver of Unit#2 identified as Azizah Ahmach, said she was traveling westbound through the intersection of Rainier Ave S and Airport Way in lane 2 of 2. As she proceeded through the intersection, Azizah noticed Unit#1 exiting the private parking lot of ARCO AMPM. As Azizah passed Unit#1, both vehicles subsequently collided in the roadway causing her airbags to deploy. Azizah said she never saw how the collision occurred, but Unit#1 fled the scene after colliding with her vehicle. Unit#1 was last seen going northbound on Hardie Ave NW. An independent witness, identified as James Levias, told me he never saw the collision but heard it while sitting at the intersection of Rainier Ave N and Airport Way. James was the first in line at the intersection facing west when he heard the collision. When he finally noticed where the involved vehicles were, he saw a white Chevy truck fleeing the scene. James tried intercepting the truck, so he made a right turn and drove north on Rainier Ave N. James then made a left turn onto NW 3rd PI and drove westbound. As he crossed Hardie Ave NW, he saw that Unit#1 had already crossed the intersection and was traveling northbound on Hardie Ave NW and approaching NW 4th St. James continued west through the intersection until he arrived at Taylor PI NW. James then made a right turn and traveled northbound until he came across Unit#1. James located Unit#1 at the intersection of NW 4th PI and Taylor PL NW. Using his cell phone, James recorded the vehicle and its driver, which I reviewed. The driver appeared to be a Hispanic male in his 40's wearing black clothing and Unit#1 was identified as a 2002 Chevy Silverado bearing Washington plates D10196H. Per the vehicles DOL return, the registered owner is Francisco Juarez- Montiel (12/02/1981) who lives in Auburn. After reviewing Francisco's DOL photograph and the recorded video, I confirmed Fransico was the driver of the white Chevy Silverado. His round face, dark hair, and mustache were identical to the driver seen in the video. I provided Azizah my contact information and case number and asked if she wanted to pursue criminal charges against the other driver. Azizah said "yes." Unit#2 sustained severe damage to the passenger side wheel and fender. The vehicle became undrivable, so Azizah arranged a private tow using Gene Meyers Towing. I reviewed ARCO's surveillance cameras and reconfirmed that Francisco was driving the Chevy Silverado. Francisco and his front passenger sat in the property for about an hour. During that time, Francisco entered the gas station, used the bathroom, and reentered his vehicle. He then leaves the property around 1243 hours and is seen entering Renton Ave Ext. Fransico enters the westbound merge lane (lane 1) and makes a U-Turn from lane 1 of 3 to lane 3 of 3. As he turns, he strikes Unit #2 which is traveling eastbound on Renton Ave S. After the two vehicles collide, they move away from the camera frame and cannot be seen afterwards. Based on the information above, I determined that the driver of Unit#1 is the proximate cause as the driver violated RCW 46.61.295. This law states that driver is not allowed to make a U-turn unless it PAGE 3 OF 5 REPORT NO. The driver of Unit#1, Francisco Juarez-Montiel (12/02/1981), also committed an attended hit and run violation by refusing to provide any of the required documents to the other driver, striking an attended vehicle, and fleeing the scene. Sector citation 5AO946250 and Sector infraction 5AO946251 were submitted to the Renton City Prosecutor for further review. I will contact ACRO management when I return so I can obtain a copy of their surveillance footage.
