EG74949 CASE# 26-817 OFC COLLISION 01/28/26 18:31 OF COLLISION NARRATIVE On 01/28/2026, 1 was working uniformed patrol in a marked police car as 3R23. At approximately 1831 hours, officers were dispatched to an unknown injury collision at the intersection of SW Grady Way/Lind Ave SW, which is located within the city limits of Renton, County of King, Washington. Dispatch advised that a witness had reported a 2-car crash at the intersection. When I arrived at the crash location along with officer-in-charge (OIC) S. Farjarillo. We were able to locate both vehicles involved and had identified both vehicles at this time as Unit 1 and Unit 2, respectively, as described above. I saw that Unit 1 had pulled off just north of the intersection. I also saw that Unit 2 was still in the middle of the intersection. I went over to the sidewalk and was able to locate all the occupants of Unit 2. 1 identified all subjects inside Unit 2 at this time, as detailed above. I spoke to the driver of Unit 2. He described the following: The driver of Unit 2 mentioned he was in lane 3 of northbound Lind Ave SW, preparing to continue northbound past SW Grady Way. The driver of Unit 2 mentioned that he had a green light and proceeded into the intersection. At this time, the driver of Unit 2 mentioned that he saw Unit 1 in the southbound turn lane of Lind Ave SW. Unit 2 then struck the front driver's side area of Unit 1 as he was trying to turn eastbound onto SW Grady Way. I took a look around Unit 2. 1 saw that a combination of airbags inside the vehicle had deployed. I also saw that the front bumper and front passenger side area sustained significant denting and scratches, which appeared to disable the vehicle. The driver of Unit 2 mentioned that all subjects inside his vehicle were wearing seatbelts. He also mentioned that the two children in the back were in appropriate car/booster seats, respectively. When I asked about injuries, they reported the following: The driver of Unit 2 reported that he believed his wrist may have been broken and was unable to move it. He also reported shoulder pain. The front passenger of Unit 2 reported she had stomach pain. The driver-side rear passenger was a young child who appeared to have minor bleeding coming from his mouth area. The passenger-side rear passenger was another adolescent who reported stomach pain. I was able to speak to OIC S. Fajarillo, who spoke to the driver and sole occupant of Unit 1. He relayed the following: OIC S. Fajarillo confirmed that the driver of Unit 1 was the sole occupant of Unit 1. The driver of Unit 1 advised officers that he was in the southbound turn lane of Lind Ave SW at SW Grady Way. The driver of Unit 1 was set up in the left-hand eastbound turn lane to head eastbound onto SW Grady Way. The driver of Unit 1 mentioned that his light was a flashing yellow. As Unit 1 began its eastbound turn, the front end hit the driver's side of Unit 2. OIC S. Fajarillo mentioned that he did not see any airbags deployed in his vehicle. He also mentioned damage to the front passenger side bumper of the vehicle. OIC S. Fajarillo also mentioned that he was not sure if the driver of Unit 1 was wearing a seatbelt. There were also no reported injuries from the driver of Unit 1 at the scene. The Renton Fire Department was called to evaluate all patients at the location. Gene Meyer tow eventually arrived and took possession of Unit 2. The driver of Unit 1 was able to leave the location in his vehicle. The occupants of Unit 2 were all transported to Valley Medical Center (VMC) to be further evaluated out of precaution. Based on the fact that the driver of Unit 1 described that he had a flashing yellow signal; the fact that there is a sign adjacent to the signal indicating for eastbound turning traffic on southbound Lind Ave SW to yield to oncoming traffic before turning; and the fact that Unit 1 had turned on the flashing yellow signal striking Unit 2 resulting in an injury collision, I found reasonable cause to issue the driver of Unit 1 a citation for RCW 46.61.050-failure to obey traffic control device. I issued a SECTOR citation to be mailed to the driver of Unit 1. Associated photos were uploaded to Evidence.com to be added to the case file. Based on the totality of circumstances, Unit 1 appears to be the proximate cause of the collision. PAGE 3 OF 5 DATE AND REPORT NO.
