1. Project Name: Rebecca Holt – Replace Fargo’s and Disconnect/Reconnect Handhole.

2. The name, source, intensity, and location of the sound/project limits. Potelco trucks
3. Description of work. Access to J-box and Handhole to change out the fargo’s and
disconnect/reconnect at the handhole
4. The equipment and vehicles used for the work (please provide a table of the equipment
to be used with its appropriate decibels).

5. Proposed Noise Mitigating Measures the applicant will implement to minimize the sound
level violations. Use hand tools when possible. Limit run time of gas-powered equipment.
