On 03/28/2026, 1 was working uniformed patrol in marked Renton Police vehicle #13379 and wearing
a jumpsuit with Renton Police patches on each shoulder as well as the words "Police" written on the
front and back of my external vest. The below described incident is intended to be a summarized
version of what was captured on Axon Body Camera.
At approximately 1248 hours, I was dispatched to an unknown-injury blocking vehicle collision at the
intersection of Southwest Grady Way and Lind Avenue South, within the city limits of Renton, King
County, Washington.
While en route, dispatch advised that three vehicles were involved and that one vehicle had rolled
over, with its occupant entrapped and requiring assistance to exit.
Upon arrival, at approximately 1250 hours, I observed that the driver of Unit#1 had already been
extracted from the vehicle via bystanders. The driver had a pulse, appeared conscious, but was
verbally unresponsive. It was unclear whether this was due to a language barrier or another issue.
Renton Regional Fire Authority (RRFA) arrived shortly after and assumed patient care. The driver of
Unit#1 was later transported to Valley Medical Center (VMC) for further evaluation and treatment.
I then checked on the occupants of the other involved vehicles. Several occupants in Unit#2
complained of injuries: the driver reported low back pain; the front passenger reported neck, back,
and right knee pain; the child in the rear left forward-facing car seat reported neck pain and sustained
a chest laceration from the seatbelt; the child in the rear middle seat reported head pain; and the right
rear passenger reported no injuries.
The driver of Unit#3 reported no injuries.
While on scene, I collected driving information and statements from each involved party, except the
driver of Unit#1, who remained verbally unresponsive and unable to provide a statement.
According to witness statements and dash camera footage, Unit#1 was occupied by a single male
driver. Prior to the collision, the vehicle was traveling westbound on SW Grady Way in the 400 block,
approaching the controlled intersection with Lind Avenue SW in lane 2 of 2. Unit#1 entered the
intersection against a red light and struck both Unit#2 and Unit#3. The impact caused Unit#1 to roll
over and sustain significant damage to its front and driver's side.
I reviewed the dash camera footage, which confirmed that the traffic control device showed a red light
for eastbound and westbound traffic on SW Grady Way. Northbound and southbound traffic on Lind
Avenue SW had begun to proceed. Unit#1, traveling westbound on SW Grady Way in lane 2 of 2, did
not slow down or stop and entered the intersection against the red light, even while uninvolved traffic
in the adjacent lane did stop. Unit#1 struck the rear passenger-side bed rail of Unit#2, causing
significant damage. The force caused Unit#1 to rotate, trip on its driver's-side tires, roll onto its
driver's side, and then onto its roof, where it came to rest in the intersection.
The driver of Unit#2 stated that he and his four occupants (two adults and two children) were
traveling northbound on Lind Avenue SW in the 1000 block, approaching the controlled intersection
with SW Grady Way in lane 2 of 3. He had a stale green light and intended to proceed straight (north)
through the intersection. As he continued northbound, Unit#1 collided with the passenger side of his
vehicle. Upon impact, Unit#2 rotated approximately 180 degrees clockwise. Debris from Unit#2's
truck bed (which had been enclosed and secured with a tonneau cover) sprayed onto the windshield
of Unit#3. Unit#2 came to rest facing southbound in the northbound lanes (lane 2 of 2). Unit#2
sustained significant damage to its passenger side. All occupants of Unit#2 were transported by
ambulance to VMC.
The driver of Unit#3 stated that she was the sole occupant and was traveling southbound on Lind
Avenue SW in the 1000 block, approaching the controlled intersection with SW Grady Way in the
dedicated left-turn lane. She had a flashing yellow light and intended to turn left onto SW Grady Way
eastbound. While stopped and yielding to oncoming traffic, she observed Unit#1 and Unit#2 collide
in the intersection. The initial damage to Unit#3 was caused by high-force debris that shattered her
windshield. She then saw Unit#1 roll over and collide with her front bumper before coming to rest.
Unit#3 sustained moderate damage to the front bumper and windshield.

NARRATIVE
A witness, identified by Officer R. Phillips as Chris A. Case (DOB 07/27/1988), provided front-facing
dash camera footage of the collision. Case offered to upload the video to the department's
evidence.com portal which has since been completed and where I was able to review the video for
this report.
I later visited the driver of Unit#1 at VMC, where he remained verbally unresponsive. A woman
identifying herself as Althea Dani, the driver's wife, stated that her husband, Ahmad Dani, had
suffered a stroke in the past. She was unsure if he had experienced another medical episode prior to
the collision. Althea indicated that Ahmad's primary language is English. Although further medical
evaluation was needed for confirmation, it appeared possible that the driver of Unit#1 suffered a
medical episode before the collision.
Based on the statements, witness accounts, and dash camera evidence, I determined that the driver
of Unit#1, Ahmad Dani, was the proximate cause of the collision. He violated RCW 46.61.055(3)(a)
by entering the intersection against a steady circular red signal. Vehicle operators facing a steady
circular red signal shall stop at a clearly marked stop line (or before entering the crosswalk or
intersection control area if none exists) and shall remain stopped until a proceed indication is shown.
I issued Dani a citation via mail for the violation.
All three involved units (Unit#1, Unit#2, and Unit#3) sustained disabling damage and required
towing. An exchange of information and applicable tow forms were provided to all parties.
