NARRATIVE
26-3606
This report is a summary of events that occurred and is not an exact sequencing of events.
Statements have been paraphrased and summarized.
On 05/10/2026 at approximately 1054 hours, myself, Officer R. Watson, was dispatched to a collision
with injuries, at the intersection of Ne 4th ST and Duvall AVE NE, within the City Limits of Renton,
County of King, State of Washington.
Dispatch notes stated that there were 2 vehicles and there were injuries.
I arrived on scene and observed two vehicles that had collided in the 400 block of Duvall AVE NE.
Unit# 1, driven by Anh B. Hong (DOB 11/25/1970) verified by WA DOL photo, was found in the
driver's seat of a Gray 2015 Toyota Sienna, WA plate DACYANG. There was a female passenger,
Kathy Duong (DOB 11/10/2012), the driver's daughter, who said that she was not injured just shook
up, and was sitting in the front right passenger seat. Anh was unable to provide me with her account
of the incident, however Kathy said that they pulled out from the QFC parking lot and unit#2 collided
with them. Both Anh and Kathy were assessed by Renton Fire Department medics and transported
from the scene by Trimed ambulance. I was able to obtain Anh's drivers license registration and
insurance information from her husband who arrived on scene. Unit#1 had significant front left side,
no airbag deployment, and was towed from the scene by Bankers.
Unit# 2, driven by Yury Chaikou (DOB 08/20/1979) verified by WA DOL photo, stated he was driving
a Red 2023 Ford Mustang Mach-E, WA plate KONYASH. There was one other passenger in the
vehicle, Stam Chaikou (DOB 07/30/2014), who was the drivers son, had a bloody nose, was
assessed by Renton Fire Department medics on scene and declined further treatment. Yury stated
that he was traveling northbound in the number 1 lane when unit#1 pulled in front of him from the
QFC parking lot. Yury was unable to stop in time and collided with unit#1. Yury said his chest was
tight and was assessed by Renton Fire Department medics on scene but declined further treatment.
When I asked for Yury `s license, registration and insurance, he was able to provide me with all
documents. Unit#2 had moderate front right-side damage, no airbag deployment, and was able to be
driven from the scene.
An independent witness who was unknown to both parties, Dennis Cortez (DOB 03/16/1984), stated
that he was traveling in the number 2 lane when he had to stop his vehicle because unit#1 had
pulled in front of him from the QFC parking lot and proceeded to pull into the number 1 lane which
caused a collision with unit#2.
An exchange of information was given to both parties involved. Unit#1 driver was cited for, vehicle
pulling onto roadway, due to the driver not providing the right of way when coming from a parking lot
to unit#2 who was traveling on the roadway (citation #6A0058587).
