EH02589 CASE# 26-3116 OFC LLI ION 04/22/26 09:38 OF COLLISION NARRATIVE Case 26-3116 1 was working as a fully uniformed City of Renton Police Officer in a fully marked City of Renton, Police Car as unit K939. All events took place in the City of Renton, County of King, and the State of Washington unless otherwise noted. This report is a summary of events that occurred and is not an exact sequencing of events. Statements have been paraphrased and summarized. At the time of this call my body worn camera was activated. On 4/22/2026, at 0940 hours, I was dispatched to the intersection of Harrington Ave NE and NE 12th ST for a two-car motor vehicle collision that had occurred. I arrived at the intersection and observed a red 2009, Ford Ranger (WA plate C58372V) parked behind a red 2011, Ford Focus (WA plate (CFV3840). Both vehicles where just west of the intersection and were pulled to the right side of NE 12th ST. I spoke to the reporting party and Unit 2 driver of the Ford Ranger. He was identified as Felipe Zarate Hernandez (DOB 08/23/1974). Felipe provided me with his driver's license, registration and proof of insurance. Felipe stated he was not injured and did not need medical aid. Felipe explained what he saw. He stated he came to the four way stop at Harrington Ave NE and NE 12th ST. Felipe saw the Ford Focus across the intersection also stopped. Felipe proceeded through the intersection and saw the Focus entering the intersection and turning left to go North on Harrington Ave NE. Felipe collided with the Focus. Felipe stated he did not see the Focus's turn signal on. I contacted the driver of Unit 1, the 2011 red Ford Focus. I identified the driver as Madison R Ehrman (DOB 07/18/2003). 1 saw that the front end of Madison's focus was damaged, and I saw that the hood was bent. I believed the damaged to be over the 1500 threshold. Madison stated she had driven up to the stop sign and saw Felipe's truck across the intersection. Madison waited a couple of seconds and entered the intersection at the same time as Felipe causing them to collide. Madison stated she was not injured and declined medical aid. Madison provided me with her driver's license and showed me her insurance on her phone. Felipe had the phone number of a third-party witness named Mark. I called Mark and he told me what he had witnessed. Mark did not see who arrived at the stop sign first and only saw that both the Ford Ranger and Ford Focus were stopped at the stop signs. Mark explained that he also does not remember seeing the Ford Focus's blinker on signifying for the left turn when he witnessed the accident. I took photos of the damage to both Felipe's and Madison's cars and later uploaded them to evidence.com. I provided Felipe and Madison with exchange of information sheets pointing out the case number and where to locate the other person's insurance policy number. Based on RCW 46.61.190 which states that motorists are to come to a complete stop at a stop sign and whoever is the first at the sign has the right way. I was unable to determine who had come to the stop sign first and thus could not determine who was at fault. I did notate that both the witness Mark, and Felipe did not see Madison's turn signal on.
