NARRATIVE
ASSIGNMENT/ARRIVAL
On 05/29/2026 at approximately 0909 hours, I was dispatched to a three-vehicle collision in the
intersection of 140th Way SE and 142nd Place SE in the city of Renton, King County, Washington.
I arrived at approximately 0912 hours.
UNIT 1: HONDA CR-V
Upon arrival I observed a silver 2013 Honda CR-V (WA/ BWH0683, Unit 1) stopped facing south in
the intersection of 140th Way SE and 142nd Place SE. Unit 1's front end was nearly in the
southbound number three lane of 140th Way SE near the north curb line prolongation of 142nd Place
SE.
Unit 1 had not been moved from its point of rest. Unit 1 had moderate front end damage from the
collision.
UNIT 2: INTERNATIONAL PUMP TRUCK
I observed a 2017 International pump truck (WA/ C27390E, USDOT 773975, Unit 2) stopped in the
same intersection facing west blocking the northbound lanes of 140th Way SE.
Unit 2 had not been moved from its point of rest. Unit 2 had minor damage to the front driver bumper.
UNIT 3: HONDA CIVIC
I observed a white 2020 Honda Civic (WA/ CTT9448, Unit 3) stopped in the southbound number
three lane of 140th Way SE just north of the intersection of 142nd Place SE.
Unit 3 had not been moved from its point of rest. Unit 3 had moderate front end damage from the
collision.
DRIVER 1: HUNG, TSUI-MIN
I contacted Tsui-Min Hung (DOB: 08/07/1968) standing near Unit 1. 1 positively identified Hung by her
valid Washington Driver License and established her as the uninjured driver of Unit 1 by her
admission.
DRIVER 2: RUASSAY, ANDREW T.
I contacted Andrew T. Ruassay (DOB: 12/07/1971) in Unit 2's driver seat. I positively identified
Ruassay by his valid Washington Driver License and established him as the uninjured driver of Unit 2
by his admission.
DRIVER 3: DE NOVA, KAREN L.
I contacted Karen L. De Nova (DOB: 09/11/1997) in Unit 3's driver seat. I positively identified De
Nova by her valid Washington Driver License and established her as the driver of Unti 3 by her
admission.
De Nova complained of pain to her neck. She was evaluated by members of the Renton Regional
Fire Authority and transported by Trilled ambulance to Valley Medical Center.
STATEMENT: DRIVER 1
1 spoke with Hung about the collision. The following is a summary of our conversation:
Hung drove Unit 1 north on 140th Way SE toward 142nd Place SE. As Hung crested the hill, the
traffic signal at the intersection changed from green to red. Hung applied Unit 1's brakes but was not
able to stop due to the downhill grade and wet road conditions.
Unit 1 entered the intersection on a red traffic signal and struck Unit 2's passenger side, which
entered the intersection westbound on 142nd Place SE.
This concluded my conversation with Hung.

NARRATIVE
I spoke with Ruassay about the collision. The following is a summary of our conversation:
Ruassay drove Unit 2 west on 142nd Place SE into the intersection of 140th Way SE to turn south
(left) onto 140th Way SE on a green traffic signal. Ruassay saw Unit 1 driving north on 140th Way SE
toward him, fail to stop at the intersection, collide with Unit 2's front-driver side, then collide with Unit
3 in the southbound lane of 140th Way SE.
This concluded my conversation with Ruassay.
STATEMENT: DRIVER 3
1 spoke with De Nova about the collision. The following is a summary of our conversation:
De Nova drove Unit 3 south on 140th Way SE in the number three lane and stopped for a red traffic
signal at the intersection of 142nd Place SE. Unit 3 was the first vehicle at the intersection.
De Nova saw Unit 2 drive west on 142nd Place SE into the intersection, and saw Unit 1 approaching
from the north on 140th Way SE>
Unit 1 collided with Unit 2's driver side, then collided head-on with Unit 3, which was stopped at the
intersection.
This concluded my conversation with De Nova.
VIDEO FOOTAGE
Ruassay's supervisor arrived and showed me dash camera footage of the incident captured by Unit
2. The footage showed Unit 2 entering the intersection on a green traffic signal and Unit 1 driving
north into the intersection, colliding with Unit 2.
PROXIMATE CAUSE
The proximate cause of the collision was determined by damage observed to all involved vehicles
and statements from all involved drivers.
Hung's actions were the proximate cause of the collision, namely her violation of RCW 46.61.050(1),
Failure to Obey a Traffic Control Device, with a contributing factor of RCW 46.61.400(a), Speed too
Fast for Conditions.
CITATION
I issued Hung a citation (#6A0174096) for her violations of RCW 46.61.050(1) and RCW 46.61.400(a)
and filed it with the Renton Municipal Prosecutor's Office.
DISPOSITION
This concluded my involvement in this case.
