ASSIGNMENT/ARRIVAL
On 05/29/2026 at approximately 0807 hours, I was dispatched to a two-vehicle collision in the
15400 block of Maple Valley Highway in the city of Renton, King County,Washington.
I arrived at approximately 0819 hours. Officer K. Petty arrived prior and had the vehicles moved
off the road into a parking lot.
UNIT 1: FORD FUSION
Upon arrival I observed a silver 2013 Ford Fusion (WA/ATG4632, Unit 1) parked in a parking
stall. Unit 1 had been moved from its point of rest prior to my arrival. I did not see any damage to
Unit 1.
UNIT 2:TOYOTA SIENNA
I observed a white 2024 Toyota Sienna (WA/CNT4378, Unit 2) parked in a separate nearby
parking stall. Unit 2 had been moved from its point of rest prior to my arrival. I did not see any
damage to Unit 2 but the driver said the rear hatch now had problems opening and might be
stuck.
DRIVER 1: GEORGEWITZ,JOHN H.
I contacted John H. Georgewitz(DOB: 09/17/1984) laying on the ground near Unit 1. 1 positively
identified Georgewitz by his valid Washington Driver License and established him as the driver of
Unit 1 by his admission.
Georgewitz believed he was having a panic attack, so members of the Renton Regional Fire
Authority arrived to evaluate him. Georgewitz was not injured in the collision.
This incident occurred in the sCity of Renton,King County,Washington State.

DRIVER 2: FERDOSIAN NAJAFABADI PEJMAN
I contacted Pejman Ferdosian Najafabadi (DOB: 09/16/1976) standing near Unit 2. 1 positively
identified him by his valid Washington Driver License and established him as the driver of Unit 2
by his admission. Pejman was not injured in the collision.
STATEMENT: DRIVER 1
1 spoke with Georgewitz about the collision. The following is a summary of our conversation:
Georgewitz drove Unit 1 west on Maple Valley Highway toward 154th Place SE in the number two
Lane behind Unit 2. Unit 1 and Unit 2 were stopped for traffic. Georgewitz saw vehicles in lane
one accelerating, so he slowly accelerated Unit 2 not knowing his lane was not yet proceeding.
Unit 2's front bumper collided with Unit 1's rear bumper.
This concluded my conversation with Georgewitz.
PROXIMATE CAUSE
I determined the proximate cause of the collision by Georgewitz's statement.
Georgewitz's action was the proximate cause of the collision, namely his violation of RCW
46.61.400 (1), Basic Speed.
DISPOSITION
Both vehicles were driven from the scene.
This incident occurred in the sCity of Renton,King County,Washington State.
