DLState DLExp Address Primary Phone&Type AdditionalContact Number&Type Additional Contact Number&Type Race Sex DOB- Height Weight Even Hair Collision 2026-00004148 Paqe 2 OF 7 Case Number 2026-00004148 Officer Narrative Narrative ASSIGNMENT/ARRIVAL On 05/29/2026 at approximately 1410 hours, I was dispatched to a hit and run collision near the intersection of SW 7th Street and Oakesdale Avenue SW in the city of Renton, King County, Washington. The victim followed the suspect's vehicle to the parking lot of the Creston Pointe Apartments, 13445 Martin Luther King Jr Way S in unincorporated Seattle. I arrived at approximately 1422 hours. UNIT 1: BUICK RENDEZVOUS Upon arrival I observed a black Buick Rendezvous (WA/ BIU0266, Unit 1) parked in a parking stall near the T-Building. I observed fresh, white paint transfer along the rear-driver door. UNIT 2: HONDA HRV I observed a white 2021 Honda HR-V(VIN /3CZRU6H55MM739845, Unit 2) parked in a stall near Unit 1. Unit 2 had black, plastic transfer along the front-passenger bumper. Unit 2 was a vehicle offered for sale in Toyota of Renton's used inventory.The driver was on a test drive at the time of the collision. DRIVER 1: HOWARD JANIS M. I contacted Janis M. Howard (DOB: 05/04/1967,verified by WA DOL records) standing near the T-Building and Unit 1. 1 established Howard as the driver of Unit 1 by her admission. Howard was not injured. Officer: Gorajewski, Raymond #R12399 Reviewed By: Gorajewski, Raymond W Date/Time: 05/29/2026 03:28 Collision 2026-00004148 Paqe 3 OF 7 Case Number 2026-00004148 Officer Narrative-Continued DRIVER 2: GOLICZ MARCIN I contacted Marcin Golicz(DOB: 01/24/1978) near Unit 2. 1 positively identified Marcin by his valid Washington Driver License and established him as the driver of Unit 2 by his admission. Marcin was not injured. PASSENGERS Kathryn M. Golicz(DOB: 03/31/1983)was positively identified and established as Unit 2's rear-driver passenger. Bronislawa R. Golicz (DOB: 01/29/1954)was positively identified and established as Unit 2's front passenger. They were uninjured. STATEMENT: DRIVER 2 1 spoke with Marcin about the collision. The following is a summary of our conversation: Marcin drove Unit 2 on a city street somewhere very near the intersection of SW 7th Street and Oakesdale Avenue SW. Marcin couldn't tell me exactly where he was because he was unfamiliar with the road. Unit 1 merged from the number one to the number two lane in front of him, with Unit 1's driver side swiping Unit 2's front-passenger bumper as it merged. Marcin quickly took a photograph when Unit 1 briefly stopped and both vehicles were still engaged at their point of rest. Howard did not pull over, but drove, presumably based on their destination, north on Oakesdale Avenue SW, north on Monster Road SW,then east on Martin Luther King Jr.Way S. to Creston Pointe Apartments where they ended up. Marcin followed Unit 1 until she stopped in the parking lot of Creston Pointe Apartments. This concluded my conversation with Marcin. STATEMENT: DRIVER 1 1 spoke with Howard about the incident. I did not tell Howard she was under arrest or detained, I did not search or pat her for weapons, I did not handcuff her or direct/restrict her movement or freedom in anyway. The following is a summary of our conversation: Howard admitted to being in a collision with Marcin in Unit 2, but said she told Howard to "follow me" and drove to Creston Pointe Apartments, despite there being many suitable areas to safely pull over and exchange information. This concluded my conversation with Howard. PHOTOGRAPHS I took photographs of both vehicles with my department issued cell phone and uploaded them to Axon's Evidence.com. I sent Marcin an Axon Community Request link for the photographs he took. I have yet to received them. Officer: Gorajewski, Raymond #R12399 Reviewed By: Gorajewski, Raymond W Date/Time: 05/29/2026 03:28 Collision 2026-00004148 Paqe 5 OF 7 Case Number 2026-00004148 Officer Narrative-Continued REGISTERED OWNER CONTACT I contacted Tony Pho (tpho@toyotaofrenton.com), a General Manager at Toyota of Renton,to see if he wanted to pursue charges for hit and run. Pho said he did wish to pursue charges. PROXIMATE CAUSE The proximate cause of the collision was determined by photographs taken by Marcin I observed, damage to both vehicles, and my conversation with Marcin. Howard's action was the proximate cause of the collision, namely her violation of 46.61.140(1), by failing to drive vehicle nearly as practicable entirely within a single lane and moving it from such lane without first ascertaining that such movement can be made with safety. PROBABLE CAUSE Based on Marcin's statement that Howard was the driver of a vehicle involved in a collision that fled the scene of a collision and failed to meet the requirements in RCW 46.52.020(3), 1 have probable cause to arrest Howard for Hit and Run Attended, RCW 46.52.020(2)(a). Based on the fact a Washington State Department of Licensing check of Howard's driving privilege status revealed she was unlicensed, coupled with the fact she had no identification on her meeting the requirements of RCW 46.20.035, 1 have probable cause to arrest her for RCW 46.20.005. I issued Howard a citation (#6A0174098)for Operating a Motor Vehicle without Insurance, RCW 46.30.020, and Driving on Roadways Laned for Traffic, RCW 46.61.140(1), and filed it with the Renton Municipal Court. DISPOSITION Howard was released from the scene. This concluded my involvement in this case.
