Narrative
ASSIGNMENT/ARRIVAL
On 06/11/2026 at approximately 1016 hours, I was dispatched to a two vehicle collision that
occurred on Maple Valley Highway and the northbound 1-405 off ramp in the city of Renton, King
County, Washington.
Both drivers moved to Bronson Way N and Meadow Avenue N for contact.
I arrived at approximately 1020 hours.
UNIT 1:TESLA MODEL Y
Upon arrival I observed a black Tesla Model Y(WA/CPA5077, Unit 1) stopped along the east
curb of Meadow Avenue N north of Bronson Way N. Unit 1 had been moved from its point of rest
prior to my arrival. Unit 1 had minor damage (scratches and scrapes) along the front driver
bumper.
UNIT 2: MAZDA CX-5
I observed a black Mazda CX-5 (WA/CTX1798, Unit 2) stopped in front of Unit 1. Unit 2 had been
moved from its point of rest prior to my arrival. Unit 2 had minor damage (scratches and
scrapes) along the rear passenger side.
DRIVER 1: HUA, KIET V.
I contacted Kiet V. Hua (DOB: 10/18/1961) standing near Unit 1. 1 positively identified Hua by his
valid Washington Driver License and established him as the uninjured driver of Unit 1 by his
admission. Hua was working as an Uber driver at the time of the collision.
This incident occurred in the sCity of Renton,King County,Washington State.

DRIVER 2:WHITAKER CHRISTIE L. M.
I contacted Christie L. M.Whitaker (DOB: 11/25/1971) standing near Unit 2. 1 positively identified
Whitaker by her valid Washington Driver License and established her as the uninjured driver of
Unit 2 by her admission.
PASSENGER
Hua said there was a passenger(Uber customer) in Unit 1 at the time of the collision, but they
Left prior to my arrival. Hua couldn't figure out how to pull up the passenger's information.The
passenger remains unidentified at this time.
STATEMENT: HUA, KIET V.
I spoke with Hua about the collision. Hua couldn't accurately describe what road he was on,
which was he was driving, or where he was going.
Hua did not know how to access his vehicle's dash camera (factory Tesla recording equipment).
STATEMENT:WHITAKER CHRISTIE L. M.
I spoke with Whitaker about the collision.The following is a summary of our conversation:
Whitaker drove Unit 2 west on Maple Valley Highway toward the northbound 1-405 off ramp. Unit
1 entered westbound Maple Valley Highway from the northbound 1-405 off ramp and collided
with Unit 2.
Whitaker couldn't tell me the conditions of the traffic signals at the controlled intersection, and
seemed unaware it was a controlled intersection.
This incident occurred in the sCity of Renton,King County,Washington State.

This concluded my conversation with Whitaker.
PROXIMATE CAUSE
I was unable to determine a proximate cause of the collision based on observed damage to both
vehicles and driver statements.
DISPOSITION
This concluded my involvement in this case.
