EH13841 CASE# 26-4537 OF COLLISION06/11/26 11:10 NARRATIVE ASSIGNMENT/ARRIVAL On 06/11/2026 at approximately 1110 hours, I was dispatched to a two vehicle collision in the intersection of Maple Valley Highway and Cedar River Park Drive in the city of Renton, King County, Washington. I arrived at approximately 1113 hours. UNIT 1: HONDA CR-V Upon arrival I observed a silver Honda CR-V (WA/ BYH2776, Unit 1) in the intersection of Maple Valley Highway and Cedar River Park Drive. I saw a bystander directing traffic as Unit 1 was moved from its point of rest to the parking lot of 1715 Maple Valley Highway. I was unable to clearly see the point of rest before Unit 1 was moved. Unit 1 had moderate damage to the rear-driver door. UNIT 2: SUBARU ASCENT I observed a blue Subaru Ascent (WA/ CVC0002, Unit 2) stopped facing east along the south curb of Maple Valley Highway east of Cedar River Park Drive. Unit 2 had minor damage to the front- passenger side. Unit 2 was moved from its point of rest prior to my arrival. DRIVER 1: TRICHAK, JOYCE S. I contacted Joyce S. Trichak (DOB: 10/24/1948) near Unit 1. 1 positively identified her by her valid Washington Driver License and established her as the uninjured driver of Unit 1. DRIVER 2: BARRETO DELA CRUZ, KATHERYN G. I contacted Katheryn G. Barreto Dela Cruz (DOB: 04/21/1997) near the intersection of Maple Valley Highway and Cedar River Park Drive. I positively identified her by her valid Washington Driver License and established her as the uninjured driver of Unit 2. Barreto had a license restriction of driving with ignition interlock. Unit 2 was not equipped with an ignition interlock device. PASSENGERS Casandra M. Trichak (DOB: 01/05/1972) was identified as the uninjured front passenger of Unit 1. Santiago Sonalo-Barreto (DOB: 11/18/2022) and Luca Solano-Barreto (DOB: 05/07/2024) were identified as the uninjured front passengers of Unit 2. STATEMENT: TRICHAK, JOYCE S. I spoke with Trichak about the collision. The following is a summary of our conversation: Trichak said she drove Unit 1 west on Maple Valley Highway and Unit 2 struck her driver side near the intersection of Cedar River Park Drive. Trichak confirmed this story multiple times but had no other details. This concluded my conversation with Trichak. STATEMENT: BARRETO DELA CRUZ, KATHERYN G. I spoke with Barreto about the collision. The following is a summary of our conversation: Barreto drove Unit 2 east on Maple Valley Highway toward Cedar River Park Drive in the number two lane. Unit 1 drove beside Unit 2 in the number one lane. For an unknown reason, Unit 2 accelerated and "did a 90 degree turn" in front of Unit 1. Unit 1's front-passenger bumper collided with Unit 2's rear-driver door. This concluded my conversation with Barreto. PROXIMATE CAUSE Due to the lack of roadway evidence, conflicting stories from both drivers, and not observing the PAGE 3 OF 5 TIME REPORT NO. EH13841 CASE# 26-4537 OF COLLISION06/11/26 11:10 NARRATIVE vehicles' points of rest, I was unable to determine the proximate cause of the collision. The diagram attached to this collision report is representative of what I believe occurred based on Barreto's statement and damage observed to both vehicles.
