Description of Change: This change order adds pay item CO1A52 “Intersection Revision” to the contract and authorizes additional work required to meet PROWAG requirements at the SW 20th St Intersection with Oakesdale Avenue. a. Proposal Schedule of Prices: Item No. Spec. Section Description Qty Unit Unit Price Amount CO1A52 8-14, 8-20 Intersection Revision 1 LS $64,777.86 $64,777.86 æϟ Specification and/or Special Provision: Ėϟ Description: The contractor shall demo 2 existing curb ramps and 6ft of sidewalk(WSDOT Spec 8-14) indicated on the attached plans and construct 2 parallelϙ ramps (WSDOT Spec 8-14) and install a new TA 25 vault to house an existingϙfiber optic line(WSDOT Spec 9-29.3). The contractor shall also remove and salvage 2ϙexisting pedestrian signal heads and 2 push button devices as called out in the plansϙand install 1 new pedestrian signal heads and 1 new pedestrian push button devices (WSDOT Spec 9-29). The work described herein and all ancillary supporting activitiesϙ shall be completed in accordance with the revised design details provided in theϙ accompanying CCD. ĖĖϟ Construction Requirements: The contractor shall adhere the specs outlined aboveϙin the description as well as the scope in Construction Change Directive CCD 001 (Seeϙattached). ĖĖĖϟ Materials: 1 PS Pole and curb base foundation, 1 pedestrian signal head, 2 APSϙ PPBs (right arrow), 1 PPB Extender, Conductors between new and existing equipment, 1ϙ iCCU to install inside the existing cabinet, 1 TA-25 Vault. ĖŽϟ Measurement: Lump Sum for all work not covered by unit items. Žϟ Payment: The payment for “Intersection Revision” includes all labor, materials,ϙ tools, mobilization(s) and equipment required for performing the above listed activitiesϙ that are not already described by unit items. èϟ Plans: revised plan sheets 1, 11, 23, 27A, 34, 37, 39, 40, 41, 41A, 41B 2. Reason for Change: The contract plans called out for paving over an existing crosswalk who’s adjacent ramps were not in conformance with the latest PROWAG requirements. Equitable Adjustment: This Change Order does include an equitable adjustment per Section 1-09.4 of the 2025 WSDOT/APWA Standard Specifications. See attached. Critical Path Extension of Time e: The Time for Completion is not extended any Working Days. 5. Sections 1-04.4 and 1-04.5 of the 2025 WSDOT/APWA Standard Specifications shall govern this Change Order. The Work of the referenced Contract is modified to include the changes detailed herein.
