Renton Case#26-4956
On 06/24/2026 at 2211 hours I was dispatched to the 1800 block of Maple Valley HWY, in the City of Renton,
King County, Washington. I was responded to the call of a recent non-injury, blocking, motor vehicle collision.
This incident was captured on my body worn video camera.This report is a summation of events that
occurred and is not an exact sequence of events.
While enroute, dispatch advised the following: NOW, EAST OF LOC,2 VEH MVA, NO INJS, BLKING
ROADWAY. RP ADVISED OTHER DRIVER SMELLS OF ALCOHOL, BLK FORD TAURUS @CVW5191/RP
IS IN A TAN TOYOTA AVALON @CWF0423/HAZARD LIGHTS WILL BE ON.
Upon arrival, I contacted both involved parties who were blocking the center lane of westbound lanes of
travel.
I identified Unit 1 as a black 2011 Ford Taurus(WA LIC#CVW5191) being driven by Perez L
Samuel-Orlando DOB 04/14/1988 (verified via WA DL).
I identified Unit 2 as a gold 2004 Toyota Avalon (WA LIC#CWF0423, being driven by Andrew J Williams
DOB 09/10/1989(verified via WA DL).
Unit 2 driver stated he was uninjured and provided me with his WA DL, registration and insurance.
Unit 1 driver stated that he did not have a valid license though he provided me with his WA ID. Unit 1
driver stated he did not have vehicle insurance.
In speaking with Unit 2 driver, he advised he was traveling westbound on Maple Valley HWY and stopped
at a red light,just east of 1-405.While stopped for traffic, Unit 2 driver said that Unit 1 vehicle approached
him from behind, and as a result of not stopping in time, collided with the rear end of Unit 2 vehicle.
I observed very minor damage to both Unit 1 and Unit 2 vehicles that was consistent with the version of
events as described by Unit 2 driver.
During my time on scene, and while speaking with Unit 1 driver, I noted his eyes were extremely bloodshot,
watery and could smell the odor of intoxicants emitting from his person. Furthermore,while Unit 1 driver was

standing in the roadway, he appeared unsteady on his feet and appeared to be slightly swaying. When asked if
he had been drinking tonight, Unit 1 driver told me that he only had two beers.
Ofc.Thompson arrived on scene shortly after my initial arrival and conducted a DUI investigation.As a result
of his investigation, Unit 1 driver was placed under arrest for DUI. Please see his report for further details.
With Unit 1 driver in custody for DUI, Bankers Towing was dispatched and removed Unit 1 vehicle from the
scene.
I then cleared the scene.
This ends my report.
