EH22563 CASE# 26-4759 OF COLLISION 06/18/26 16:06 OF CbLLI510N NARRATIVE On June 21, 2026, at approximately 1606 hours, I was dispatched to a non-blocking collision at the intersection of Sunset Blvd N and N 4th St, within the City Limits of Renton, County of King, State of Washington. Upon my arrival, I confirmed there were no complaints of injury requiring immediate medical response at the time of report. There, I was able to collect each involved party's information and independent summary of the events leading to the collision. The driver of Unit 1, identified as Douglas A. Fenty, said he was traveling southbound on Sunset Blvd N and approaching the intersection of N 3rd St in lane 2 of 3. Doughlas stated that he needed to get into the left lane (lane 3 of 3). Douglas stated that Unit 2 had created a gap in traffic allowing vehicles to merge, so he believed he could proceed left and make his lane change. While making the lane change, Unit 1 and Unit 2 collided in lane 3 of 3 causing moderate damage to the driver's door of Unit 1. Douglas said that most of his vehicle was already in lane when the collision occurred. He insinuated that Unit#2 purposely sped up in their lane, causing the collision. The driver of Unit 2, identified as Isaac Hoffman, said he was the sole occupant of his vehicle and was also traveling southbound on Sunset Blvd N and approaching the intersection of N 3rd St in lane 3 of 3. Isaac confirmed that he created a gap in traffic prior to the collision. Issac stated he was intending to continue straight when Unit 1 changed lanes from lane 2 of 3 and into lane 3 of 3 which Unit 2 was occupying. Issac said he was unable to avoid the collision and Unit 1 subsequently collided with Unit 2. Unit 2 sustained minor scuffing to his right front tire. Based on the above statements, I determined that the Driver of Unit 1 (Douglas) is the proximate cause for the cause of collision as Douglas violated RCW 46.61.140(1) which states that a vehicle shall be driven as nearly as practicable entirely within a single lane and shall not be moved from such lane until the driver has first ascertained that such movement can be made with safety. Douglas made a lane change into Unit 2's lane of travel which had the right of way and was underway. Douglas did not ensure that traffic in lane 3 of 3 was clear of hazard before making the unsafe lane change. I cited Douglas via mail for Improper Lane Usage causing a collision. Both Unit 1 and Unit 2 were drivable, so no tows were required. An exchange of information was provided to all involved parties. I certify (or declare) under penalty of perjury under the laws of the State of Washington that the foregoing is true and correct. Electronically signed by Officer C.
