self-funded health insurance plan and ancillary employee benefit plans. This document includes brief

About our Plans
The City is pleased to provide a comprehensive and affordable benefits program to 550 eligible
employees working 20 or more hours a week. The City offers a self-funded medical, prescription,
dental, and vision plan, currently administered through Health Management Administrators (HMA) and
Envision Rx. The individual stop loss is currently set at $250,000. The Plan is "grandfathered" under
the Affordable Care Act. The health plan has no deductible, a $30 copay for office visits with
comparable copays for other services, and out of pocket maximum of$500/Individual and
$875/Family. The plan uses the Regence network, with a small cost difference to the member for using
out-of-network providers. The plan has been self-funded for over 25 years, and is run on a calendar
year. Employees may choose to participate in Kaiser Permanente as an alternative to the self-funded
plan. Less than 5% of employees currently elect Kaiser.

Spending Accounts currently through Benefit Administration Company, and an Employee Assistance
Program currently through First Choice. The health plan is governed by the Renton Employee Health
