Description of Work
Management Reserve request for additional surveying services and utility locates.
The Local Agency of City of Renton
desires to supplement the agreement entered in to with KPG,P.S.
and executed on 10/23/2015 and identified as Agreement No, CAG-15-089
All provisions in the basic agreement remain in effect except as expressly modified by this supplement.
The changes to the agreement are described as follows:
Section 1, SCOPE OF WORK, is hereby changed to read:
The General Scope of Work of the Original Agreement and Supplemental Agreements No. 1,2,3,and 4 are supplemented to
include the attached Exhibit 6.
II
Section IV,TIME FOR BEGINNING AND COMPLETION, is amended to change the number of calendar days
for completion of the work to read: No Change.
Ill
Section V, PAYMENT, shall be amended as follows:
See attached Exhibits"A", I,and D.

AGENCY: CITY OF RENTON

PROJECT: RAINIER AVE S-S 3rd St to NW 3rd PI
The Consultant is requesting that the Agency authorize the use of the Management
Reserve Fund for additional utility locating and surveying services to be rendered for the
Rainier Ave S-S 3rd St to NW 3rd PI Project (CAG 15-089).
A fee estimate labeled as Exhibit D is attached to this request that outlines the costs for
each service to be rendered with a "Not To Exceed" amount of$13,300. A summary of
the current management reserve budget and authorized amounts are the following:
1) Unauthorized Management Reserve:$57,660(Supplement 3)
2) Management Reserve Request#3: $13,300
3) Remaining Unauthorized Management Reserve: $44,360
SCOPE OF WORK
TASK M1—Additional Surveying
• The budget shown in Exhibit D assumes this task is one (1) additional month in
duration.
The Consultant will provide additional surveying and basemapping services in the areas
shown in the Survey Limits Exhibit. Specific surveying activities are as follows:
1.1 Additional field survey within the project limits identified in the Survey Limits
exhibit.
• Field survey of planimetric features, utilities, and mapping of the existing ground
will be taken at approximately at 25-ft intervals (to 2-ft. contour intervals).
• Locate paint marks, including surface features (valves, manholes, catch basins,
junction boxes, vaults, etc.). Perform observation and measure-downs of existing

storm drain catch basins and manholes. The approximate size, type (brick,
concrete), and general condition of the structures to confirm suitability for
continued use, and approximate size and location of pipes will be documented.
These observations will be made from the surface.
• The Consultant shall coordinate utility locates, identification, survey, and other
work, as necessary, to locate all other franchise and private underground utilities
in order to identify any potential conflicts with the proposed improvements. The
Consultant will utilize a 3`d party locating service (CNI)to paint the utilities.
1.2 CADD mapping work to update 1"=20'topographic basemap and DTM in AutoCAD
format of the project within the areas described above.
