Creston Backbone 288 count SM (FCB288) C. Project Description Provide a fiber optic network connection from Seattle Public Utilities Fairwood Control Works (FCW)to the S. Creston Substation (SCS) for Seattle Public Utilities (SPU) and Seattle City Light (SCL). Extend the University of Washington (UW) fiber optic ring from the S. Creston Substation to downtown Renton(approximately 5 miles). Provide backbone fiber optic cable adjacent to Renton High School (RHS), Black River High School (BRHS), Dimmit Middle School (DMS),Nelsen Middle School (NMS), Campbell Hill Elementary School (CHES), Talbot Hill Elementary School (THES), and Tiffany Park Elementary School (TPES) and splice into to the existing backbone to the Kohlwes Education Center(KEC). Subsequent projects will provide home run connections for the above schools and 8 other schools that connect through DMS and NMS. Install a 288 count single mode (SM) fiber backbone cable from the FCW master distribution frame (MDF) to the vault on the NW corner of S Grady Way and Wells Ave. S. Install a 288 count SM fiber cable from the vault on the NW corner of S Grady Way and Wells Ave. S to a new vault near S 126th St. and 84th Ave. S. Install a 288 count SM fiber cable from the new vault near S 126th St. and 84th Ave. SE to an existing aerial fiber near S Leo St. and 51st Ave. S. E. Project Route Segment A: Southeast FCB288 (SE-FCB288) From the FCW MDF through approximately 1,500' of existing building entrance conduit to a SCL pole near the facility entrance on 132nd Place SE, transition to aerial through a new pole riser and follow the existing SCL pole line NW then west (entering the City of Renton) along the SPU pipeline right-of-way approximately 13,000' to Talbot Rd. S and S 26th St., then north along the PSE pole line on Talbot Rd. S approximately 2,700' to Benson Drive S., down a new 3 pole riser connecting to existing City of Renton Traffic telemetry conduit on the west side of Talbot Rd. S (Benson Drive S. becomes Talbot Rd. S at this location), through the telemetry conduit on the west side of Talbot Rd. S approximately 2,700' to the NE corner of Talbot Rd. S and S Grady Way, into an existing 2" City of Renton conduit on the north side of S Grady Way, east approximately 1,000' to the NW corner of S Grady Way and Wells Ave. S into an existing 25-TA splice vault. Segment B: Central FCB288 (C-FCB288) From splice vault on the NE corner of Grady Way S and Wells Ave. S north approximately 1,700' along the west side of Wells Ave. S to the SE corner of S 3r St., loop through an existing, adjacent 25-TA vault, continue west on the south side of S 3rd through traffic telemetry conduit approximately 1,900' to the SE corner of S 3rd St. and Shattuck Ave. S. where a new 25-TA will be installed to interconnect the traffic telemetry conduit system with an existing 3" Airport conduit system. North from S. 3rd on the east side of Shattuck Ave. S approximately 650' to the north side of S 2nd St., then west on the north side of S. 2° St. approximately 650' to the NW corner of S 2nd St. and Lake Ave. S, then north on the west side of Lake Ave. S approximately 1,400' to the north side of Airport Way, loop through an existing, adjacent 2.5 X 2.5 splice vault labeled "Electrical"and continue west on the north side of Airport Way approximately 400' to the NE corner of Airport Way and Rainier Ave. S, turn north through the park area then along and beyond the east edge of the parking area then jog east down the embankment turning north to parallel West Perimeter Rd. a total of approximately 2,400' to the West Hill Pump Station#1. An existing 2" conduit interconnects the airport 3" conduit system to the Water Department West Hill telemetry conduit. Continue west from the West Hill Pump Station approximately 1,300' crossing Rainier Ave. N, through the Rainier Station shopping area, up the hill to the north side of NW 6th St., continuing along the north side of the NW 6th St. to the NE corner of NW 6th St. and Taylor Pl. NW, then turn north on the east side of Taylor Pl. NE approximately 250' then sweep west onto the NW corner of S 124th St. and 87th Ave. S, while entering King County. Continue on the north side of S 124th St. approximately 1,200' to a 25-TA located IN the westbound lane of S 124th St.just west of 84th Ave S, continue south on the west side of 84th Ave. S approximately 700' to the NW corner of 84th Ave. S and S 126th St., turn west on the north side of S 126th St. approximately 150' to a point adjacent to the SCL pole where a new 25-TA vault will be installed adjacent to the conduit and a new pole riser will be installed. Segment C: Northwest FCB288 (NW-FCB288) From the new 25-TA vault up the SCL pole through a new pole riser then east on a new span approximately 200' to an existing aerial pathway on the west side of .84th Ave. S, south on 84th Ave. S approximately 700' to the SE corner of 84th Ave. S and S 128th St.,west approximately 5,700' on the south side of S 128th St. to the SW corner of S 128th St. and Beacon Ave. S, northwest on Beacon Ave. S approximately 7,500' following the primary pole route to the NE corner of 4 Beacon Ave. S / 51st Ave S and S Leo St. where there is an existing City of Seattle 144 count single mode fiber. B. Points of Demarcation Fairwood Control Works MDF Existing 25-TA on NW corner of Grady Way S and Wells Ave. S Existing 25-TA on SW corner of S 3rd St. and Wells Ave. S New 25-TA at S 126th St. 150' west of 84th Ave S Aerial splice point on NE corner of Beacon Ave. S and S Leo St. C. Facilities to be Used, Owners and Managers New aerial conduit system installed by City of Seattle, DoIT, on Seattle City Light poles and on Puget Sound Energy poles. City of Renton conduit system D. Project Specifications and Scope of Work This project shall meet all the requirements of the City of Renton Public Works Department and the King County Public Works Department, i.e. traffic control plans, street use permits, waiver of existing underground ordinance, Renton Business License, etc. • Route Preparation • Work to be done during normal working hours: Segment A: Southeast FCB288 (SE-FCB288) 1. At the FCW adjacent to the SCL pole near the facility entrance on 132nd Place SE,provide and install a 2" schedule 80 PVC conduit in a 36"deep trench from the hand hole at the end of the building entrance conduit approximately 10 feet to the adjacent SCL pole. Sweep the conduit into a pole riser ending 3 feet below the cable attachment. Bed the underground conduit in 6" of rock free fill and backfill the trench. Restore all adjacent landscaping to the condition that existed prior to the start of work. 2. Inventory the SCL poles on the route from the SCL pole near the FCW facility entrance on 132nd Place SE to Talbot Rd. S and S 26th St. Inventory the PSE poles from Talbot Rd. S and S 26th St. to Benson Drive S. Obtain the required permits and permissions, make-ready any existing strands as needed, provide and install a new aerial cable strand with required support equipment according to PSE and SCL specifications and provide and lash a 1" aerial conduit to the strand. 3. Locate the existing 1 %2"rigid galvanized steel (RGS) conduit adjacent to the PSE pole nearest the west side of Talbot Rd. S approximately 160' south of the intersection of Talbot Rd. S and S Puget Drive. Provide and install a new 25-TA vault over the conduit adjacent to the pole. Cut and sweep the 5 %roe 140 telemetry conduit into the new hand hole. Preserve the existing 6 strand copper telemetry cable in the conduit. 4. Provide and install a 2" schedule 80 PVC conduit in a 36" deep trench from the new hand hole to the adjacent PSE pole. Sweep the conduit into a pole riser ending 3 feet below the cable attachment. Bed the underground conduit in 6" of rock free fill and backfill the trench. Restore all adjacent landscaping to the condition that existed prior to the start of work. 5. Provide and install approximately 2,700' of pull-rope into the pole riser and through the conduit pathway to the NE corner of S Grady Way and Talbot Rd. S. Segment B: Central FCB288 (C-FCB288) 1. At the SE corner of S 3rd St. and Shattuck Ave. S. provide and install a new 25-TA splice vault. Provide and install 3" schedule 80 PVC conduits to the adjacent traffic signal type III hand hole and to the type I hand hole for the existing 3"Airport conduit system located approximately 15' south of the corner marked "LT". Replace the type I hand hole on the airport conduit. 2. Provide the vault with a non-skid lid with welded raised lettering that reads: "LT/ ITS". Replace sidewalk panels with 3,000 PSI concrete and finish to city specifications. 3. Provide and install approximately 1,900' of pull rope in the traffic signal telemetry conduit on the south side of S 3rd St. between Wells Ave. S and Shattuck Ave. S. 4. Provide and install approximately 1,700' of pull-rope. Remove the existing 24 count single mode cable from the 2" conduit on the west side of Wells Ave. S between Grady Way S and S 3rd St. using the cable to pull-in the pull-rope. Temporarily store the fiber cable in the 25-TA on the SW corner of S 3r St. and Wells Ave. S. Segment C: Northwest FCB288 (NW-FCB288) 1. Inventory the SCL poles on the route from the SCL pole on 84th Ave. S just north of S 126th St. to the NE corner of Beacon Ave. S / 51St Ave S and S Leo St., obtain the required permits and permissions, make-ready any existing strands as needed, provide and install a new aerial cable strand with required support equipment according to SCL specifications and provide and lash a 1" aerial conduit to the strand. 2. Provide and install a new aerial strand from the SCL pole on the north side of S 126th St approximately 150' west of 84th Ave. S. to the new aerial strand on the west side of 84th Ave. S, intercept the span at approximately the midpoint between the poles to align the aerial pathway with S 126th St. and lash a 1" aerial conduit to the strand. 3. Locate the West Hill telemetry conduit adjacent to the SCL pole on the north side of S 126th St. approximately 150' west of 84th Ave. S. Provide and install a 25-TA vault between the conduit and the SCL pole. Avoid locating the vault over the existing, adjacent West Hill Reservoir water main. Expose the existing conduit, cut and remove a segment sufficient to allow the installation 6 Now' 4400 of 3"conduit sweeps with a radius of 24". Do not damaging the existing 12 strand SM fiber cable in the conduit. Move 50' of the existing 12 count SM cable from slack storage in the type II hand hole on the NW corner of S 126th St and 84th Ave. S. Provide and install 3" schedule 80 PVC sweeps with a radius of 24" and interconnect the West Hill Reservoir conduit into the new vault. Provide the vault with a non-skid lid with welded, raised lettering that reads: "City of Renton LT/ITS". 4. Provide and install a 2" schedule 80 PVC conduit in a 36"deep trench from the new hand hole to the adjacent SCL pole. Sweep the conduit into a pole riser ending 3 feet below the cable attachment. Bed the underground conduit in 6" of rock free fill and backfill the trench. Restore all adjacent landscaping to the condition that existed prior to the start of work. Replace damaged grass with live turf. Underground and Aerial Fiber Placement • Work to be done during normal working hours: Segment A: Southeast FCB288 (SE-FCB288) 1. Reel bare fiber test and provide test results of the customer-provided 288 count single mode (SM) fiber cable. 2. Install the 288 count SM fiber cable from the FCW MDF through the building entrance conduit, through the pole riser and through the aerial conduit route to the PSE pole on the west side of Talbot Rd. S approximately 160' south of S Puget Drive, continue down the pole riser and through the City of Renton telemetry conduit to the splice vault at S Grady Way and Wells Ave. S. Provide a 50' service loop in the MDF. Provide not less than 200' of slack storage for every 2,000' of pathway and 100' at each end of the pathway. 3. Provide at least 100' of slack storage at the following locations along the aerial route: a. 123rd Ave. SE just south of SE 160th St. where fiber to Tiffany Park Elementary school will be spliced, b. 116th Ave. SE at 160th St. where fiber to Nelsen Middle School will be spliced, c. S 23rd St. 275' west of Benson Drive S where fiber to Teasdale Park will be spliced. d. S 23rd St. at Morris Ave. S where fiber to Talbot Hill Elementary School will be spliced. 4. Provide at least 200' of slack storage in the new 25-TA vault adjacent to the pole riser at Talbot Rd. S 160' south of S Puget Drive. This slack will be used when the cable under the I-405 project is relocated and spliced. 5. Attach a"002"tag and label to the 288 count SM fiber cable at each access point reading"NW-FCB288—Contact City of Seattle Department of Information Technology, (206) 386-1212". 6. Provide and install toneable mule tape along fiber cable in the underground. 7 Segment B: Central FCB288 (C-FCB288) 1. Reel bare fiber test and provide test results of the customer-provided 288 count SM fiber cable. 2. Install a 288 count SM fiber cable from the splice vault on the NW corner of Wells Ave. S and S Grady Way to the SW corner of Wells Ave. S and S 3rd St. Loop the cable through the 25-TA adjacent to the traffic signal type III hand hole and store cable in the vault. Continue west on S 3rd to Shattuck Ave. S. through the signal telemetry conduit. In addition to the 288 count SM cable, install the existing 24 count SM cable stored in the hand hole on the NW corner of S 3rd St. Store 100' of the 24 count SM cable in the Wells Ave. S vault and store the remainder of the SM cable in the traffic hand hole on the SW corner of S 3rd St. and Whitworth Ave. S. At Shattuck Ave. S store slack 288 count SM fiber in the 25-TA vault, continue north on Shattuck Ave. S to S 2°d St. If the conduit interconnect to Renton High School is complete, store slack 288 count SM fiber in the vault that will be at this location. Continue west on S. 2nd St. to Lake Ave. S and north on Lake Ave. S to Airport Way, loop the fiber through the adjacent vault and store slack 288 count SM fiber in the vault. Continue west on Airport way and north on Perimeter Rd. west to the West Hill Pump house. Pass through the interconnecting 2" conduit to the West Hill telemetry conduit. Continue west across Rainier Ave. N into the Rainier Station shopping center. Store slack 288 count SM fiber in the existing 25-TA at the base of the hill. Continue west to the 25-TA on the northwest side of NW 6th St. Cul-de-sac, store slack 288 count SM fiber in the vault. Continue west on NW 6th St. to Taylor Pl. NW, north to S 124th St. and west to the existing 25-TA in the westbound lane of S 124th St.just west of 84th Ave. S. Store slack 288 count SM fiber in the vault. Continue south on 84th Ave. S and west on S 126th St. to the new 25-TA vault. Store slack 288 count SM fiber in the vault. 3. Attach a label to the 288 count SM fiber cable at each access point reading "C-FCB288 —City of Renton contact City of Renton Information Services Division". 4. Attach a label to the 24 count SM fiber cable at each access point reading "City of Renton Traffic Signal Department contact City of Renton Information Services Division". 5. Provide and install toneable mule tape along fiber cable in the underground. Segment C: Northwest FCB288 (NW-FCB288) 1. Reel bare fiber test and provide test results of the customer-provided 288 count SM fiber cable. 2. Install a 288 count SM fiber cable from the 25-TA vault on the north side of S 126th St. 150' west of 84th Ave. S up the pole riser and through the aerial pathway, west to 84th Ave. S, south to S 128th St.,west to Beacon Ave. S, and northwest to the NE corner of Beacon Ave. S / 51St Ave S and S Leo St. where there is an existing SPU 144 count SM fiber. Provide not less than 200' of 8 Noe Now' slack storage for every 2,000' of pathway and 100' at each end of the pathway. 3. Provide at least 100' of slack at the following locations along the aerial route: a. S 128th St. at 80th Ave. S, b. S 128th St. at 78th Ave. S where fiber to Black River High School will be spliced, c. Approximately 7404 S 128th St. immediately across S 128th St. from the Skyway Water and Sewer District elevated reservoir at the top of the hill, d. Beacon Ave. S at S 124th St where fiber to Campbell Hill Elementary will be spliced. 4. Attach a"002"tag and label to the 288 count SM fiber cable at each access point reading"NW-FCB288—Contact City of Seattle Department of Information Technology, (206) 386-1212". Fiber Terminations and Splicing • Work to be done during normal working hours: 1. See attached spreadsheet A tab Assignments for details. 9 low ,4000 E. Construction Cost Apportionment In consideration for the City of Renton allowing Renton School District, Seattle Public Utilities, Seattle City Light, City of Kirkland, City of Bellevue, and the University of Washington fiber to occupy certain portions of the City's conduit system, approximately 3 miles in length with an estimated value of$1,500,000 for the useful life of the fiber, as defined by the participating agencies, Renton School District, Seattle Public Utilities, Seattle City Light, City of Kirkland, City of Bellevue, and the University of Washington agree to pay for most costs such as but not limited to: fiber optic cable, conduit upgrades, splice enclosures, fiber patch panels, miscellaneous parts and supplies and labor to install, test and certify the fiber system. All upgrades to the conduit system on city property and in the city right of way will be owned and maintained by the City of Renton. Conduit on Seattle Public Utilities property will be owned and maintained by the Utility. At such time as all of the parties to this agreement shall determine that utilizing the fiber optic cable is no longer in their interest, the parties shall transfer ownership of the fiber to the City of Renton or remove the fiber from the city's conduit system, leaving the conduits in original condition. See attachment for an estimate and all agencies will be billed based on actual costs in proportion to their fiber ownership, not including spares. 12 C. Maintenance Responsibilities and Costs, including Ongoing Maintenance and Responsibilities for Moves. Expected future maintenance has been broken down into the categories below. If a maintenance issue arises that is not listed below, those costs will be proportionately split based on fiber ownership in each fiber segment by the participating agencies as detailed in the attached table. Locates The City of Renton agrees to perform all locates required and requested through Underground Utilities Locate Center(UULC) for the fiber and conduit installed as a result of this agreement that is on property owned by the City of Renton or in the City of Renton right-of-way or in areas of King County immediately adjacent to City of Renton Water Department water mains. This includes all of the underground portions of this project as of December 31, 2008. Relocation Any and all relocations on Seattle City Light poles, Puget Sound Energy poles or City of Renton conduit will be proportionately split based on fiber ownership by the participating agencies. See attachment for the ownership percent of each fiber segment. Important Note: The Interstate 405 widening project, currently underway, will cause approximately 1,200' of conduit on Talbot Rd. S between Renton Village Place and S Puget Drive to be relocated sometime in 2009 or early 2010. The City of Renton will pay the State's contractor to install a new 4"conduit in the project area. This cost will be deducted from the 25% maintenance cost apportionment that the City if Renton is responsible for in this fiber segment for this fiber relocation only. Repairs/Breaks The City of Seattle DoIT shall contract with a competent and qualified vendor to provide 7x24x365, four hour response to any fiber breaks or outages that happen on the SE- FCB288 fiber segment. The City of Renton shall contract with a competent and qualified vendor to provide 7x24x365, four hour response to any fiber breaks or outages that happen on the C-FCB288 fiber segment. This service level agreement will require an annual premium, estimated to be $3,000.00, to ensure services are available when needed. The City of Seattle DoIT shall contract with a competent and qualified vendor to provide 7x24x365, four hour response to any fiber breaks or outages that happen on the NW- FCB288 fiber segment. 13 IN lie 40111f Annual Pole Attachment Fees Seattle City Light charges an annual attachment fee for each pole to which an aerial strand has been attached. This fee is currently$18.97 per attached pole per year. This project will attach aerial strands to 92 SCL poles. The associated annual fee is $1,745 per year. In consideration for the significant savings to Seattle City Light afforded by the joint construction of this fiber project, estimated to be $325,000, Seattle City Light hereby waives the pole attachment fees that would normally be assessed for the fiber segments associated with this and only this project. This waiver is specific to the fiber segments specified in this document and does not preclude Seattle City Light from collecting the usual pole attachment fees required by agreements with agencies participating in this project currently in force or that may be made in the future. IV. Apportionment of Liability During construction, the Contractor shall be responsible for the project work and the project site. In the event that the project work causes or contributes to any harm,damage, injury or loss to any person or property, whether during construction or after completion of the project, each party shall be responsible to the extent the party is found to be at fault. City of Seattle shall ensure that the City and all persons performing the Work, including without limitation the Utility's suppliers and subcontractors,maintain in effect at all times during the Work, coverage or insurance in accordance with the applicable laws relating to worker's compensation and employer's liability insurance (including,but not limited to, the Washington Industrial Insurance Act and the laws of the state in which any such person was hired), regardless of whether such coverage or insurance is mandatory or merely elective under the law. The Utility shall furnish to Participating Agencies such assurance and evidence of such coverage or insurance (such as copies of insurance policies and Certificates of Compliance issued by the Washington State Department of Labor and Industries) as Participating Agencies may request. City of Seattle shall maintain in full force and effect throughout the term of this Agreement, a minimum of One Million Dollars ($1,000,000) liability insurance for property damage and bodily injury. In satisfying the insurance requirements set forth in this section, the City may self-insure against such risks in such amounts as are consistent with good utility practice or shall obtain a coverage agreement through a Risk Pool authorized by Chapter 39.24 RCW which shall provide liability coverage to the City for the liabilities contractually assumed by the City in this Agreement. At the time of execution of this Agreement, and prior to commencement of performance of any of the Work, the City shall furnish Participating Agencies, upon request, with a Certificate of Insurance as evidence that policies providing insurance (or self-insurance) with such provisions, coverage's and limits are in full force and effect. 14 Nsie V. VI Changes or Addenda to General Terms and Conditions This Agreement shall run concurrent with the Interlocal Agreement General Terms and Conditions document.
