CALL TO ORDER:
The October meeting was called to order at 8:00 a.m. by J. Phelan.
APPROVAL OF MINUTES:
A motion was made by G. Taylor, seconded by R. Anderson and carried to approve the minutes
of the August 25, 2009, Regular Meeting. The motion carried.
CORRESPONDENCE:
There have been a few inquiries regarding whether the City would reimburse members for the
H1N1 vaccine. A motion was made by K. Parker and seconded by G. Taylor to approve
reimbursement for members who receive the H1N1 vaccine. The motion carried.
Request from retiree 1431 for reimbursement in the amount of$ 6,659.00 for payment of 28
prescribed decompression treatments. This was not a covered benefit by the medical plan. A
motion was made by K. Parker and seconded by G. Taylor to approve reimbursement for the
cost of the decompression treatments. A request was made to receive progress reports and a
treatment plan prior to authorization of further treatment. The motion carried.
NEW BUSINESS:
None
UNFINISHED BUSINESS:
Ballots were received and tabulated for the nomination of the Firefighter representative of the
LEOFF Disability Board. The following individuals were nominated: Greg Berquist, Henry Dykes,
Stan Engler, Glen Gordon, Kris Hanson, AI Koskovich, Art Larson, Jim Mathews, and Bruce

Phillips. Staff will contact those nominated to determine if they are interested in serving.
Ballots will be sent out and tabulated at the next scheduled meeting.

None
APPROVAL OF BILLS:
Board Members reviewed the claims processed under the Secretary's authority during October,
2009.
COMMENTS/ANNOUNCEMENTS:
The meeting schedule for 2009: January 27, February 24, March 24, April 28, May 26,June 23,
July 28, August 25, September 22, October 27, November 24, December 15.
ADJOURNMENT:
There being no further business, G. Taylor moved and K. Parker seconded to adjourn the
meeting at 8:35.
