THE CONTRACT IS MODIFIED AS FOLLOWS:
Description of Change:
Driveway Regrade at Sta. 101+80 RT: The Contractor is directed remove and dispose of
approximately 62.2 SY of the recently constructed and existing cement concrete driveway pavement to
a distance of 35 feet behind the back of sidewalk,and reconstruct the driveway pavement to the grades
shown on the attached Driveway Profiles Sheet 21/RD-7. The Contractor is directed to protect the
recently constructed driveway entrance. The driveway repair shall have a broom finish.
The above changes shall be in accordance with the plans listed below and the attached License
to Construct.
Reason for Change:
Driveway Regrade at Sta. 101+80 RT: The driveway regrade was built per plan,within
the limits of the existing right of way. The property owner found that the regraded
driveway was steeper and had a grade change at the back of the new driveway entrance
that made it problematic to access with his trailer. The property owner agreed to allow
the regrading to extend onto his private property in order to soften the grade and
improve access.
Materials:
This change order will result in the addition of 10.37 tons of bid item 17 (Crushed
Surfacing Top Course) and 62.22 square yards of bid item 39 (Cement Conc. Driveway
Repair).The removal of the existing concrete driveway and preparation of subgrade will
be paid by force account.

The Time for Completion is extended 4 Working Days.
Equitable Adjustment:
The City and Contractor agree that this Change Order does not cause an equitable
adjustment.
Sections 1-04.4 and 1-04.5 of the 2018 WSDOT/APWA Standard Specifications shall govern
this Change Order.The Work of the referenced Contract is modified to include the changes
detailed herein.The payment provided for herein shall constitute the complete and final
settlement for all costs of labor, equipment, materials, overhead, profit, permit fees,and all
other claims that may be made by the Contractor as a result of this change.

Subject: Driveway Regrade at Sta. 101+80 RT
Scope:
The Contractor is directed remove and dispose of approximately 42.7 SY of the recently
constructed and existing cement concrete driveway pavement to a distance of 24 feet
behind the back of sidewalk,and reconstruct the driveway pavement to the grades shown
on the attached Driveway Profiles Sheet 21/RD-7. The Contractor is directed to remove •
and dispose of approximately 14.4 SY of the recently constructed driveway entrance and
reconstruct it with a reverse grade as shown on the attached Driveway Profiles Sheet
21/RD-7. The Contractor is directed to remove and dispose of approximately 26 LF of
the adjacent curb and gutter and reconstructed per the Contract Provisions.
Plans (attached): Driveway Profiles Sheet 21/RD-7.
Measurement and Payment:
Measurement and payment for the removal and disposal of the cement concrete
driveway, cement concrete curb/gutter and cement concrete driveway entrance to be
determined. Payment for the new cement concrete driveway will be via contract item 38
Cement Conc. Driveway Entrance. Payment for the cement concrete driveway repair will
be via contract item 39 Cement Conc. Driveway Repair. Payment for the cement concrete
curb and gutter will be via contract item 37 Cement Concrete Traffic Curb and Gutter.

contractors and other parties acting on its behalf, permission to enter upon my property at
8035 S 130th St and modify:
❑ the property by changing front yard grade(s)to ensure a smooth transition to the
new street, sidewalk and/or driveway improvements;
® the existing driveway(s) by changing the existing grade(s)to ensure a smooth
transition to the new street improvements;
❑ the existing parking lot(s)by revising parking stall(s), driveway aisle(s),
walkways, curbing and/or parking island configuration(s)and grade(s);
❑ the property by installing retaining wall(s);
❑ the existing landscape area(s) by changing grade(s)to ensure a smooth
transition to the new street, sidewalk and/or driveway improvements;
❑ the property by planting tree(s), shrub(s)and/or groundcover;
❑ the property by:
❑ Installing new irrigation system(s);
❑ restoring existing irrigation system(s)to pre-construction operating
condition to match new planting bed configuration(s);
