At the client's request, ESNW rep was onsite to observe vault excavation and earthwork activities.
Vault Excavation
Rep observed contractor grading the vault excavation to design elevation with a large excavator. Rep observed the
south slope to be near vertical, the east and north slope to be 1:1 (H:V) to a near 7' vertical toe, and the west slope to
be (H:V) to a near 3' vertical toe. Although over -steepened, slopes are stable in current configuration and stable to
work within. The contractor is responsible for providing all safety measures in accordance with state and federal
regulation.
ESNW rep will return to observe earthwork activities when requested by client.

At the client's request, ESNW rep was onsite to observe lock and load wall excavation, storm water discharge, vault
excavation and earthwork activities.
Lock and Load Wall Excavation
While onsite, rep observed contractor excavating the temporary berm material that was placed previously along the
south slope cuts for site safety. Rep observed contractor use a large excavator to excavate material from the slope.
Contractor expressed plans to continue excavating material throughout the week. Rep recommended placing plastic
and covering the exposed cuts. Due to design, water line will be placed —3'-4' from the face of the bottom of the lock
and load wall. Due to the space constraints, ESNW recommended that contractor backfill the water line with controlled
density fill to increase stability near the south site slopes, lock and load wall, and general subgrade.
Storm Water Discharge
While onsite, rep used IMHOFF cone to test baker tank sewer water discharge. Rep used IMHOFF cone and found
sediment (<3 ml) after testing water for one hour. Rep took PH test and found water suitable for discharge at a PH
level of 6.5.
Vault Excavation
Rep observed contractor grading the vault excavation to design elevation with a large excavator. Rep observed the
south slope to be near vertical, the east and north slope to be 1:1 (H:V) to a near 7' vertical toe, and the west slope to
