At the client's request, ESNW rep was onsite to observe and test water line backfill, vault excavation and earthwork
activities.
Water Line Backfill
While onsite, rep observed contractor use a large excavator to excavate a water line (W5'xD6'). Rep observed
contractor place 8" ductile iron pipe at the toe of the excavation in a 6" lift of pea gravel. Rep will return to take
compaction test as needed tomorrow, November 5, 2019.
Vault Excavation
Rep observed contractor grading the vault excavation to design elevation with a large excavator. Rep observed the
south slope to be near vertical, the east and north slope to be 1:1 (H:V) to a near 7' vertical toe, and the west slope to
be (H:V) to a near 3' vertical toe. Although over -steepened, slopes are stable in current configuration and stable to
work within. The contractor is responsible for providing all safety measures in accordance with state and federal
regulation. Rep will return to take compaction test as needed tomorrow, November 5, 2019.
ESNW rep will return to observe earthwork activities when requested by client.

FAtthelient's request, ESNW rep was onsite to observe or test lock and load wall and vault backfill.
Lock and Load Wall
While onsite, rep observed contractor use a large excavator to excavate the slope and excavation for the lock and
load wall. Rep observed contractor excavate the keyway for the wall. Although over -steepened (near vertical), rep
observed that the cut was stable in its current configuration. ESNW recommends that ENSW representative be onsite
while contractor is working near or within the cut slope. Rep recommended that contractor place plastic sheeting over
the exposed slope to prevent stormwater from permeating the surface. Rep will return Monday, November 11, 2019,
to observe the conditions of the slope.
Vault Excavation and Backfill
Rep observed contractor place brown silty sand with gravel material in loose 12"-18" lifts and compacting with a hoe
pack. Rep took compaction tests on backfill and observed material to be adequate of at least 95% of its maximum dry
density (130.7 lbs./cu. ft.). Contractor expressed plans to continue backfilling vault Monday, November 11, 2019. Rep
will return to observe and test compaction as needed.
