Project No. Permit No. Supplementary Report: At the client's request, ESNW rep was onsite to observe or test vault excavation and backfill. Vault Excavation and Backfill While onsite, rep observed contractor working near or in the vault excavation. Rep observed south slopes of the excavation to be near vertical. Rep observed that the west slopes of the excavation were near a 1.5:1 (H:V) slope. Although over -steepened, slopes are stable in current configuration and stable to work within. Rep observed that contractor had covered the exposed slopes with plastic sheeting. The contractor is responsible for providing all safety measures in accordance with state and federal regulation. Rep will return to observe the conditions of the slopes while contractor is working in the excavation. Rep observed contractor use a medium sized excavator to excavate and place native brown silty sand with gravel material for the vault storm drain. Rep observed contractor place 6" perforated storm pipe at the toe of the excavation embedder in a 12" lift of 1 %2" wash rock. Rep observed contractor wrapping both drain pipe and wash rock in a geo-textile. Rep will return Monday, November 25, 2019, to observe and test the compaction of the backfill as needed. ESNW rep will return to observe earthwork activities when requested by client. Rep observed contractor grade and place import gray 5/8" minus crushed recycled concrete in loose—6"--12" lifts. Rep observed contractor place geo-grid as per plan. Once lifts were placed, rep observed contractor use a medium sized plate compactor or a medium sized double drum roller for compaction. Rep took compaction tests on fill and observed material to be adequate of at least 95% of its maximum dry density (122.0 lbs./cu. ft.). Behind the first panel of the second tier, rep observed contractor place a 4" corrugated drain pipe. Rep observed the south slopes of the lock and load wall to be near vertical. Although over -steepened, slopes are stable in current configuration and stable to work within. Rep observed that contractor had covered the slopes with plastic sheeting to minimize stormwater on exposed surfaces. Once lifts were placed, rep observed contractor use a medium sized plate compactor or a medium sized double drum roller for compaction. Rep took compaction tests on fill and observed material to be adequate of at least 95% of its maximum dry density (122.0 lbs./cu. ft.). Rep observed the south slopes of the lock and load wall to be near vertical. Although over -steepened, slopes are stable in current configuration and stable to work within. Rep observed that contractor had covered the slopes with plastic sheeting to minimize stormwater on exposed surfaces. Rep met with ESNW's Ray and Key -West's Steve to discuss possible changes to the slope. ESNW observed that the slope approximately 100' from the east end was very dense and unyielding. ESNW recommended that bank also be cut to fit at least 4' of geo-grid for the first --4' of the third tier and the top -4' of the third tier be cut back to fit 8' of geo-grid. For the third tier, contractor expressed plans that were confirmed by ESNW to use all 5/8" minus crushed recycled concrete for backfill instead of the clean crushed behind the panels.
