The Local Agency of City of Renton
desires to supplement the agreement entered in to with.CivilTech Engineering,Inc.
and executed on 05/05/2014 and identified as Agreement No. CAG-14-068
All provisions in the basic agreement remain in effect except as expressly modified by this supplement.
The changes to the agreement are described as follows:
Section 1, SCOPE OF WORK, is hereby changed to read:
The Scope of Work of the Original Agreement is supplemented to include the attached Exhibit A-1 (Sup 6).
II
Section IV, TIME FOR BEGINNING AND COMPLETION, is amended to change the number of calendar days
for Completion of the work to read: Change contract completion date from December 31,2019 to December 31,2021.
III
Section V, PAYMENT, shall be amended as follows:
See the attached in Exhibit"A"and Exhibit E-1 (Sup 6). •

SCOPE OF WORK
NE 315T STREET CULVERT REPAIR PROJECT
FEMA NO. 033-57745-00
For this Supplemental Agreement, CivilTech Engineering, Inc. ("Consultant") will provide engineering
support services to the City of Renton ("City") for the NE 31'Street Culvert Repair Project ("Project").
These services include engineering support during the following phase: post-construction.
I. INTRODUCTION
The following scope of services and associated costs are based upon the assumptions outlined below.
General Assumptions:
• Consultant project team is anticipated to include a project manager, project engineer, CAD
technician and admin staff member.
• The duration for completion of the record drawings is anticipated to be no more than 6 months,
once the red line mark-up set of as-built drawings is completed by the City and delivered to the
Consultant.
II. SCOPE OF WORK
The objective and purpose of this Agreement is for the Consultant to provide engineering support during
post-construction. Specifically,completion of record drawings for the project.
TASK 4—Post-Construction
Task 4 of the original agreement is modified as follows:
4.0 Management/Administration for Preparation of Record Drawings: Review monthly
expenditures and engineering activities during preparation of Record Drawings. Prepare and
submit to City a monthly invoice and progress report describing services provided that month.
4.1 Draft set of Record Drawings:The Consultant shall prepare a draft set of Record Drawings for
the City to review.The City will provide final red line mark-up set of as-built drawing sheets to
Consultant for incorporation into the draft set of Record Drawings. Revisions shall not be
clouded.Significant revisions,as determined by the City,shall be marked with revision triangles
and assigned numbers.Revised sheets shall include revision triangles, numbers and description
in revision block (e.g. RFI U, Change Order#, etc.). The description for each revision resulting
from construction will be provided to the Consultant by the City.

The Consultant shall provide a draft set of Record Drawings to the City within twelve(12)weeks
from the date of receipt of the final red line mark-up set of as-built drawing sheets from the
City.
The City will review the draft set of Record Drawings and provide comments to the Consultant
(if any)within four(4)weeks from the date the City receives the draft set of Record Drawings.
4.2 Record Drawings: Upon receipt of final review comments from the City, the Consultant shall
prepare the final Record Drawings.The Consultant shall provide a final set of Record Drawings
to the City,stamped and signed by a Professional Engineer licensed in the State of Washington.
The final Record Drawings shall be delivered to the City within eight(8)weeks from the date of
receipt of the final review comments from the City.
Deliverables
• Monthly invoices and progress reports.
• Draft Final Record Drawings—one electronic file in pdf format, sheet size 11x17.
• Record Drawings—two electronic files:
o pdf format(with PE stamp and signature on each sheet),sheet size 11x17;
o Auto CAD format(without PE stamp and signature).
