Project No Permit No Supplementary Report: At the client's request, ESNW rep was onsite to observe or test pH of stormwater, footing excavations and earthwork activities. Test pH of Stormwater While onsite, rep took pH tests to determine the acidity of stormwater. Rep took pH tests at the east end of the quarry spall entrance and observed a pH level of 6.5/7 pH. At the west of the quarry spall entrance stormwater (temporary) collection pit and observed a pH level of 9 pH. On the street, north and south of the west entrance, rep took a pH tests and observed stormwater to be a 6.5/7 pH. Rep tested banker tanks with stormwater and observed that contractor had added dry ice as recommended by ESNW. Rep tested a 6.5/7 pH in both baker tanks which is compliant with required pH levels for sewer discharge. Rep observed that pH level was contained onsite and that stormwater pH level in street was in compliance of required pH level at 6.5/7 pH. Footing Excavations Rep observed contractor removing form boards for the footings. Rep observed east slopes of the excavation to be near vertical Rep observed that the east slopes of the excavation were near a 0.75:1 (H -.V) slope. Although over -steepened, slopes are stable in current configuration and stable to work within. The contractor is responsible for providing all safety measures in accordance with state and federal regulation. Rep observed that contractor had sloped the southeast corner of the footing excavation to a 1:1 (H:V). Midway at the east footings to —30' north of the corner, rep observed that material had sloughed. Rep observed a seam and recommended that contractor excavate and remove excess material above the seam in the bank. Rep observed using a medium sized excavator to remove the material above the footings in that designated location. (See Image) Rep also observed contractor pumping stormwater that had been collecting in the footing excavations to the baker tanks and the stormwater vault onsite. Rep discussed recommendations with Accrete representatives onsite. Rep recommended that contractor add crushed rock to continue to berm the toe of the lock and load wall in the footing excavation. ESNW rep will return to observe earthwork activities when requested by client 4-1-S _ Andrew Kimn, Senior Technician, 206-430-9183 " Earth Solutions NWLLC ESNW Rep. Water Line Backfill Rep observed contractor use a medium sized excavator to excavate and install the water line south of the stormwater vault. Rep observed contractor install a 10" ductile iron pipe at the toe of the excavation (W4'xD4') embedded in a 12" lift pea gravel Rep observed contractor place brown silty sand with gravel material in loose 12"-18" lifts and compacting with a hoe pack. Rep took compaction tests on backfill and observed material to be adequate of at least 95% of its maximum dry density (130.7 lbs./cu. ft.). Test pH of Stormwater Rep took pH tests to determine the acidity of stormwater. Rep tested a 6.5/7 pH in both baker tanks which is compliant with required pH levels for sewer discharge. Rep observed that pH level was contained onsite and that stormwater pH level in street was in compliance of required pH level at 6.5/7 pH Sewer Discharge Rep used IMHOFF cone to test baker tank sewer water discharge. Rep used IMHOFF cone and found sediment (<3 ml) after testing water for one hour. Rep took PH test and found water suitable for discharge at a PH level of 6.5. ESNW rep will return to observe earthwork activities when requested by client. .1 . , J ;nior Technician, 206-430-9183 Earth Solutions NWLLc ` t 15365 NE 90"' Street, Suite 100 Redmond, WA 98052 Main (425) 449-4704 1 Fax (425) 449-4711 www.esnw.com DAILY FIELD REPORT Project Name Weatherly Inn Contractor #1 Rino Construction Contractor #2 ESNW Rep.
