hereby made a pad of the Plans and Contract Provisions (Contract Documents) for the above-named
project. Bidders shall take this Addendum into consideration when preparing and submitting their bids.

Proposal Signature Page or by signing in the space provided below and submitting the signed addendum
with the bid package. Failure to provide this written acknowledge may result in disqualification of the Bidder's
submittal.

All cement concrete pavement used for constructing the raised concrete intersections shall be high
early Portland cement concrete Class 4OOOwith 1-day cure and meets the requirements of Section
5-O5.3(17).
8. ADO new Special Provision Section 8-06.2Materials:
All cement concrete pavement used for constructing the driveway entrances shall be high early
Portland cement concrete Class 4000 with 1-day cure and n)ae&a the requirements of Section 5-
05.3(17).

4. ADD to Special Provision Section 8-14.2 Materials (Page SP-1 87):
All cement concrete pavement used for constructing sidewalks shall be high early Portland cement
concrete Class 4000 with 1-day cure.
ALL OTHER REQUIREMENTS OF THE CONTRACT DOCUMENTS REMAIN IN EFFECT.
