Project No Permit No. Supplementary Report: Reviewed 4"1/x5 At the client's request, ESNW rep was onsite to observe or test cut slopes, footing backfill and earthwork activities. Cut Slopes While onsite, rep observed south slopes of the cut slopes to be near vertical and covered in plastic sheeting. Rep observed that the east slopes of the excavation were near a 0.75:1 (H:V) slope. Although over -steepened, slopes are stable in current configuration and stable to work within. The contractor is responsible for providing all safety measures in accordance with state and federal regulation. Rep observed that contractor had sloped the southeast corner of the footing excavation to a 1:1 (H:V). Footing Backfill Rep observed contractor use a medium sized excavator to mix and cement treat native brown silty sand with gravel material. Rep observed contractor treat native material with a 3% cement mix. Rep observed contractor place 8"- 8" lifts and compaction with a jumping jack. Rep took compaction tests on backfill and observed material to be adequate of at least 95% of its maximum dry density that was adjusted based t treatment (130.7 lbs./cu. ft.).
