Supplementary Report:
At the client's request, ESNW rep was onsite to observe or test water line backfill, storm line backfill, cut sloS
sewer discharge and earthwork activities.
Water Line Backfill
While onsite, rep observed contractor use a medium sized excavator to excavate and install the water line located
at the northeast corner of the site. Contractor expressed plans to continue installing water line this week.
Storm Line Backfill
Rep observed contractor use a medium sized excavator to excavate and install the storm line. Rep observed
contractor install a 12" corrugated pipe at the toe of the excavation (W2'xD4') embedded in a 12" lift pea gravel.
Rep observed contractor place brown silty sand with gravel material in loose 12"-18" lifts and compacting with a
hoe pack. Rep took compaction tests on backfill and observed material to be adequate of at least 95% of its
maximum dry density (130.7 lbs./cu. ft.).
Cut Slopes
Rep observed south slopes of the cut slopes to be near vertical and covered in plastic sheeting. Rep observed
that the east slopes of the excavation were near a 0.75:1 (H:V) slope. Although over -steepened, slopes are stable
in current configuration and stable to work within. The contractor is responsible for providing all safety measures
in accordance with state and federal regulation. Rep observed that contractor had sloped the southeast corner of
the footing excavation to a 1:1 (H:V).
Sewer Discharge
Rep used IMHOFF cone to test baker tank sewer water discharge. Rep used IMHOFF cone and found sediment
(<3 ml) after testing water for one hour. Rep took PH test and found water suitable for discharge at a PH level of
6.5.
ESNW rep will return to observe earthwork activities when requested by client

Rep took pH tests to determine the acidity of stormwater. In red baker tank rep observed a pH of 8.5/9, rep
recommended that contractor add dry ice to lower the pH level. Rep observed that pH level was contained onsite
and that stormwater pH level in street was in compliance of required pH level at 6.5/7 pH. Contractor expressed
plans to add dry ice tomorrow, January 23, 2019, to lower the pH. Rep will return to test the pH tomorrow.

Footing Excavations
Rep observed contractor use a large excavator to excavate the footings for the north footings of the building. Rep
used a'/z" steel t-probe and observed that the subgrade was very dense, firm, unyielding, and suitable for support
of the proposed building footings.

(<3 mI) after testing water for one hour. Rep took PH test and found water suitable for discharge at a pH level of
6.5.
