Supplementary Report:
At the client’s request, ESNW rep was onsite to observe and test water main junction backfill and slope stability.
Water Main Junction Backfill
ESNW Rep observed contractor backfill around a water main junction vault. Rep observed contractor backfill with
native brown silty sand with gravel material. While onsite, rep used a ½” steel t-probe and observed the subgrade
to be very dense, firm, unyielding, and suitable for support of the wall. Rep observed contractor place 18’’ of pea
gravel and then 1’ lifts of native material compacted with a jumping jack to grade. ESNW rep observed that the
contractor had achieved minimum compaction standards of 95% or greater in areas tested.
Cut Slopes
While onsite, rep observed slopes of the cut slopes to be near vertical. Rep observed that the north slopes of the
excavation were near a 0.75:1 (H:V) slope. Although over-steepened, slopes are stable in current configuration
and stable to work within. The contractor is responsible for providing all safety measures in accordance with state
and federal regulation
ESNW rep will return to observe earthwork activities when requested by client

Supplementary Report:
At the client’s request, ESNW rep was onsite to observe roof drain subgrade, water main junction backfill, slope
stability.
Roof Drain Trench
ESNW rep observed contractor using a medium sized excavator to grade along the northern margin of the main
building in preparation for excavation of a 2’ deep 100’ long trench for a roof drain and a 3’ deep trench for an 8’’
sewer. Rep used a ½” steel t-probe and observed the subgrade to be very dense, firm, unyielding, and suitable
for support of the proposed roof drains and sewer.
Water Main Junction
While onsite, ESNW rep observed contractor placing form boards for concrete thrust blocks supporting a 6’’ ductile
iron water main. Contractor relayed that thrust blocks would be poured Thursday and water main junction vault
would be backfilled to grade with native material and compacted with a hoe pack and jumping jack.
Cut Slopes
While onsite, rep observed slopes of the cut slopes to be near vertical. Rep observed that the north slopes of the
excavation were near a 0.75:1 (H:V) slope. Although over-steepened, slopes are stable in current configuration
and stable to work within. The contractor is responsible for providing all safety measures in accordance with state
and federal regulation
ESNW rep will return to observe earthwork activities when requested by client

ESNW Rep observed contractor backfill around cast in place wall North of main building. Rep observed contractor
backfill with native brown silty sand with gravel material. While onsite, rep used a ½” steel t-probe and observed
the subgrade to be very dense, firm, unyielding, and suitable for support of the wall. Rep observed 1’ lift of native
material compacted with a jumping jack to grade.
Water Main Connection
ESNW Rep observed contractor working on the 12’’ ductile iron water main connection to site in Talbot Road S.
Contractor relayed backfill would not take place until after pressure testing was completed.
