You are ordered to perform the following described work upon receipt of an approved copy of this change order:
Due to the existing cross slope of the sidewalk near STA 89+00 RT, the planned grade break in the
sidewalk would result in a cross slope from the grade break to back of curb in excess of 12%. The attached
plans provide a 2” increase to the typical curb height. The cross slope and increased curb height transition
would create a trip hazard at the tree grate frame. This change also includes the change to have the tree
grates and frames omitted from the tree pits and the addition of plantings per the attached plans. The
tree grates and frames will be delivered to the City.
CO1-04 item is based on the sum of Force Account Sheets #9, #9.1 and #11. This time is for the setting of
forms to create a grade break in the sidewalk on the west side of the project as per FWD #002.

Provide proposal and time impact analysis, within fifteen (15) days or as stated above for the scope of
work described:
NOTE: This is NOT a change order and does NOT constitute approval or notice to proceed on the
issue.
SCOPE:
Due to the existing cross slope of the sidewalk near STA 89+00 RT, the planned grade break in the
sidewalk would result in a cross slope from the grade break to back of curb in excess of 12%. The
attached plans provide a 2” increase to the typical curb height. The cross slope and increased curb height
transition would create a trip hazard at the tree grate frame. This change also includes the ch ange to
have the tree grates and frames omitted from the tree pits and the addition of plantings per the attached
plans. The tree grates and frames will be delivered to the City.
MATERIAL:
The attached plan sheets identify the new plant materials and ir rigation head changes. All materials not
specifically identified in the plans or specifications shall conform to the WSDOT Standard Specifications.
MEASUREMENT AND PAYMENT:
No change is required for the bid items #25, #26, #27, 28, #29, #30 and #46
This RFP will add the following bid items to the contract;
CO-XX – Sesleria autumnalis/ Autumn moor grass; 1 gal. cont. (48 EA)
CO-XX – Bark or Wood Chip Mulch (1 CY)
CO-XX – 8” Cement Concrete Traffic Curb, Incl. Transition, LS (1 LS)
