INSPECTION OBSERVATION / REMARKS
Arrived onsite as requested to inspect placement and sample and test concrete for vault footings. Corliss Resources supplied approximately
45cys of mix design P13010. Concrete was placed by boom pump and consolidation was via mechanical vibration. AAR sampled and tested
concrete, one set of five cylinders was cast and left onsite in insulated box for initial cure. To the best of my knowledge work observed meets
project plans and specifications.
Items inspected are in conformance with approved plans, specifications and jurisdictional requirements
