Subject: Parking Modification
Dear Lesley:
This letter is written to provide additional information for the purpose of obtaining a parking
modification.It is my understanding that the parking modification is necessary for the city to
approve the warehouse project with no new parking.stalls added.It is the hospital's position
that no new parking stalls are necessary since the new floor will provide storage for material
goods and office space for Information Systems staff who are currently on campus.Most of o
the staff members who are not in the Information Systems office in the warehouse now are
using surplus patient rooms as office space.
Attached you will find excerpts from the 1991 master plan documenting building square
footages for existing conditions and phase 1 additions.All of the projects identified as part of
phase 1 (see page A-7)have been completed except for the Chemical Dependency Unit,and
the Hospital Third Floor West Expansion.The only other project completed on campus which 0
was not identified in the master plan is the Cogeneration Facility(5,000 sf).

We have calculated the parking requirements for the hospital based on 3 categories of use per
4-14-8 of the Renton Building Regulations.The three categories are 4-14-8-F.1 Medical and
Dental Offices for the medical office buildings,NW pavilion and radiation oncology,4-14-8-
F.3 Hospital for the main hospital building,and 4-14-8-C.2 Warehouse and Storage Buildings
for the warehouse and cogeneration facility.The calculation is attached.
Also attached is Valley Medical Center's most recent traffic management report to Metro,
which shows a total of 2,256 existing stalls on campus.Since the number of stalls currently on o
campus is greater than the number required,it seems reasonable to allow the warehouse
addition to proceed without additional parking spaces being added,and without a more
elaborate parking analysis being performed.

Phase I Development Plan (1991-1994)
The facilities identified for completion in the first phase of this Master Plan are already in
or near the final stages of planning and design. There will be a number of remodeling
and renovation projects in existing buildings at various times throughout all three phases.

PART ONE: PROJECT DESCRIPTION/BACKGROUND
The applicant is requesting Administrative Site Plan approval and environmental review for a
10,670 square foot second story addition to an existing 12,200 square foot building on the main
hospital campus. The proposal also includes the addition of a mezzanine entrance on the south
side of the building. The new entrance will be ADA accessible and will connect to the hospital's
sidewalk system.
Approximately 4,318 square feet of the addition will be occupied by the Information Services (IS)
Department and the remainder of the space will provide general storage for the hospital. The
addition will consolidate the IS department in one location by relocating employees from the first
floor of the project site and from the main hospital.
PART TWO: ENVIRONMENTAL REVIEW
In compliance with RCW 43.21C.240, the following project environmental review addresses only
those project impacts that are not adequately addressed under existing development standards
and environmental regulations.
A. Environmental Impacts
The Proposal was circulated and reviewed by various City Departments and Divisions to
determine whether the applicant has adequately identified and addressed environmental
impacts anticipated to occur in conjunction with the proposed development. Staff reviewers
have identified that the proposal is likely to have the following probable impacts:
(1) Fire Prevention
Impacts: The proposal will add 10,670 square feet of new construction to the City which will
potentially impact the City's Fire Department. A Fire Mitigation Fee, based on $0.52 per
square foot of new construction, applies to all new construction in the City. The mitigation
fee for this proposal is estimated at$6,207.24. The fee is payable prior to the issuance of
building permits.
Mitigation Measures: The applicant shall pay the applicable Fire Mitigation Fee at the rate
of$0.52 per square foot of new construction. The Fire Mitigation Fee is estimated at
$6,207.24 and is payable prior to the issuance of building permits.
Policy Nexus: Environmental Ordinance (SEPA), Fire Mitigation Fee Resolution and
adopting ordinance.
(2) Transportation
Impacts: The proposal will result in an increase in traffic trips and is therefore subject to the
City's Traffic Mitigation Fee. The proposal will generate approximately 52 new average
daily trips. The Traffic Mitigation Fee is calculated at$75 per each new average daily trip.
The fee is estimated to be $3,905.25 (52.07 trips x$75 = $3,905.25). The fee is payable
prior to the issuance of building permits.

C. Consistency with Site Plan Criteria
GENERAL CRITERIA:
a. Conformance with the comprehensive plan, its elements and policies;
The site is designated Center Institution (CI) on the Comprehensive Plan Map. The objective of the
CI designation is to assure that adequate land and infrastructure are available for the development
and expansion of facilities to serve the post secondary educational and health care needs of the
area and protect adjacent uses from impacts of these more intensive uses. The proposed
development is consistent with this designation.
b. Conformance with existing land use regulations;
The subject site is located in the Commercial Office (CO) Zone. Hospitals are a permitted use in
the CO Zone with the condition that the site is located in the Center Institution Comprehensive Plan
Designation. The proposed expansion is to an existing, permitted hospital located in the CI
Comprehensive Plan designation.
The expansion is proposed at a height of approximately 92 feet, under the 250 foot maximum
height allowed in the CO Zone. The project includes a new mezzanine entrance at the south side
of the building. The existing landscaping in this area will be replaced once the construction of the
mezzanine is complete. The proposed expansion complies with the setback, landscape, and
height development standards of the CO zone.
The Parking and Loading Ordinance requires one space for every 200 square feet of area
dedicated to medical offices, one space for every 1,500 square feet of warehouse or storage
space, and for hospitals--one space for every 3 beds, one for each staff doctor, and one for every
three employees. The VMC campus has approximately 310,800 gross square feet dedicated to
medical office use, 25,400 gross square feet of warehouse and storage use, 12 staff doctors, 800
employees, and 276 beds. With the proposed addition, a total of 2,126 parking spaces are

required for the campus. Currently, the Valley Medical Center campus provides 2,256 parking
spaces. There is adequate parking on site to accommodate the proposed addition.
c. Mitigation of impacts to surrounding properties and uses;
The site of the proposed expansion is located in the eastern portion of the Valley Medical Center
Campus. The expansion site is entirely surrounded by the campus on the north, west, and south
and is bounded by State Route 167 on the east. The impacts to surrounding properties will be
minimal considering the property is currently fully developed.
d. Mitigation of impacts of the proposed site plan to the site;
The proposal is not expected to adversely impact the site. There are no significant natural features
which would be impacted by the development. Construction activities related to the proposed
development will be required to utilized best management practices which will reduce potential
construction impacts to the site.
e. Conservation of area-wide property values;
The proposed development will allow an existing facility to expand in its current location. The
expansion of the medical campus is anticipated to conserve property values in the vicinity.
f. Safety and efficiency of vehicle and pedestrian circulation;
There are no changes proposed to the existing vehicle circulation and parking layout for the site.
The proposed mezzanine entrance on the south side of the building will provide ADA access and
will also connect to the hospital's sidewalk system.
g. Provision of adequate light and air;
The proposed expansion will allow adequate light and air to the building and the site.
h. Mitigation of noise, odors and other harmful or unhealthy conditions;
The proposal is not expected to create any harmful or unhealthy conditions. Noise and odor
impacts in the vicinity will not have a significant increase due to the proposed development. Noise,
dust, and odors which may result from the temporary construction of the site will be mitigated by
the applicant's construction mitigation plan.
i. Availability of public services and facilities to accommodate the proposed use;and
The subject site is adequately served by public services and facilities.
j. Prevention of neighborhood deterioration and blight.
No deterioration or blight is expected to occur as a result of the proposal.
In reviewing the proposal with respect to the site Plan Approval Criteria set forth in Section 4-31-
33(D) of the Site Plan Ordinance, the following issues have been identified by City Departmental
Reviewers and Divisional Reviewers:

The City of Renton Environmental Review Committee has determined that it does not have a probable significant
adverse impact on the environment. An Environmental Impact Statement (EIS) is not required under RCW
43.21C.030(2)(c). Conditions were imposed as mitigation measures by the Environmental Review Committee under their
authority of Section 4-6-6 Renton Municipal Code. These conditions are necessary to mitigate environmental impacts
identified during the environmental review process.
Appeals of either the environmental determination [RCW 43.21.0075(3),WAC 197-11-680] and/or the land use
decision must be filed in writing on or before 5:00 PM October 26, 1998. If no appeals are filed by this date, both
actions will become final. Appeals must be filed in writing together with the required $75.00 application fee with: Hearing
Examiner, City of Renton, 1055 South Grady Way, Renton, WA 98055. Appeals to the Examiner are governed by City

The project consists of a new second story addition to an existing one-story office/warehouse
building on the Valley Medical Center Main Campus.Site is on Lot 1 of 9.21 acres.The Main
Hospital is also located on this lot.The project site covers 3% of the lot(12,200 SF first floor/9.21 "
acres).

Access to the project site is through the main hospital campus only,following the main loop
road.Access to the hospital is off South 43rd Street on the south and Talbot Road South on the o
east.Rou hte 167 runs along the west side of the hospital campus with an exit at Sout 41st o
Street/south on East Valley Road to South 43rd Street.
The current building houses the main computer network for the hospital,the Purchasing
department,medical records storage,x-ray film storage,and the hospital's substation/generator.
The second story addition will provide 10,670 SF of space.The Information Services(IS) o
Department will occupy 4318 SF of the addition,with the remainder of the space providing o
general storage for the hospital,not to exceed 70 PSF.Currently employees of the IS department Z
are on the first floor of the project site and in the main hospital.This addition will consolidate the Z
IS department in one location on campus near the hospital's central Computer Room which is
located on the first floor of the existing building.
Building height above finished grade varies as the site slopes down from the high side on the
east to the low side on the west.The west elevation is 39'-0" above finish grade and the east
elevation is 26'-0"above finish grade.See Exterior Elevations submitted with this package. E

Parking Lot Coverage and Landscape Analysis
Gentlemen:
Valley Medical Center requests a waiver for the parking lot coverage and landscape
analysis requirement from the Site Plan Approval package.
The project provides new office space to consolidate the Information Services
Department in the same building as the main hospital computer system. Employees of
the Information Services Department are currently located on the first floor of the
project site and on the first and second floors of the main hospital. The two buildings
are next door to each other and share the same parking lots.
In accordance with city requirements, eighteen spaces are required for this project.
The hospital will not hire new employees to fill these stalls. The assigned employee
parking spaces will remain unchanged. Employees without assigned spaces will park
in the same lots after completion of the project.
The hospital acknowledges that for the next main campus expansion project, it will
provide a parking lot coverage and landscape analysis as required.

Drainage Report

For the above named project,I am requesting a waiver of the Drainage Report as part of the Site
Plan Approval Package.This construction project is a second story addition directly on top of a
one story building,with the area of exposed roof remaining the same.
The new roof will have four roof drains with overflow drains right next to the main drains.The
vertical rainwater piping from the new roof drain will tie into the existing exterior downspout
through existing opening in the concrete parapet.This downspout already ties into the hospital's
storm sewer system.The overflow drain will run parallel to the new/existing with the
downspout draining at grade.The existing roof uses an overflow scupper.

General. We have completed a preliminary review of the pre-application materials for the above-
referenced development proposal. The following comments on development and permitting issues are
based on the pre-application submittals made to the City of Renton by the applicant. The applicant is
cautioned that information contained in this summary may be subject to modification and/or
concurrence by official decision makers(e.g.,Hearing Examiner,Zoning Administrator,Board of
Adjustment, Board of Public Works,and City Council). Review comments may also need to be
revised based on site planning and other design changes required by City staff or made by the
applicant. The applicant is encouraged to review all applicable sections of the Renton Municipal
Code.
As we understand your proposal,you intend to add a 10,670 s.f. second floor addition to an existing
building on the Valley Medical Center campus. The space will be used for Information Services (IS)
Departmer it(4,318 s.f.)and the remainder would be used as storage for the hospital. The applicant is
proposing no new parking facilities to serve the use, as existing employees would be redistributed on
the campus and the parking need would not increase.

Zoning. The site is zoned Public( 1). All uses within the P-1 Zone require Site Plan Review. WJA :04 iSh4UCt�
Environmental Review. The proposal is subject to environmental review(SEPA)based on the size
of the addition. Typical mitigation measures include payment of a Fire Mitigation Fee for the new
construction.
Site Plan Review. The proposal is subject to administrative site plan review,based on the size of the
proposal. Site Plan Review looks at issues such as the horizontal and vertical arrangement of
buildings on the site,landscaping,parking and circulation,pedestrian circulation, signage and lighting.
Parking and Loading. If additional parking is not proposed to meet the City's standards for the
addition,tlien the applicant will need to apply to the Administrator of the Planning/Building/Public
Works Department for either a deviation or modification. A parking analysis should be provided to

GEOTECHNICAL ENGINEERING SERVICES
PROPOSED WAREHOUSE ADDITIONS
VALLEY MEDICAL CENTER
RENTON, WASHINGTON
FOR
VALLEY MEDICAL CENTER
INTRODUCTION
This report presents the results of our geotechnical engineering services for the proposed
addition to the existing warehouse building located at Valley Medical Center in Renton,
Washington. The existing warehouse is located in the west portion of the Valley Medical Center
campus on the north side of the engineering building and west of the main hospital building. The
project location is shown in the Vicinity Map, Figure 1.
We understand that development plans include constructing a second floor level to the
warehouse with future plans to add a third floor level. We also understand that the existing
warehouse building will remain in operation while the new structure is constructed above it.
Final design details have not been developed, but it is our understanding that the building will
likely be supported on a combination of shallow foundations and auger-cast piles. Columns will
be located at about 30 feet on center. Foundation loads on each column are expected to be as
follows:
• Axial loads: 324 to 389 kips, plus 189 and 145 kips seismic, respectively. No uplift.
• Lateral loads: 127 to 177 kips seismic(east-west and north -south, respectively).
We also understand that shallow foundations will be supported at the same elevation as
adjacent shallow foundations that support the existing building.
SCOPE
The purpose of our geotechnical engineering services is to perform subsurface explorations
and engineering analyses, and provide recommendations for foundation support and design of the
proposed warehouse addition. Our specific scope of services includes the following tasks:
1. Review available reports and studies for the site and surrounding area and complete a brief
site visit to evaluate exploration requirements and access.
2. Drill two borings to depths of about 22 feet at the warehouse site using truck-mounted drilling
equipment. The borings will extend about 10 to 15 feet into the dense native soil. We will
also drill two borings with portable drilling equipment to about 10 to 15 feet along the east
and north sides of the warehouse where access is limited.
3. Drill one boring to a depth of about 19 feet at the laundry facility site using truck mounted.

4. Evaluate pertinent physical and engineering characteristics of the foundation soils based on
laboratory tests performed on samples obtained from the borings and comparison with
' previous data referenced above. The laboratory tests include moisture/density determinations
on selected samples obtained from the explorations.
5. Describe the subsurface soil and ground water conditions.
6. Provide recommendations for earthwork including the following:
• Requirements for stripping, removal of soft,organic or other unsuitable material,
• Suitability of on-site soil for use as structural fill,
• Imported structural fill specifications,
• Placement and compaction of structural fill for support of parking areas and adjacent
improvements,
• Utility trench backfill placement and compaction,
• Effects of weather and construction equipment on the site soils,
• General recommendations for erosion and sedimentation control.
7. Provide recommendations for allowable temporary cut slope inclinations and permanent cut
and fill slope inclinations.
8. Provide recommendations for temporary dewatering during construction. This will include
recommendations for general dewatering of excavations and trenches, as appropriate.
9. Evaluate foundation support options including shallow spread footings, piles and drilled piers.
10. Attend one design team meeting to discuss our conclusions and evaluation of foundation
options.
11. Develop geotechnical design recommendations for selected foundation types to support the
building on a combination of shallow spread footings, and either drilled piers or pile
foundations, as appropriate. Our recommendations include the following:
• Allowable capacity and settlement estimates for shallow spread footings, or allowable
axial capacity versus penetration requirements for downward and upward loads (including
allowable end bearing and skin friction) and settlement estimates for piers and piles.
• Allowable earth pressure and base friction coefficient for resisting lateral loads on
shallow foundation elements.
• Allowable lateral pile or drilled pier capacity as a function of lateral deflections for fixed
and pinned pile or pier cap conditions.
• Capacity reductions for group action of piles and/or piers.
• Recommendations for construction monitoring and testing requirements, as appropriate.
12. Provide recommendations for seismic design including site coefficient in accordance with
Uniform Building Code.
13. Provide a written report presenting our findings, conclusions and recommendations, along
with supporting field and laboratory data.

SITE DESCRIPTION
BACKGROUND INFORMATION
We were provided several sources of background information related to the existing facilities
for our review. These included the following:
1. "Report of Soils Investigation, Proposed Valley General Hospital, Renton, Washington for
Public Hospital District No. 1" by Dames & Moore dated May 11, 1967.
2. "Summary Report, Engineering Services During Site Work, Proposed Valley General
Hospital, Renton, Washington by Dames & Moore dated October 30, 1967.
3. "Soils Investigation, Parking Lot Addition, Valley General Hospital, Renton, Washington"
by Dames& Moore dated December 18, 1978.
4. Portion of report prepared by Roger'Lowe Associates, Inc. dated April 25, 1980 completed
for additions to the hospital building.

SURFACE CONDITIONS
The existing warehouse, a one story building, is about 180 feet long (north to south) and 60
feet wide (east to west). The original warehouse was constructed in 1974. A 40-foot long
addition was constructed in 1980 at the north end of the original warehouse. We understand that
the finish floor in the warehouse is at Elevation 57.5 feet. Paved parking is located along much of
the west side of the warehouse. An exposed on-grade slab is located within a fenced area
(emergency generator) along the west-side of the warehouse at the north end. Lawn areas with
sidewalks are located to the north, east and south of the warehouse. The ground surface in the
warehouse area rises from about Elevation 52 feet in the northwest portion of the site to about
Elevation 64 feet in the southeast portion. The ground surface is about Elevation 64 feet along
much of the east side of the warehouse.
SUBSURFACE CONDITIONS
Site Explorations
We explored subsurface soil and groundwater conditions at the site by drilling six borings to
depths of about 6.5 to 21.5 feet below the existing ground surface. Gregory Drilling drilled two
borings (B-1 and B-2) on April 8, 1998 using truck-mounted hollow-stem auger drilling
equipment. CN Drilling drilled four borings (B-3a, B-3b, B-3c and B-4) on April 8, 1998 using
portable solid stem auger drilling equipment to access the landscaped portions of the site. The
locations of the explorations are shown in the Site Plan, Figure 2. The details of our field
exploration program, laboratory testing and exploration logs are presented in Appendix A.
Soil Conditions
Subsurface soil conditions encountered in the borings generally consist of dense and hard
native soil along the east side of the existing building (borings B-3a, B-3b and B-3c) at elevations
above the planned foundation level. We understand that excavation for a recently constructed

utilidor that extends along the east side of the warehouse encountered very dense native soil in
this area. On the north and west sides of the building (borings B-1, B-2 and B-4) we encountered
loose to medium dense fill overlying dense and hard native deposits. The fill in these borings was
encountered to a depth of about 8 to 10 feet below the planned foundation level. The firm native
soil encountered in our borings includes dense to very dense silty sand with gravel and hard silt.
No ground water was encountered in the borings. However, ground water conditions should
be expected to fluctuate as a.function of season, precipitation and other factors
CONCLUSIONS AND RECOMMENDATIONS
GENERAL
Based on the results of our explorations, our understanding of the anticipated design loads and
analyses, we conclude that the proposed addition may be supported on a combination of shallow
and deep foundations. We therefore recommend that shallow foundations be used to support the
east portion of the proposed addition where dense and hard native soil is expected to be present at
foundation elevation, and that auger-cast piles be used to support portions of the structure where
fill is present below the proposed foundation level.
Based on our understanding of the proposed structural design, we expect that footing and pile
cap excavations will extend no deeper than the bottom of the shallow footings supporting the
existing building, and that temporary shoring will not be necessary. It is possible the adjacent
structures and utilities may require temporary shoring for support.
CONSTRUCTION CONSIDERATIONS
Based on our discussions with Jerry Griffith at Skilling.Ward Magnusson Barkshire, Inc.
(SWMB), we expect that footing and pile cap excavations will extend no deeper than the bottom
of the shallow foundations supporting the existing building. We therefore expect that temporary
shoring will not be necessary and that excavations will be completed as open cuts.
It is possible the adjacent structures or utilities may be affected by the excavations and some
temporary shoring may be required. GeoEngineers should be contacted to review the
contractors excavation plan so shoring requirements can be evaluated if necessary.
We expect that auger-cast piles for support of much of the building can be installed using
conventional crane supported drilling equipment. However, installation of auger-cast piles within
the existing building for interior column support will require either removing portions of the
existing building roof for crane access or using low-overhead drilling equipment capable of
operating within the building. In our experience, both approaches are suitable for this type of
construction. Since it is desirable to keep the existing building operational for some of its current
functions, we recommend that consideration he given to working within the building with low-
overhead drilling equipment.

We expect that the new structure will be supported on a combination of shallow foundations
(east portion of building) and auger-cast piles (west portion). Our explorations were completed
outside the existing building and limited to readily accessible portions of the site. We recommend
that explorations be completed at each column location as part of the contractors scope to assess
the appropriate final design configuration for foundation support. This will allow determination
of column locations where shallow foundation support can be used and column locations where
• auger-cast piles will be necessary to support the building loads.
We expect that full time monitoring by a representative of GeoEngineers will be necessary
during construction to evaluate subgrade conditions and determine the appropriateness of shallow
foundations versus deep foundations at various foundation locations around the structure.
SITE PREPARATION AND EARTHWORK
Site Preparation
We expect that site preparation and earthwork will include removal of existing pavement and
walkways within the work area, and excavation to achieve design subgrade elevation in the
planned footing and pile cap areas. Excavation depths in the area of pile caps will likely be on
the order of 3 to 5 feet. On the east side of the building where spread footings will be constructed
excavations will be on the order of 6 to 8 feet below the existing ground surface. Suitable cut
slopes as described in a subsequent section of this report should be used to protect adjacent
structures and reduce the risk to workers within the excavations.
Effective erosion and sedimentation controls must be implemented during construction so that
potential impacts to the adjacent areas are minimized. The erosion potential of the on-site soils is
moderate. The erosion and sedimentation control measures used for this project should be in
accordance with the requirements of the city of Renton.
Structural Fill
New fills in building and pavement areas should be placed and compacted as structural fill.
The suitability Of existing fill or native soil for use as structural fill will depend on its gradation
and moisture content. In our opinion, the near surface silty soils can be used as structural fill
where compaction to 90 percent of maximum dry density (MDD) (determined in accordance with
ASTM D-1557). Much of the on-site soils will not be suitable for use in areas where compaction
to at least 95 percent is required. The on-site soils will not be suitable for use as fill during
periods of wet weather. We recommend, therefore, that the project be planned to include
importing structural fill for backfill around pile caps and footings and below floor slab areas
where compaction to at least 95 percent of MDD is required.
We recommend that imported structural fill consist of sand and gravel or crushed. rock
containing less than 5 percent fines (material passing U.S. Standard No. 200 sieve) by weight
relative to the fraction of the material passing the 3/4-inch sieve. This material should be free of
debris, organic contaminants and rock fragments larger than 6 inches.

Structural fill should be mechanically compacted to a firm, non-yielding condition. Structural
fill placed in building areas should be compacted to at least 95 percent of MDD. Pavement area
fill, including utility trench backfill and fill to support walkways should be compacted to at least
90 percent of MDD, except for the upper 2 feet below finished subgrade surface, which should be
compacted to at least 95 percent of MDD.
Structural fill should be placed in loose lifts not exceeding 8 to 10 inches in thickness. Each
lift should be conditioned to the proper moisture content and compacted to the specified density
before placing subsequent lifts. We recommend that a representative from our firm be present to
perform in-place moisture-density tests in the fill to evaluate whether the compaction
specifications are being met, and advise on any modifications to procedure which might be
appropriate for the conditions encountered.
Temporary Slopes
We anticipate that un-shored temporary cuts will be used. for the footing and pile cap
excavations. The stability of cut slopes is a function of soil type, ground water conditions, slope
inclination, slope height and nearby surface loads. Oversteepened temporary cut slopes could
impact the stability of adjacent work areas and existing utilities, and endanger personnel. All cut
slopes and temporary excavation support, if necessary, must be constructed or installed, and
maintained in accordance with the requirements of the appropriate governmental agency.
We expect that temporary near-vertical cut slopes can be used for portions of the excavations
on the east side of the building where dense till or hard silt was encountered. For excavations
deeper than 4 feet but less than about 8 feet in the dense till and hard silt, the lower 4 feet can be
near-vertical and the slope above cut to 1H:1V (horizontal to vertical). Temporary cut slopes for
excavations on the west side of the building, and in the interior of the building where existing fill
is likely present in the proposed foundation areas should be inclined no steeper than 1-1/2H:1V.
The above guidelines assume that surface loads such as equipment loads and materials storage
will be kept a sufficient distance away from the top of the cut so that the stability of the
excavation is not affected. We recommend that surface loads be kept away from the top of the
cut, a distance at least equal to the height of the cut. The guidelines also assume that the excava-
tions will be dewatered in such a.way that significant seepage is not present on the slope face.
Flatter slopes and/or shoring may be necessary for portions of the excavation where there is
significant seepage in order to maintain the stability of the cut.
It should be expected that the excavation face in the fill soil will experience some sloughing
and raveling. Berms, swales, or drainage ditches should be installed around the perimeter of the
excavation to intercept surface runoff and reduce the potential for sloughing and erosion of the cut
slope.

Permanent Cut and Fill Slopes
We recommend that permanent cut and fill slopes be constructed at 2H:1V, or flatter. Flatter
slopes might be considered for ease of maintenance.
Unprotected cut and fill slopes will be subject to erosion until a protective vegetative cover is
well established. Therefore, we recommend that slope surfaces be mulched and planted as soon
as practical to minimize the potential for erosion.
Appropriate drainage measures, as described below under the "Drainage Considerations"
section of this report, should be implemented to collect and control surface runoff and ground
water seepage.
Wall Backfill
We recommend that a minimum 12-inch-thick layer of free draining crushed rock or sand and
gravel with less than 3 percent fines be used for backfill adjacent to existing building walls. The
zone of free draining backfill should extend from the base of the wall to within 1 foot of the
finished ground surface. The upper 1 foot should consist of relatively impermeable on-site soil or
be capped with pavement to reduce surface water infiltration. We expect that an existing drainage
system is in place. This system should typically include smooth-walled, perforated drainpipe
having a minimum diameter of 4 inches which is embedded within the zone of free-draining
material at the base of the wall along its entire length. This drainpipe should discharge to a
tightline collection system.
We recommend fill within 5 feet of the building walls be compacted to between 90 and
92 percent of MDD except for the upper 2 feet which should be compacted to at least 95 percent
of MDD if supporting pavement or walkways (in accordance with ASTM D-1557).
Overcompaction near the wall should be avoided to prevent build-up of excessive lateral pressures
on the wall:

FOUDATION SUPPORT
Shallow Foundation Support
Footing Design. We recommend that shallow foundations be used to support the anticipated
design loads where dense and hard native soils are encountered at foundation depth. We
understand that shallow foundations will be located at the same elevation as the existing building
foundations. We recommend that exterior column footings be founded at least 18 inches below
lowest adjacent finish grade. Interior column footings should be found at least 12 inches below
the bottom of the adjacent floor slab. Column footings should be at least 3 feet wide. Footings
supported as recommended may be designed for an allowable bearing pressure of 7,500 pounds
per square foot (pst) for the total of all dead and live loads. This value is exclusive of the weight
of the footing and any overlying backfill. The allowable bearing pressure may be increased by
one-third when considering wind or seismic loads.

Based on our understanding of the proposed building loads,we expect that footings will be
about 6 feet square. We therefore estimate the total settlement of footings supporting the proposed
addition should be on the order of 1/2 inch and postconstruction settlements should be less than
about 1/2 inch. Postconstruction differential settlements between comparably loaded column
footings are not expected to exceed 1/2 inch.
Lateral Resistance. The available resistance to lateral foundation loading is a function of the
• frictional resistance that can be developed on the base, and the passive resistance that can develop
on the face of below-grade elements. The allowable frictional resistance for foundation elements
may be computed using a coefficient of friction of 0.35, applied to vertical dead-load forces. The
allowable passive resistance on the face of footings or other embedded foundation elements may
lie computed using and equivalent fluid density of 300 pounds per cubic foot (pcf) (triangular
distribution) for structural fill if all soil extending out from the face of the foundation element for
a distance at least equal to two and one-half times the height of the element consist of structural
fill compacted to at least 95 percent of MDD. The above coefficient of friction and passive
equivalent fluid density value includes a factor of safety of about 1.5.
Deep Foundation Support
General. We expect that deep foundations will be required for support of much of the
building where existing fill is present at the anticipated foundation depth. The following sections
summarize our recommendations for 14- and 18-inch diameter auger-cast piles and 30-inch
diameter drilled shafts. For both systems of foundation support the piles/shafts must penetrate to
at least a depth of 20 feet and into the dense native soil at least 10 feet. We understand that
auger-cast piles will likely be used for the project because their installation will be more suitable
for existing site conditions.
Auger-Cast Piles. We recommend that auger-cast piles extend to a depth of at least 20 feet
and at least 10 feet into the dense and hard native soil. Auger-cast piles may be designed for the
following values:

These values include a factor of safety of about 2.5. These capacity values may be increased by
one-third when considering design loads of short duration such as wind or seismic forces.
The allowable axial capacities presented above are based on the strength of the supporting
soils for the penetrations indicated and ignore the weight of the pile. The capacities apply to
single piles. If piles within groups are spaced at least 3-pile diameters on center, no reduction for
pile group action is required.

Allowable lateral loads on closely spaced piles or other subgrade elements are, in part, a
function of the spacing between these elements. We recommend a minimum pile spacing of
3-pile diameters center-to-center, both parallel and normal to the principal direction of load
application. Allowable lateral capacities for various pile spacings, expressed in terms of D (pile
diameter) parallel to the direction of load application are tabulated below. Allowable lateral
capacities are presented for various pile head deflections.

We recommend that reinforcing sufficient to resist the expected bending moments be installed full
depth in the 14- and 18-inch-diameter auger-cast piles.

Drilled Shafts. We understand that drilled shafts will likely not be used to support the
structure. However, the following paragraphs of this report present conclusions and
recommendations for drilled shafts that were presented in our preliminary memorandum dated
May 3, 1998. This information is presented here for reference only. If the configuration of the
proposed structure changes and drilled shafts are considered further for support of the structure,
we should be contacted to provide further evaluation as necessary.
We conclude that 30-inch diameter drilled shafts extending to a depth of at least 20 feet and at
least 10 feet into the dense and hard native soil may be designed for an allowable axial capacity of
130 tons. An uplift capacity of 30 tons may be used for_the drilled shafts. These values include a
factor of safety of about 2.5. These capacity values may be increased by one-third when
considering design loads of short duration such as wind or seismic forces.
The allowable axial capacities presented above are based on the strength of the supporting
soils for the penetrations indicated and ignore the weight of the shaft. The capacities apply to
single shafts. If shafts within groups are spaced at least 3-shaft diameters on center, no reduction
for group action is required

The above lateral capacities are based on shaft-head fixity against rotation and a maximum shaft
deflection of approximately 1/2 inch.. We recommend that reinforcing sufficient to resist the
expected bending moments be installed full depth in the 30-inch-diameter drilled shafts.
Settlement. We estimate that the total settlement of auger-cast piles designed and installed as
recommended will be about '/ inch. Most of this settlement is expected to occur rapidly as loads
are applied. Postconstruction differential settlements between adjacent columns should less than
'/;inch, depending upon the distribution of building loads.
Installation. We recommend that auger-cast piles be installed by an experienced contractor
to the recommended penetration using a continuous-flight, hollow-stem auger. The pile is
constructed by pumping grout under pressure through the hollow stem as the auger is withdrawn.
Reinforcing steel for bending and uplift loads is placed in the fresh grout column immediately
after withdrawal of the auger. We recommend that installation of piles be sequenced to avoid

disturbance of fresh grout in previously cast piles. We recommend that minimum 3,500 pounds
per square inch (psi)grout strength be used for auger-cast piles.
Grout pumps must be fitted with a volume-measuring device and pressure gauge so that the •
volume of grout placed in each pile and the pressure head maintained during pumping can be
measured. A minimum grout line pressure of 100 psi should be maintained. The rate of auger
withdrawal should be controlled during grouting such that a minimum head of 10 feet of grout is
maintained above the auger tip during withdrawal and the volume of grout pumped into the drilled
hole is equal to at least 115 percent of the theoretical hole volume.
I We recommend that pile installation be monitored by a member of our staff who will observe
the drilling operations, record indicated penetrations into the supporting soils, monitor grout .
injection procedures, record the volume of grout placed in each pile relative to the calculated ,
volume of the hole, and evaluate the adequacy of individual pile penetrations.
RECONSTRUCTION OF FLOOR SLAB AREAS
We recommend that new floor slabs constructed to replace slabs removed during construction
be supported on structural fill compacted to at least 95 percent of MDD as recommended above
under Site Preparation. We did not complete explorations in the existing floor slab area but
expect that the floor slab is supported on a capillary break and may include a vapor retarder
.(plastic sheeting. We recommend that new floor slab be constructed on a base course consisting
of at least 4 inches of well-graded sand and gravel or crushed rock containing less than 3 percent
fines. The base course will provide uniform support and serve as a capillary break to reduce
moisture migration through the slab. If a vapor retarder is present it should be replaced at areas
disturbed'during construction. A 2-inch-thick layer of sand should be placed over the vapor
retarder to protect it during slab construction and to aid in uniform curing of the concrete.
SEISMICITY
General
The Puget Sound area is a seismically active region and has experienced thousands of
earthquakes in historical time. Seismicity in this region is attributed primarily to the interaction
between the Pacific, Juan de Fuca and North American plates. The Juan de Fuca plate is
subducting beneath the North American Plate. Each year 1,000 to 2,000 earthquakes occur in
Oregon and Washington. However, only 5 to 20 of these are typically felt because the majority
of'recorded earthquakes are'smaller than Richter magnitude 3.
In recent years two large earthquakes occurred which resulted in some liquefaction in loose
alluvial deposits and significant damage to some structures. The first earthquake, which was
centered in the Olympia area, occurred in 1949 with a Richter magnitude of 7.1. The second
earthquake, which occurred in 1965, was centered between Seattle and Tacoma and had a Richter
magnitude of 6.5.

Uniform Building Code (UBC) Site Coefficient
The Puget Sound region is designated as a Seismic Zone 3 in the 1994 and 1997 editions of
the Uniform Building Code (UBC): For Zone 3 locations, a Seismic Zone Factor (Z) of 0.30 is.
applicable. In our opinion, the soil profile at the.site is best characterized as Type S2 (1994
UBC)or Type Sc (1997 UBC).
Design Earthquake Level
The key seismic design parameters are the peak acceleration and the Richter magnitude of the
earthquake. In general, a design earthquake is chosen based on a probability of exceedence (the
probability that the design earthquake will not be exceeded over a given time period). The level
of seismicity recommended in the 1994 edition of the UBC for human occupancy buildings is an
earthquake with a 10 percent probability of exceedence in a 50-year period. The design
earthquake event that corresponds to this probability of exceedence is an earthquake with a
Richter magnitude of 7.5 and a peak horizontal ground acceleration of approximately 0.3g.
Liquefaction Potential
Liquefaction is a condition where soils experience a rapid loss of internal strength as a
consequence of strong ground shaking. Ground settlement, lateral spreading and/or sand boils
may result from soil liquefaction. Structures supported on liquefied soils can suffer foundation
settlement or lateral movement that may be severely damaging to the structures.
Conditions favorable to liquefaction occur in loose to medium dense, clean to moderately silty
sand that is below the ground water table. This condition is present at this site. Therefore, we
performed an engineering evaluation of the liquefaction potential of the site soils.
The evaluation of liquefaction potential is dependent on numerous parameters including soil
type and grain size distribution, soil density, depth to ground water, in-situ static ground stresses,
and the earthquake induced ground stresses. Typically, the liquefaction potential of a site is
evaluated by comparing the cyclic shear stress ratio induced by an earthquake with the cyclic
shear stress ratio required to cause liquefaction. The cyclic shear stress ratio required to cause
liquefaction was estimated using an empirical procedure based on the in-situ static ground
stresses, the blow count data obtained during sampling in the borings, and the design earthquake
magnitude.
The results of our explorations and analyses indicate that the loose to medium dense silty sand
encountered in the borings has a low potential for liquefaction during an earthquake with a
Richter magnitude of 7.5 or greater.
DRAINAGE CONSIDERATIONS
We expect that shallow perched groundwater may be encountered during foundation
excavation. We anticipate that this water can be temporarily handled during construction by
ditching and pumping from sumps, as necessary. All collected water should be safely routed to
suitable discharge points.

In our opinion, additional foundation drainage systems are not necessary since the structure
likely has a perimeter drainage system in place. However, construction activities will likely
disrupt or damage the existing system and careful attention should be directed toward protecting
or repairing the existing system before backfilling foundations. This is especially important on
the east side of the building where below grade walls will be exposed during construction and
backfilled following foundation construction. A 12-inch wide zone of drain material should be
placed against the building walls to intercept drainage. Additional recommendations for wall
1 ackfill are discussed above under Wall Backfill in the Site Preparation and Earthwork Section of
this report.
LIMITATIONS
We have prepared this report for use by Valley Medical Center, Mahlum Architects, SWMB
and other members of the design team for use in the design of a portion of this project. The
conclusions and recommendations in this report should be applied in their entirety. The data and
report should be provided to prospective contractors for bidding or estimating purposes; but our
report, conclusions and interpretations should not be construed as a warranty of the subsurface
conditions.
If there are any changes in the grades, location, configuration or type of construction planned,
the conclusions and recommendations presented in this report might not be fully applicable. If
such changes are made, we should be engaged to review our conclusions and recommendations
and to provide written modification or verification, as appropriate. When the design is
finalized, we recommend that we be engaged to review those portions of the specifications
and drawings that relate to geotechnical considerations to see that our recommendations
have been interpreted and implemented as intended.

There are possible variations in subsurface conditions between the locations of explorations.
Variations may also occur with time. Our explorations were completed outside the building
footprint and some of the proposed foundation work will be completed within the building. Some
contingency for unanticipated conditions should be included in the project budget and schedule.
We strongly recommend that sufficient monitoring, testing and consultation be provided by our
firm during construction to (1) determine if the conditions encountered are consistent with those
indicated by the explorations, (2) provide recommendations for design changes should the
conditions revealed during the work differ from those anticipated, and (3) evaluate whether or not
earthwork and foundation installation activities comply with the contract plans and specifications.

accordance with generally accepted practices in this area at the time the report was prepared. No
warranty or other conditions, express or implied, should be understood.

. FIELD EXPLORATIONS AND LABORATORY TESTING
FIELD EXPLORATIONS
We explored subsurface soil and groundwater conditions at the site by drilling 6 borings to
depths of about 6.5 to 21.5 feet below the existing ground surface. Gregory Drilling of
Redmond, Washington drilled two borings (B-1 and B-2) on April 8, 1998 using truck-mounted
hollow-stem auger drilling equipment. CN Drilling of Seattle, Washington drilled four borings
(B-3a, B-3b, B-3c and B-4) on April 8, 1998 using portable solid flight auger drilling equipment
to allow access to landscaped portions of the site. Locations of the explorations were determined
in the field by measuring distances from existing site features. Ground surface elevations shown
on the exploration logs are based on topographic information contained in previous reports
provided to us for this study (see Site Description section of this report for listing of previous
reports). The locations of the explorations are shown on the Site Plan, Figure 2.
The explorations were continuously monitored by a representative from our firm who visually
examined and classified the soils encountered, obtained representative soil samples, observed
ground water conditions, and prepared a detailed log of each exploration. A 2.4-inch inside-
diameter, heavy-duty split-barrel sampler with brass liner rings was used to obtain relatively
undisturbed samples from the borings (B-1 and B-2) completed by Gregory Drilling. The blow
counts resulting from driving this sampler with a 300-pound hammer falling 30 inches area
roughly equivalent to those from the Standard Penetration Test. A 1.5 inch inside-diameter
standard split spoon sampler was used to obtain disturbed samples form borings(B-3a, B-3b, B-3c
and B-4) completed by CN Drilling. The number of blows required to drive the samplers the last
12 inches, or other indicated distance, is recorded on the boring logs. Soils encountered were
visually classified in general accordance with the classification system described in Figure A-1.
A key to the boring log symbols is presented in Figure A-2. The boring logs are presented in
Figures A-3 through A-8. The logs are based on our interpretation of the field data and indicate
the various types of soils encountered. They also indicate the depths at which the soils or their
characteristics change, although the change might actually be gradual. The densities noted on the
boring logs are based on the blow counts. The borings were backfilled in general accordance
with local regulatory requirement.
LABORATORY TESTING
Selected soil samples obtained from the test pits were returned to our lab and visually
examined to confirm or modify field classifications. Selected soil samples were tested to
determine their natural moisture content and dry density. The results of the moisture content and
density determinations are presented on the boring logs.
