VALLEY MEDICAL CENTER TANK REPLACEMENT

The applicant seeks approval to replace two existing underground tanks at Valley Medical Center. Tank one, an
existing 500 gallon single-walled gasoline storage tank,will be removed and replaced with a new 1,000 gallon
double-walled tank. Tank two,an existing 20,000 gallon single-walled,fuel oil storage tank,will be abandoned or
removed and a new 20,000 gallon double-walled tank will be installed. The proposal includes transfer of fuel
. between tanks, cleaning and filling of abandoned tank, and street, sidewalk, and floor slab patching as
necessary. Contaminated.soils, if any,will be properly disposed of off-site: Location: 400 South 43rd Street.
Appeals of the environmental determination must be filed in writing on or before 5:00 PM October 12, 1998.
Appeals must be filed in writing together with the required $75.00 application fee with: Hearing Examiner, City of Renton,
1055 South Grady Way, Renton, WA 98055. Appeals to the Examiner are governed by City of Renton Municipal Code

In compliance with RCW 43.21 C. 240, the following project environmental review addresses only
those project impacts that are not adequately addressed under existing development standards and
environmental regulations.

conjunction with the proposed development?
1. Earth
Impacts: The proposal includes the excavation of soils surrounding the tanks to be removed as well as any
contaminated soils in the immediate area, if any. Excavation will be backfilled and compacted with suitable fill
material free of organic material and debris. The site will be restored to its original grade. The applicant's Temporary
Erosion and Sedimentation Control Plan will serve to mitigate any potential impacts.
Mitigation Measures: No further mitigation is recommended.

2. Water
Impacts: According to the applicant's Environmental Checklist, groundwater is anticipated at a depth of
approximately six to eight feet below current site grade. If water enters the areas of excavation,the contractor will
pump the excavation dry before backfilling. Water pumped from the excavations will be collected, contained, tested
and disposed of in accordance with the test results. The applicant's Temporary Erosion and Sedimentation Control
plan will serve to mitigate any potential impacts to surface water and storm water.
Mitigation Measures: No further mitigation is recommended.
Nexus: N/A
3. Environmental Health
Impacts: All soils excavated from the site will be tested for contamination and, if contaminants are found, will be
properly disposed of in an approved off-site location. Upon excavation, soils will be stockpiled on plastic bottom liners
and covered with plastic liners. The top and bottom liners will be folded together and secured to prevent infiltration of
surface water into the piles. Excavated soils will be segregated in "contaminated"and "non-contaminated"stockpiles.
Potential environmental health impacts may result if contaminants are found, however, these impacts will be
mitigated by the Model Toxics Control Act(MTCA).
Mitigation Measures: No further mitigation is recommended.

August 18, 1998
This proposed project at Valley Medical Center, entitled"Underground Tank Replacement", consists of
replacement of two single-walled underground storage tanks with two double-walled underground
storage tanks.
The site is a multi-acre parcel located north of South 43rd Street,bounded on the east by Talbot Road
South, on the south by S.W.43rd St., and on the west by Highway 167. The site is currently used as a
medical center.
There are no special site features such as wetlands,bodies of water, or steep slopes.
No existing views will be obstructed as a result of this project. At the completion of this project, all new
work will be underground.
An existing single-walled, 20,000-gallon underground fuel oil storage tank will be abandoned in place
and a new double-walled,20,000-gallon underground storage tank with environmental monitoring will
be installed in the same area of the medical center.
Also as part of this project, an existing single-walled, 500-gallon underground gasoline storage tank will
be removed and replaced with a new double-walled, 1,000-gallon underground gasoline storage tank
with environmental monitoring in the same location.

CONSTRUCTION MITIGATION DESCRIPTION
August 20, 1998
This proposed project at Valley Medical Center, entitled"Underground Tank Replacement",will begin
construction approximately September 28, 1998 and continue for 30 calendar days, ending
approximately October 27, 1998.
Hours of construction are expected to be from 6 am to 6 pm.
Materials and equipment will be transported on Valley Medical Center private access roads from S.W.
43rd Street and Highway 167.
The site is located on the western edge of the VMC complex and is located quite far away from non-
hospital buildings. See site plan attached. Only hospital employees should be affected by any dust,
traffic or noise impacts. The contractor will take appropriate measures as outlined in the Construction
Specifications to minimize erosion or mud at the project site.
Some deliveries may take place during weekend hours. However,the Medical Center operates 24 hours
a day, 7 days a week, and has daily deliveries on a routine basis. Any weekend deliveries will be routed
on major arterials and VMC access roads only.
Construction traffic will be routed on major arterials and VMC access roads. Therefore,no traffic
control plan is needed.

PART 1 GENERAL
1.1 USE OF DOCUMENTS
A. Divisions 0 and 1 apply to the work specified in this section.
1.2 DESCRIPTION
A. Project Identification and Location: Refer to Section 00002, Introduction.
B. Form of Contract: The work shall be performed under a single General Contract, including
mechanical and electrical work, as specified in Divisions 0, 1, 2, 15 and 16, and as shown on
drawings in Division C, M and E..
C. Governing Regulations: The work shall be performed in accordance with applicable codes,
ordinances and regulations.
D. Special Precautions:The Contractor will be performing the work in occupied and unoccupied
areas of the Building. Work in occupied areas shall be done off-hours; evenings and
weekends. After each work "day" before area occupants return for the normal work day, the
areas of construction shall be thoroughly cleaned. Ceiling tiles shall be replaced, and
temporary covers installed over other ceiling, soffit and wall openings. Work on .site that
disrupts roads and access shall be done on weekends. Loading dock access shall be
maintained throughout project. All excavation shall be completely covered and secured during
the week. It is a part of this contract that the Contractor provide necessary barriers and
protection to keep noise, dust and any other disturbance, inconvenience or otherwise to a
minimum. The Contractor will rent or purchase tools to reduce or minimize noise and
disturbance, as required. All trades will comply with this requirement as a part of this
Contract. The Contractor shall immediately reschedule work that impairs Building operations
to time agreeable with the Owner. Work in adjacent occupied spaces shall be coordinated with
the Owner at least one week in advance of the work.
1.3 WORK SUMMARY
A. Building Overview
1. Project Scope includes installation of two underground storage tanks, removal of one,
and abandonment of one for Valley Medical Center. Contractor is responsible for
installation, and for providing all work and material in accordance with all applicable
codes and ordinances.
2. The Project Area is located on the Southwest corner of the Hospital site off Talbot Road
South.
3. Groundwater may be present at a depth of 8 to 9 feet below the current site grade.
4. The subsurface geology of the site is generally composed of silty sands.
B. The Work Includes:
1. Installation of two owner-furnished underground storage tanks: (1) 20,000 gallon fuel oil
tank; and (1) 1,000 gallon gasoline tank. Removal and disposal of one 500 gallon
gasoline tank. Abandonment in place including fuel transfer to a new tank, cleaning of
old tank and filling old tank with slurry of one 20,000 gallon fuel oil tank.
2. Provide all excavation, backfill, and street, sidewalk and floor slab patching required for
the installation of the 2 new tanks and abandonment of one and removal of one tank.
3. Work includes furnishing all labor, equipment. materials, supplies, and incidental items
necessary to dispose of water, product, and residue; remove, dismantle, purge,
decontaminate, haul away, and dispose of the 500 gallon underground storage tank

(UST), associated piping, auxiliary equipment and debris. Work also includes
abandonment in place of the 20,000 gallon UST, removal and disposal of concrete and
asphalt, and backfilling excavations. Engineer will provide an independent site assessor
to observe and document the work.
4. Locate and turn off utilities(i.e. gas,water,electrical) during the life of the project;
The Contractor shall be responsible for all utility locates and turning off all utilities prior
to excavation. Contractor shall exercise care during excavation and shall alert Engineer
and site assessor of undocumented utilities, if found. Provide adequate means of
protecting active utilities and storm drains during excavations operations. Inactive
buried power lines exposed shall be disconnected'and removed. Inactive water and
sanitary sewer lines exposed shall be isolated, capped, and abandoned in place.
Damage or destruction to any of the utilities shall be repaired at no additional cost to
owner.
5. Secure site;
Provide temporary construction fence to identify exclusions zones and around work
areas. Limit access to work areas to only health and safety trained and authorized
personnel identified in the Health and Safety Plan.
6. Remove and dispose of the liquids and sludge in all the tanks;
Remove and dispose liquids and sludge in accordance with federal, state, and local
regulations.
The tank and piping contents shall be removed and disposed by the contractor.
Clean and rinse the tanks, and piping in accordance with federal, state and local
• regulations regarding UST cleaning. Specific attention shall be directed toward
controlling sources of ignition, electrically isolating the tank, and vapor freeing the tank.
Initial cleaning shall be performed from outside the tank, and entry to the tank, if
required, shall be tightly controlled per the contractor's UST Health and Safety Plan.
Transport hazardous waste solutions using the required manifests and a licensed
hazardous waste hauler. Haul and dispose of non-hazardous waste solutions in
conformance with the requirements of federal, state, and local regulations.
Product,sludge or cleaning fluids shall not be mixed with soil for disposal.
7. Remove and dispose of all asphalt and concrete necessary to access tanks, fuel
lines and excavations;
Cut, demolish, remove and dispose all pavement that lies wholly or partially within the
designated limits of the excavations. Relocate support utilities (such as storm and
sanitary sewers), if necessary, and other obstructions to UST removal that are not
designated for demolition under the active construction phase.
Remove concrete or asphalt pavement in excavation areas, not more than three days
prior to excavation at a given location.
8. Excavate and Stockpile Soils;
Provide shoring, if necessary, or slope excavations to remove tanks and to prevent the
sides of the excavation from collapsing into the excavation (500 gallon UST).
Excavate as required to remove the 500 gallon UST and any contaminated soil around
the tank. Soil surrounding existing 500 gallon UST shall be temporarily stockpiled on

plastic sheeting within the limits of work in the vicinity of each excavation until receipt of
analytical test results of samples taken by the independent site assessor. Select
methods and equipment to remove soil to minimize disturbance of areas beyond the
limits of the excavation area. Material that becomes contaminated as a result of the
Contractor's operations shall be removed and disposed at no additional cost to Owner
and replacement backfill shall be provided at no additional cost to Owner.
Excavated soil will be segregated by use of field screening procedures by the
independent site assessor and stockpiled in temporary "contaminated" and "non-
contaminated"stockpiles.
Temporary stockpiles for contaminated soils shall at a minimum, meet the following
requirements. Contractor shall prepare stockpile areas by placing plastic bottom liner
over the pavement or unpaved areas where soil will be placed. The liner must extend at
least 5 feet beyond any edge of the stockpiled material. The preparation may be
accomplished in phases as the project proceeds. The surface upon which the liner will
be placed shall be free of large rocks, stones, and other debris which may damage the
liner. Contractor shall use overlapping single liner sheets for temporary stockpiles.
f
At the end of each work day and during inclement weather, temporary stockpiles shall be
covered with plastic liners. The top and bottom liners shall be folded together to prevent
infiltration of surface water into the piles and the migration of leachate from the piles.
The top liners shall be secured.
Contractor shall uncover contaminated soil stockpiles only when soil is moved.
Stockpile top liner and hold down systems shall be placed on the soil immediately
following loading or unloading activities.
Equipment operating in lined stockpile areas shall have rubber tires to prevent tearing of
liner materials and shall avoid driving directly on bottom liner.
The contractor shall maintain stockpiles until final disposal of materials for the duration
of the work so that no precipitation or surface water contacts the material and so that pile
erosion by wind, rain or surface water shall not occur.
9. Remove and dispose of the 500 gallon underground storage tank at the site and
abandon in place the 20,000 gallon underground storage tank;
Before removing the 500 gallon UST, the contractor shall establish an inert atmosphere
in the tanks by federal, state or locally approved methods. Prior to abandoning the
20,000 gallon tank in place, the contractor shall remove all residual product and/or
sludge and clean the interior of the UST by federal, state and locally approved
procedures for UST abandonment
After completing all the necessary preparation, the contractor shall remove the 500
gallon underground storage tanks from the excavations according to API standard 1604
and all federal, state, and local regulations and abandon the 20,000 gallon UST in
accordance with all federal, state, and local regulations.
After the 500 gallon tank has been removed from the excavation, the tank exterior shall
be scraped to remove soil and mud. The tank will be inspected for corrosion damage
and breaches prior to transporting from the he site. The tank shall be labeled before
leaving the site.
The 500 gallon Tank shall be transported from the facility the same day they are
removed from the excavation. The removed tank shall not be stored overnight.

Haul the removed 500 gallon tank and associated materials for disposal to a state-
approved disposal site. Provide copies of all certificates of acceptance for the tank and
piping materials provided by the firms accepting these materials for final disposal.
After removing the 500 gallon tank, the Contractor shall excavate additional soil as
necessary to allow the independent site assessor to sample excavations in accordance
with Washington department of Ecology guidelines.
10. Remove and dispose of the fuel lines;
Distribution piping shall be removed. Extreme care shall be exercised not to undermine
other utilities or building foundations. Distribution piping not removed shall be capped or
plugged with Portland cement grout a distance of at least four times the outside diameter
of the pipe.
Haul removed piping to a state-approved disposal sit. Provide copies of all certificates
of acceptance of the piping materials provided by the firms accepting these materials for
final disposal.
11. Provide all the groundwater removal and disposal services necessary to complete
the project; .
Contractor shall prevent surface water from flowing into the excavation and excavate
only in dry weather when possible. Groundwater is anticipated at a depth of
approximately six to eight feet. Groundwater should be anticipated during tank removal.
If water enters the excavation, the contractor shall pump the excavation dry before
backfilling. Water pumped from the excavations shall be collected, contained, tested
and disposed in accordance with the test results. Excavation dewatering shall be
performed by the contractor at no additional expense to the Owner. Treatment and/or
disposal cost shall be at the unit rates provided on the bid summary form.
The Contractor shall immediately inform the independent site assessor if there is any
indication that the groundwater may be contaminated. Groundwater contamination is
suspected if an oily sheen is observed or if MICA Method A standards are exceeded.
12. Provide backfill, compaction and soil density testing for all excavations onsite;
Excavations shall be backfilled and compacted with suitable fill material free of organic
material and debris. The site shall be restored to its original grade. Contractor shall
compact all soil placed in the excavations and provide all soil density testing necessary
to confirm the soil has been compacted to 90% of the modified proctor dry density in
accordance with ASTM 1557-78.
13. Replace and compact asphalt and replace concrete paving removed during
project;
14. Tank Closure Documentation
The Contractor shall submit three copies of all records to the Engineer within 15 calendar
days after completion of the tank removal from the ground. The Tank Closure Record
will include the following:
A) Thirty day closure notification and permanent closure check list.
B) City of Renton Fire Department approval stating that the UST has been properly
prepared for closure.

15. Quality Control I
Ir The Contractor shall be fully licensed with the Washington Department of ecology as a
tank service provider, under WAC 173-360-630, and employ a tank service supervisor
licensed to perform UST decommissioning, under WAC 173-360-650. The licensed tank
11 service supervisor shall be present on site when tank removal procedures are underway,including excavation around the tank, tank cleaning, tank purging or inerting, and
removal of the tank, associated piping and appurtenances. The licensed tank service
supervisor shall complete a Ecology permanent UST closure checklist subsequent to

Assessor.
The Contractor performing the work must employ workers that have received health and
safety training as required in Chapter 296-62 WAC, General Occupational Health
Standards, and medical monitoring in accordance with the Occupational Safety and
Health Act and Washington Industrial Safety and Health Act (WISHA).

1.3 OWNER RESPONSIBILITY
A. The abatement of hazardous materials shall be the responsibility of the Owner.
B. Any questions that arise related to hazardous materials and their abatement work shall be
referred to the Owner for resolution.
C. The Owner will bear the cost of hazardous materials abatement by a certified hazardous
materials abatement contractor.
1.4 CONTRACTOR RESPONSIBILITY
A. If any hazardous materials abatement must be undertaken in the course of performing
Contractor's work, Contractor shall discuss the timing of such abatement work with Owner and
plan for the same in Contractor's work schedule prior to commencing any work on the project.
B. Contractor shall not remove, disrupt or disturb any hazardous materials at any time. The
Contractor shall have on site an employee who is certified for asbestos recognition during all
phases of work.
C. In the event that Contractor encounters any previously unidentified hazardous materials, the
Contractor shall immediately suspend work and inform the Owner when materials suspected of
being hazardous are encountered at the project site.
