Reviewed By LAC Initials/Date SSR Initials/Date Field Supervisor Project Manager Limitations: The presence of our field representative at the site is to provide our client with a source of professional advice, opinions, and recommendations based upon the field representative’s observations and testing of the contractor’s work. Our services do not include supervision or direction of the contractor, their employees, or agents. Geotechnical recommendations for obtaining project objectives may be made by our representatives; however, direction of the actual work should come from the owner or contractor, as appropriate. The contractor is responsible for complying with the contract documents at all times, regardless of the presence of our field representative. Jobsite safety, including compliance with all applicable state or federal regulations, is the sole responsibility of the contractor. The observations, recommendations, and conclusions provided in this field report are preliminary until reviewed by the ESNW project manager. Per client request, ESNW rep arrived onsite at the Weatherly Inn development to observe and document wall subgrade excavation and slope conditions. While onsite the following was observed: Cast-In-Place Wall Excavation Observations: Upon arrival ESNW rep met with the onsite contractor (Brad, Johansen) and observed the contractor was beginning excavation for the planned 13’ concrete cast in place wall in the location noted on the page (2) plan detail. Following initial excavation, ESNW rep observed the contractor encounter two (2) existing ductile iron water service utility pipes within the area of planned wall excavation. Following later exposure of the water utility lines and discussion with the contractor, ESNW rep was informed that further excavation would be placed on hold until temporary shoring of the existing utilities can be planned. The contractor later informed ESNW rep that additional excavation around the existing utilities would be carried out before end of day. ESNW rep returned at the end of day and observed that the contractor had exposed a 6” and 4” ductile iron water service utilities (pictured), and following discussion it was determined that excavation would resume tomorrow (2/17) morning, of which the contractor would fully unearth all water utilizes and remove existing structure footing backfill to reduce overall over-steepened slope height, ESNW rep will return onsite tomorrow morning to observe and document ongoing wall excavation and slope stability. Eco-Block Wall Stability Observations: ESNW rep further observed that the 6-block high eco block wall within the excavation remained stable and unchanging in its current configuration. The wall was observed to be stable and unchanging in its current temporary configuration, however it is the responsibility of the contractor to perform all necessary safety measures in accordance with state and federal regulations. It is recommended that the contractor inform ESNW if any changes in the slope conditions occur. Please refer to the attached plan detail and photo documentation provided below.
