HomeMy WebLinkAboutAddendum - 1AMENDMENTNO.1TOAGREEMENTFORAGE-FRIENDLYRENTONACTIONPLANANDRESOURCEWEBPAGEDEVELOPMENTTHISAMENDMENT,datedforreferencepurposesonlyasMarch29,2023,isbyandbetweentheCityofRenton(the“City”),aWashingtonmunicipalcorporation,andVictoriaKvitekofLomaxKvitek(“Consultant”),asoleproprietor.TheCityandtheConsultantarereferredtocollectivelyinthisAmendmentasthe“Patties.”OncefullyexecutedbytheParties,thisAmendmentiseffectiveasofthelastdatesignedbybothparties.Whereas,theCityengagedtheservicesoftheConsultantunderAgreementCAG-23-026,dated1/12/2023,toprovidenecessaryservicesfortheAge-FriendlyRentonActionPlanandResourceWebpageDevelopment.(referredtohereinasthe“Agreement”);Whereas,thePartieswishtoamendtheAgreementtochangethescopeofwork,changethetimeforperformance,andchangethecompensationinordertocompletethenecessaryAge-FriendlyRentonservices.NOWTHEREFORE,ItismutuallyagreeduponthatCAG-23-026isamendedasfollows:1.ScopeofWork:Section1,ScopeofWork,isamendedtomodifytheWorkasspecifiedinExhibitA,whichisattachedandincorporatedherein.2.TimeofPerformance:Section3,TimeofPerformance,isamendedpursuanttotheschedule(s)setforthinExhibitA.AllWorkshallbeperformedbynolaterthan1/31/2024.3.Compensation:Section4,Compensation,isamendedsothatthemaximumamountofcompensationpayabletoConsultantisincreasedby$7,500from$15,000to$22,500,plusanyapplicablestateandlocalsalestaxes.TheadditionalcompensationshallbepaidbaseduponWorkactuallyperformedaccordingtotherate(s)oramountsspecifiedinExhibitAoftheAgreement.4.AlltermsoftheAgreementnotexplicitlymodifiedhereinshallremaininfullforceandeffectandsuchtermsshallapplytoWorkperformedaccordingtothisAmendmentasiffullysetforthherein.INWITNESSWHEREOF,thePartieshavevoluntarilyenteredintothisAmendmentasofthedatelastsignedbythePartiesbelow.CITYOFRENTONCONSULTANTBy:______________________________By:fr1k.KellyBeymeVictoriaKvitekParksandRecreationAdministratorSoleProprietor13t/2-3__________DateDateCAG-23-026 Adden #1-23

EXHIBIT 1: SCOPE OF WORK
Objective – The objective of this modified contract is to assist in the development and
finalization of an Age-Friendly Renton resource webpage for seniors, and assist with quarterly
King County Veterans, Seniors, Human Services Levy grant reports. This will include the
following:
• Bi-monthly check-in meetings with project lead, to assess project status.
• Work with the City Recreation and Communications Divisions to develop Age-Friendly
resource webpage on city website.
• Highlight specific services/programs/resources for seniors
• Visual call-out of 8 domains utilizing graphic elements
• Link to completed Age-Friendly Renton Action Plan
• Assist with quarterly King County Veterans, Seniors, Human Services Levy grant reports.
Timeline: January 20, 2023 – January 31, 2024
Payment Rate: (billed hourly, $40/hour – not to exceed $22,500.
