1.0 PURPOSE:
To identify responsibility for Hazard Reporting and to outline steps to follow in reporting
hazardous situations and conditions that may endanger the health and safety of City
employees or members of the public.

4.0 POLICY:
Hazard reporting is part of the City’s Accident Prevention Program. Each employee has
the responsibility to report any potential or actual hazardous condition/act that may
endanger their health and safety or that of others. Employees may report hazards
anonymously.
Some workplace hazards may be effectively resolved at the department level by a
department representative such as a supervisor, team lead, or manager. In some
circumstances as outlined in 6.1.3, the department representative should notify Risk
Management in accordance with section 6.0.

5.0 DEFINITIONS:
Hazard: Any condition or action in your work environment that could lead to someone
developing an illness or someone being injured.
6.0 PROCEDURES:
6.1 Direct notification of a hazard
6.1.1 Employees may report hazards verbally, in writing, or by any effective
communication means to their supervisor, a representative on the Safety
Committee, or Risk Management.
6.1.2 If able to directly, effectively, and immediately resolve the hazard,
department representatives such as supervisors, team leads, or managers
may do so.
6.1.3 When Safety Committee members are notified of a hazard, they will
inform Risk Management directly or in accordance with section 6.2 of this
policy.
6.1.4 Risk Management must be notified in accordance with 6.2 or directly in
the following circumstances:
• The decision-making authority and/or resources needed to
resolve the hazard and/or prevent reoccurrence are outside the
scope of the department’s ability or authority, or,
• The hazard may lead to an acute injury or illness that poses an
immediate risk to a person's life or long-term health, or,
• The hazard is reoccurring.
Supervisors should err on the side of caution and notify Risk
Management if they are uncertain whether the circumstances of the
hazard meet the conditions above or if they would like to request
collaboration.
6.1.5 Do not report medical or other emergencies via the hazard report – dial
9-1-1 for immediate assistance.
6.2 Reporting a hazard through a Hazard Report
6.2.1 An employee may report a safety hazard by submitting a Hazard Report
through the Origami Risk Portal maintained by Risk Management for
Incident, Injury, and Hazard Reporting. Reporting through this method
may be done anonymously or with contact information.

6.2.2 Once notified through the Portal or otherwise, Risk Management will
investigate the reported hazard and document any action taken.
6.3 Reviewing reported and resolved hazards
6.3.1 The Safety Committee will review and discuss all submitted Hazard
Reports at the Safety Committee meetings.
6.3.2 Risk Management will communicate the conclusion of the investigation
to the employee who submitted the report, if contact information was
provided.
