Dear Gordy:
As of December 20, 2009, all major pump station components of the Denny's Lift Station have
been installed and tested. The lift station submersible pumps are now operating automatically.
Therefore, RH2 Engineering, Inc. has determined that Omega Contractors, Inc. has substantially
completed work on the Denny's Lift Station Replacement project in accordance with the
Contract Plans. Final Acceptance of this Contract is subject to completion of the following
items.
1. Complete the attached punchlist.
2. Provide final as -built and construction records.

SUBJECT: Entry Permission for Denny's Lift Station Rehabilitation Project
Dear Denny's Management:
The sewage lift station located in the Lake Washington Boulevard right-of-way adjaceni.
to your restaurant has become increasingly less reliable. In order to maintain reliable
service, the City of Renton will have a contractor do some rehabilitation and
improvements on the station. The majority of the work will be performed within City
right-of-way with all permanent improvements being in the right-of-way or within the
existing easement on your property. Work will include tasks interior to the existing
structure, excavation to install a new valve vault, and work on or around the above
ground electrical panel.
We are looking to start work on October 14, 2009, and are expecting to complete the
K work in three to four weeks.
In order to facilitate the work, the City would like to obtain permission from you to
temporarily utilize a portion of your property (as shown on the attached map) during
construction for the purpose of constructing the planned project improvements
associated with the Denn 's Lift Station Rehabilitation project.':t
The City will provide continuous access to the remaining portion of the site throughout 1 i

Y construction. The City shall, upon completion of any work within the property covered
by the agreement, restore the surface of the site, and any private improvementsiz

OMEGA was on lunch break upon arrival.
DISCUSSIONS WITH CONTRACTOR:
Discussed work to date and timelines with Gordy on site. Gordy indicated that they would be setting up the
temporary bypass using a "Y" fitting at the existing force main. One branch would go to the temporary bypass
while the other branch would be for the permanent installation. He said that they expected to be excavating
for the vault on November 2°d. The vault access hatch that they are using for the lift station is larger than
anticipated, so ensuring the pipe and fitting alignments and grades are crucial; therefore, they are taking
additional time and care to ensure that all of the measurements are exact.
The gravity sewer inlet is lower than expected. Gordy was in the process of resolving why the drop
connection was so low. He indicated that he preferred to have the temporary bypass in the inside of the
existing manhole but it might be on the outside due to the depth of the gravity sewer inlet.
I told Gordy that I would stop by on Monday afternoon.

OMEGA has just begun the excavation for the vault. Two personnel were repairing the 480V line to the
existing pump control panel.
DISCUSSIONS WITH CONTRACTOR:
Spoke to Gordy briefly on site concerning the 480V line repair. He said that while they were potholing for the
outside drop connection, they found an existing PVC conduit. They traced it back for several feet and found
that it was abandoned. They also found another PVC conduit nearby that they had mistakenly thought was the
same abandoned conduit they had been tracing. OMEGA began to take the conduit and wire out and
immediately realized that the conduit with wires went to the existing pump control panel and was active.
Gordy said that they were unsuccessful in finding the outside drop connection and were approximately 5 feet
below grade. Gordy felt that going any deeper was not worth it and he would install the temporary bypass
inside the existing manhole.
OMEGA stopped work at 3:30 for the day. Fred said that they should at the bottom of the excavation by mid-
morning on November 3`d. They will be installing the crushed rock the afternoon of November 3`d

OMEGA was in the process of completing the excavation for the vault. The 480V line to the pump control
panel had been temporarily repaired using PVC conduit and duck tape.
I inspected the excavation for a firm and unyielding surface. The area closest to the existing manhole was soft
and had a small amount of water. In addition, the sides of the excavation were not secure due to the sandy
material of the excavation.
DISCUSSIONS WITH CONTRACTOR:
I discussed the soft spot with Fred on site. The soft spot is outside of the area of the vault. Fred will remove
the soft material that was approximately 1 square foot. It appears that the water is coming from the manhole
through the pick hole that was only grouted on the interior of the manhole.
After I expressed my concern, OMEGA indicated that they would provide shoring in the excavation due to the
vulnerability of the sides of the excavation.
Fred pointed out that the existing force main may have a leak. Warm water is observed in the trench by
OMEGA every morning. Based on the odor, I believe that there is a leak in the line. Fred said that he had
notified the City of his concern. -

OMEGA had installed two swing check valves and two plug valves in the new vault along with associated
piping and appurtenances. Fred indicated they had also installed a 2-inch PVC drain from the vault to the
wetwell. The tree that was located on the comer of the property has been removed. Fred indicated that City
personnel requested that he remove the tree.
DISCUSSIONS WITH CONTRACTOR:
In discussions with Fred, he indicated that they were going to install the temporary bypass system on Tuesday,
November 17`h. They were waiting for the City to complete testing on the MCC that would be completed on
Monday. OMEGA will not be on site Monday. We also discussed the existing force main leak issue. Fred
said that he has not seen any additional signs of leakage since they placed the crushed rock in the excavation.

OMEGA was on site working on the wiring between the wetwell, vault and telemetry panel. Fred was
installing minor appurtenances in the wetwell. The temporary pump was operational. The sump pump that
OMEGA had installed was operational near the vault to keep the groundwater level low.
DISCUSSIONS WITH THE CONTRACTOR:
Fred and I discussed when he would be ready to install the permanent pumps. He said they were ready any
time and were just waiting for it to be scheduled by Gordy. I also notified Fred that the City did not want to
coat the piping in the wetwell or vault.
I spoke with Gordy on the phone an hour later. The switchover from the temporary system to the permanent
system will occur on Wednesday, December 2"d in the morning.

OMEGA was on site pulling wires from the permanent Flygt pumps to the telemetry panel. The other
personnel were conducting a general clean up of the site. OMEGA has filled in the excavation as much as
possible while leaving the .vye exposed for the City.
DISCUSSIONS WITH THE CONTRACTOR:
Hector and I discussed the operation of the pumps since their installation. He indicated that he had not wired
the high level alarm to the telemetry panel but that he would complete it by the end of the day. They had
pumped the wetwell down on Wednesday evening so there would be no overnight issues with high levels.
Hector and I also discussed what alarms are required for the Flygt pumps. Jim Swanson from RH2 will be on
site in the afternoon to work with OMEGA on alarms and statuses.
I spoke with Fred on the phone concerning the coordination with the City to conduct the television inspection
the next few days. He indicated that Gordy had contacted the City for the TV inspecting.

Gordy and Hector were on site with Jim Swanson, Mark Miller, Mike Benoit and Oscar Cantu to demonstrate
the operation of the submersible pumps. Both pumps were operated to the satisfaction of RH2. Average flow
was calculated at 305 gpm for both pumps. This flow rate is above the design flow for the station, which is
120 gpm to meet flushing velocity for the 4-inch PVC force main. Flow into the receiving manhole was
observed during pump operation. Flow at the receiving manhole was laminar. The amp draw for both pumps
when operating was well below the full load atnp rating for the 7.5 hp motors.
Test Data:
Pump 1: Pumped 34.5 inches of water or 608 gallons in 2 minutes for an average flow of 305 gpm. Last
minute was calculated at 361 gpm. The flushing valve is in operation for the first minute. Amps = 7.6.
Pump 2: Pumped 34.5 inches of water or 608 gallons in 2 minutes for an average flow of 305 gpm. Last
minute was calculated at 317 gpm. It is not believed that a flushing valve is installed on Pump 2. Amps = 7.6.
DISCUSSIONS WITH THE CONTRACTOR:
Oscar arranged for Tying of the force main on Wednesday at 1:00 pm with City staff. Omega will be on -site
to assist. Omega was planning on starting site cleanup after the completion of the Tying. Omega was
informed that the City would like them demobilized within the next week. Landscaping will need to occur
when the weather improves.
Jim Swanson worked with Hector to install the level transmitter. The pump station was running on backup
floats connected to the telemetry system. Hector has some cleanup work with the electrical and control that
will be completed next week. R112 will need to return for final testing of the instrtunentation when Omega has
completed their work. See attached checklist for testing to be completed.
Omega was unsun-e if a hushing valve was installed on both pumps. Gordy will confirm and install a second
flushing valve if necessary.
