JUSTIFICATION FOR CHANGE
1. Brief description of proposed contract changes and locations.
C0P#1: 2nd Floor ACM Tile Removal
Per RFI#2, asbestos containing floor tile and mastic were discovered underneath the carpet in five rooms
on the 2nd floor of the 820 building. This change provides payment for the removal and disposal of all
discovered asbestos containing floor tile and mastic.
COP#2: Column ACM Abatement
Per RFI#1 and as noted in PBS Environmental daily reports, asbestos containing black felt paper and
mastic was found in 17 locations around existing concrete columns on the 1 st floor of the 820 building.
This change provides payment for the removal and disposal of these asbestos containing materials.
2. Reason for the changes.
COP#1:The asbestos containing floor tile and mastic were not identified in the hazardous materials survey,
and are an unforeseen condition. This material must be removed and disposed of as hazardous material.
COP#2: Sheet HZM-1 shows asbestos containing flack felt paper and associated mastic around 6 of the 17
column locations. However, additional asbestos containing materials were discovered below grade,
extending approximately 6" below slab grade around 17 columns. Removal required the use of a jack-
hammer. This is a change of conditions and quantities to what was originally bid. This material must be
removed and disposed of as hazardous material.
3. Justification for unit prices or total cost.
COP#1:The Contractor submitted a change order proposal with a lump sum price to complete the work.
This cost was reviewed by the project team and found to be acceptable for the work required.
COP#2: The Contractor provided an estimated cost to complete this work. Time and materials were
tracked on site by the inspector, and a completed lump sum price was agreed to by the Contractor and
the Engineer.
