SUMMARY OF PROPOSED CHANGE:
Reason/Justification: It was determined that a change in mast arm mounted vehicle signal heads,from Type L to Type M,is required
on all mast arm mounted vehicle signal heads at the Lake WA Blvd/Garden Ave N/N Park Dr/Logan Ave N intersection to maintain a
minimum vertical clearance of 16.5'.This change order establishes payment under lump sum for the Contractor to furnish and install
Type M mast arm mounted vehicle signal heads and back plates.See response to RFI#41.
Contract Change: On Drawing No.27,SIGNAL HEAD ASSIGNMENT NOTE#1,change TYPE"L"to TYPE"M".This work is to be
performed on a lump sum basis including all material(including back plates),labor,equipment and coordination to complete the work.
Two additional working days are requested to complete this work.The unit price and amount include the prime Contractor's 12%
markup.

Subject: Request for Change—Signal Mount Brackets& Backplates
Dear Darci,
After careful review of the plans and specifications, it has been determined that the mounting
hardware for the traffic signal heads is wrong. The signal detail chart indicates the mounting
style and that is how our supplier provided the signal head mounts.
The traffic signal heads have been built at our shop and are being stored there.
In order to make the revision as requested by the City of Renton,we will have to purchase new
mountings, and back plates to fit the new mounting style. Once we are able to pick up these
parts we will take the signal heads apart and install the new signal mounting hardware. I would
like to remind everyone, it takes approximately 4 to 6 weeks to procure the material from the
manufacturer.
Our lump sum cost proposal is set at$9,869.17. Please see the attached breakdown. I am
requesting an additional two days be added to the contract beyond those already requested to
complete our work for this change.
We await your written direction before proceeding.
This proposal is based solely on the usual cost elements such as labor, material and normal
markups and does not include any amount for changes in the sequence of work,delays,
disruptions,rescheduling,extended overhead,acceleration and/or impact costs. We expressly
reserve our right to make claim for any and all of the aforementioned items or other items
associated with this change prior to any final settlement of this contract.

Information Required:
Please reference sheet 27, Notes under the Signal Head Assignment#1. This describes what style
mouting hardware is to be used for the traffic signal heads. Our suppiler supplied all type"L"mounts as
prescribed.
The traffic signal detail chart on sheet 30 indicates the height of the mast arms from the ground.
My question is"will there be enough clearance between the signal heads and the roadway once we
install the signal heads"?
The signal heads measure with the back plates for your information the 4 section signal heads measure
5'6"and the 3 section signal heads measure 4'4". Then add 6 inches for the"L"mount and the signal
heads are from 6'6"to 4'10"tall.
The simplex of the poles range from 16.5 feet to 19 feet. The minumum clearance is 16-6" per sheet
30.
Please verify if we have the proper clearance prior to the installation.
Please direct as how to proceed.
Response:
Type"M" mounts shall be used on all mast arm mounted vehicle signal heads
in accordance with direction from the City.A minimum clearance of 16.5' is
required in accordance with the Contract Plans.
