The Local Agency of City of Renton
desires to supplement the agreement entered into with KBA Inc.
and executed on 5/19/11 and identified as Agreement No. CAG-11-105
All provisions in the basic agreement remain in effect except as expressly modified by this supplement.
The changes to the agreement are described as follows:

Section 1, SCOPE OF WORK, is herebyebenged to read: revised to include:

Section IV,TIME FOR BEGINNING AND COMPLETION, is amended to change the number of calendar days for
completion of the work to read: No change.

Section V, PAYMENT, shall be amended as follows:
Authorized maximum amount payable shall increase by$ 75,000.00 to a new maximum amount
payable of$ 2,413,157.00, as set forth in the attached Exhibit E-1-Supplement No. 4.

EXHIBIT A-1 - SUPPLEMENTAL 4
SCOPE OF SERVICES
for Construction Contract Management Services(CCM) -
Rainier Avenue S. (SR 167), South Grady Way to South 2"d Street
Contract No. CAG-11-105
By City of Renton (City) request, this Supplemental 4 Scope of Services adds additional administration
and inspection services for the installation of Filterra Stormwater units within the Project limits of the
Rainier Avenue Improvement Project for the purpose of cleaning the storm water runoff. The work is
funded by a Washington State Department of Ecology (Ecology)grant.
I. CONSTRUCTION CONTRACT MANAGEMENT SERVICES
A. Consultant Contract and Team Management. Continue overall day-to-day management
of the consultant contract and team as outlined in Exhibit A-1, Exhibit A-1 - Supplemental 1, and
Exhibit A-1 -Supplemental 2.
B. Construction Phase Services — Contract Administration (Filterra Stormwater
System). Addition of the following contract administration services for the installation of the
Filterra Stormwater System:
1. Liaison with the Client, construction contractor, Designer, appropriate agencies, property
owners, and utilities.
2. Provide the Client with brief monthly construction progress reports for the Filterra Stormwater
System, highlighting progress and advising of issues which are likely to impact cost,
schedule, or quality/scope.
3. Schedule Review:
a. Review construction contractor's schedules for compliance with Contract Documents.
b. Monitor the construction contractor's conformance to schedule and require revised
schedules when needed. Advise Client of schedule changes.
c. Evaluate construction contractor's construction schedule for Filterra Stormwater System
construction items. Prepare schedule review comments and submit to Ecology.
4. Prepare a Construction Quality Assurance Plan (Filterra QA Plan) as required by Ecology as
referenced in Task 3b of the FY 2011 Stormwater Retrofit and LID Grant Program — Proviso
Funding Agreement, attached hereto as Attachment A, to follow the needed policy and/or
procedures that occur during the Project, and to orient CCM Team to the changes. Submit
the Filterra QA Plan to the Client and Ecology for review.
5. Manage Submittal Process. Track and review, or cause to be reviewed by other appropriate
party, work plans, shop drawings, samples, test reports, and other data submitted by the
construction contractor for general conformance to the Contract Documents.
6. Record of Materials (ROM). Provide ROM indicating anticipated material approvals, material
compliance documentation, and materials testing requirements. Maintain ROM compliance
documentation received and advise of any known deficiencies.
7. Manage Request for Information (RFI) process. Track and review/evaluate, or cause to be
reviewed/evaluated by other appropriate party, RFIs. Manage responses to RFIs.
8. Change Management. Evaluate entitlement, and prepare scope, impact, and independent
estimate for change orders. Facilitate resolution of change orders.

9. Monthly Pay Requests. Prepare monthly requests for payment and/or review payment
requests submitted by the construction contractor. Review with Client and construction
contractor and recommend approval, as appropriate. Prepare cost breakdown for Client to
submit to Ecology for reimbursement. .
10. Assist the Client in the investigation of malfunctions or failures during construction.
11. Prepare change orders. Negotiate, estimate, and prepare the change order documents to
include the needed work to complete the installation of the Filterra Stormwater System.
12. Record Drawings. Review not less than monthly, the construction contractor's redline set of
contract plans. Maintain a CCM Team set of conformed drawings tracking plan changes,
location of discovered anomalies and other items, as encountered by the CCM team. Use
these markups to check the progress of the contractor-prepared Record Drawings.
13. Document Control. Establish and maintain document filing and tracking systems for the
Filterra Stormwater System work, following Client and Ecology guidelines and funding agency
requirements. Collect, organize, and prepare documentation on the Project.
a. One hard copy of files will be kept in the Project field office.
b. Electronic documentation will be stored in a Project Website, using SharePoint software,
managed and hosted by the Consultant.
14. Filterra Stormwater System Closeout. Prepare Stormwater Construction Completion Form
for Ecology approval. Prepare final pay estimate.

C. Construction Phase Services — Field (Filterra Stormwater System). Addition of the
following field services for the installation of the Filterra Stormwater System:
1. Observe the technical conduct of the construction, including providing day-to-day contact with
the construction contractor, Client, utilities and other stakeholders, and monitor for adherence
to the Contract Documents and Ecology guidelines. The Consultant's personnel will act in
accordance with Sections 1-05.1 and 1-05.2 of the Standard Specifications.
2. Observe material, workmanship, and construction areas for compliance with the Contract
Documents, Ecology guidelines, and applicable codes, and notify construction contractor of
noncompliance. Advise the Client of any non-conforming work observed during site visits.
3. Prepare daily construction reports, recording the construction contractor's operations as
actually observed by the Consultant; includes quantities of work placed that day, contractor's
equipment and crews, and other pertinent information.
4. Interpret Contract Documents, in coordination with Designer.
5. Decide questions which may arise as to the quality and acceptability of material furnished,
work performed, and rate of progress of work performed by the construction contractor.

6. Coordinate with permit holders on the Project to monitor compliance with approved permits, if
applicable.
7. Prepare field records and documents to help assure the Project is administered in
accordance with funding agency requirements.
8. Attend and actively participate in regular on-site meetings.
9. Take periodic digital photographs during the course of construction. Photographs to be
labeled and organized as detailed in the Filterra-CCM Plan.
10. Punch List. Upon substantial completion of the Filterra Stormwater System work, coordinate
with the Client and affected agencies to prepare a `punch list' of items to be completed or
corrected. Coordinate final inspection with those agencies.

11. Testing. Conduct or cause to be conducted, materials and laboratory tests. Coordinate the
work of the Field Representative(s) and testing laboratories in the observation and testing of
materials used in the construction; document and evaluate results of testing; and inform
Client and construction contractor of deficiencies.

• Punch List, Certificate of Substantial Completion

D. Assumptions. Addition of the following assumptions for the Filterra Stormwater System work:
1. Budget:
a. Staffing levels are anticipated in accordance with the attached budget estimate.
Consultant services are budgeted for a 12-month period. This is intended to span the
originally planned construction duration, plus time allotted for additional inspection and
closeout.
b. If additional budget is needed to cover such instances as the following, Client and
Consultant will negotiate a supplement to this Agreement:
i. The contractor's schedule requires inspection coverage of extra crews and shifts.
ii. The construction contract runs longer than the time period detailed above.
iii. Any added scope tasks.
c. The budget allocations shown on Exhibit G-1 Supplemental 4 are itemized to aid in
Project tracking purposes only. The budget may be transferred between tasks or people,
or between labor and expenses, provided the total contracted amount is not exceeded
without prior authorization for work associated with the Filterra Stormwater System.
d. Retain Engineer of Record for shop drawing review, RFI's, design changes, and final
record drawings.
e. Coordination with and enforcement of utility franchise agreements and/or contracts and
schedules for services related to the Filterra Stormwater System.
f. Verify that the required permits, bonds, and insurance have been obtained and submitted
by the construction contractor.
g. Construction Survey. Provide Project control survey and staking that is not already
assigned to the construction contractor.
2. Scope: Scope assumptions remain the same for the Filterra Stormwater System work, as
already outlined in the original Exhibit A-1, Exhibit A-1 - Supplemental 1, and Exhibit A-1 —
Supplemental 2.

II. OPTIONAL SERVICES
All services not detailed above are considered Optional Services which, along with any other Extra Work
requested by the Client, will be performed only when a mutually negotiated Supplement to this Agreement
is executed, specifying scope of services and budget. In addition to the Potential Optional Services
already outlined in Exhibit A-1, Exhibit A-1 — Supplemental 1, and Exhibit A-1 — Supplemental 2, the
following has been added:

1. Coordinate media releases and traffic advisory updates.
