CONTRACT CHANGE ORDER AGREEMENT
CONTRACT: CAG 10-120 C.O.# 2
LAKE WASHINGTON BEACH LIFT STATION REPLACEMENT
CONTRACTOR: Kamins Construction.
SUMMARY OF PROPOSED CHANGE:
Provide all labor, equipment, subcontracting, bonding, insurance for the complete work required for the extra
work as outlined below:
Additional Items Under This Change Order Working Days
NEW ITEM:Additional working days due to revision of the approved 165
motor control panel. The need to send the motor control panel back to
PumpTech for modifications caused a delay in the project. This caused the
project to extend past key dates when use of the park significantly increases.
The City and the Contractor determined that it was in the best interest of both
parties to extend the working days of the project to allow the scheduling of
work to minimize impacts to the users of the park.

NEW ITEM:Additional work to remobilize due to revision of the LS 1 $2,300.00 $2,300.00
approved motor control panel. As part of the extension of the working
days,it was understood that there would be a point where the contractor
would move off-site and remobilize back in after the Labor Day Weekend
when use of the park lessens.

Sales Tax(9.5%) $218.50
