CONTRACT CHANGE ORDER AGREEMENT

LAKE WASHINGTON BEACH LIFT STATION REPLACEMENT
CONTRACTOR: Kamins Construction.
SUMMARY OF PROPOSED CHANGE:
Provide all labor, equipment, subcontracting, bonding, insurance for the complete work required for the extra
work as outlined below:

NEW ITEM:Additional cost to upgrade Hot Box HB.75 to Hot Box HB 1 LS 1 $257.43 $257.43
NEW ITEM:Remove and dispose of additional asphalt outside scope of LS 1 $223.00 $223.00
work. Additional removal was to clean up failing asphalt adjacent to the
work zone to provide for proper restoration of the parking lot.
NEW ITEM:remove existing base and replace with recycled concrete. LS 1 $1,006.69 $1,006.69
The site was prepared for paving. The old utility leaked into an area creating
a soft spot that needed to be removed and replaced in order to get the
parking lot paved to allow reopening for public access.
NEW ITEM:Additional asphalt paving outside scope of work. Additional LS 1 $616.00 $616.00
asphalt paving adjacent to the work zone to provide for proper restoration of
the parking lot.
NEW ITEM:Additional work to revise approved motor control panel. LS 1 $2,773.71 $2,773.71
Additional work to send motor control panel back to Pump-rech for
modifications including costs associated with the time delay while waiting for
the return of the panel.
Sub Total $4,876.83
