GENERAL INFORMATION
Mack Cameron requested Mayes/Terracon be at the Rainier Welding shop located at 12441 Bartholomew for the following
services: Steel Fabrication. The results of observations for today were reported to Welder on site.
The construction documents referenced for this observation included: shop drawing 127.
FABRICATION OBSERVATION
Terracon provided shop fabrication observation at the request of the fabricator.
Welding was performed prior to Mayes/Terracon’s arrival. Welder qualifications, filler metal and WPS could not be verified.

ADDITIONAL COMMENTS
Performed ultrasonic testing of two beam splices per shop drawing 127.
Backing was left in place on one flange at both splices (on inside of flange), drawing called for backing to be removed.

Based on our observations and testing, the above noted items appear to be in general accordance with the project
documents.
