Reason for Change: The reasons for the deletion of each bid item are as follows: Bid Item No.5,FASite Cleanup of Bio.and Physical Hazards This bid item was included in the construction contract.Over the years,the project site has a history of being periodically occupied by transients.Therefore,the probability of encountering associated biological and physical hazards requiring cleanup for a safe working environment was not zero.A site assessment was performed by a certified industrial hygienist as required by the contract.The assessment was part of the Health and Safety Plan,required by Bid Item No.4,Health and Safety Plan.The site Change Order No.11_____ Contract No.:CAG22062 Contract Title:Bronson Way Bridge Seismic Retrofit and Painting Project No.:TED4004020 Page 3 of 5 assessment determined that cleanup was not required.Consequently,Bid Item No.5,FASite Cleanup of Bio.and Physical Hazards,was not necessary or used. Bid Item No.8,Flaggers The Bronson Way Bridge was fully closed to vehicles and pedestrians for the duration of construction activities over two separate periods.As a result,the traffic control plans included in the contract for a full bridge closure were utilized.These traffic control plans did not include flagging stations because a full bridge closure does not warrant them.The WSDOT Standard Specifications states that flaggers are paid to be at designated flagging station(s)per approved traffic control plans.Consequently,Bid Item No.8,Flaggers,was not necessary or used. Bid Item No.19,Job Mix Compliance Price Adjustment This bid item only applies if the HMA job mix prepared at the asphalt plant tests outside the acceptance limits specified in the contract for asphalt binder content,aggregate gradation and air voids.For this project,the test results of the HMA job mix from the asphalt were within the acceptance limits specified in the contract. Consequently,Bid Item No.19,Job Mix Compliance Price Adjustment,was not necessary or used. Bid Item No.20,Compaction Price Adjustment This bid item only applies if the HMA fails to meet compaction requirements specified in the contract.For this project,the HMA placed met compaction requirements.Consequently,Bid Item No.20,Compaction Price Adjustment,was not necessary or used. Bid Item No.27,Deficient Strength Conc.Price Adjustment This bid item applies if the concrete fails to meet the compressive strength specified in the contract.However,the contract also required the concrete precast bearing pedestals and concrete jacking pads to reach a minimum compressive strength prior to commencement of bridge jacking.Concrete was delivered and placed for these elements.Unfortunately,the concrete was not achieving the specified strength as determined from independent materials testing results.Further investigation revealed additional water was added to the mix during concrete placement that exceeded the supplier’s recommendations.The concrete was subsequently demolished and removed and a second pour commenced that ultimately achieved the minimum specified compressive strength per the specifications.Consequently,Bid Item No.27,Deficient Strength Conc.Price Adjustment,was not necessary or used. Bid Item No.32,Modify Bridge DrainSidewalk This bid item was intended to cover the costs associated with the installation of new drain pipe extensions at four locations in the downstream sidewalk on Span 2.The purpose of drain pipe extensions is to convey surface water to discharge points below the main steel girders.When the bridge was originally constructed,drain pipe extensions were installed for all bridge deck drains 4 locations)and sidewalk drains 8 locations). In the late 1970’s WSDOT allowed a franchise utility to install new conduit runs on the bridge. The proposed alignment of one of these conduit runs conflicted with the existing drain pipe extensions for the downstream sidewalk drains.Rather than adjust the alignment of the new Change Order No.11_____ Contract No.:CAG22062 Contract Title:Bronson Way Bridge Seismic Retrofit and Painting Project No.:TED4004020 Page 4 of 5 conduit run to avoid the conflict,the drain pipe extensions were removed.The sidewalk drain modification work was removed from this contract,prior to bid advertisement,because the work requires relocation of existing franchise utility conduits and the structure presently has no clear pathways for relocation.The contract plans state that this work is NOT IN CONTRACT”. Consequently,this work was not performed and Bid Item No.32,Modify Bridge DrainSidewalk, was not necessary or used. Bid Item No.40,Erosion/Water Pollution Control The Erosion/Water Pollution Control bid item is a Force Account item that was included in the contract to pay for the maintenance of water pollution and erosion control devices e.g.Inlet Protection,Wattle).Due to the weather dry conditions during the work and the relatively short duration of construction activities, maintenance was not required for the types of devices used on the project.Consequently,the bid item Erosion/Water Pollution Control was not necessary or used. Bid Item No.43,Wattle This bid item was intended to control soil erosion in the slopes adjacent to the bridge.However,the configuration of the temporary work access constructed for the project did not disturb the slopes.Consequently,Bid Item No.43,Wattle,was not necessary or used. Bid Item No.44,High Visibility Fence This bid item was intended to define the project limits and control access to the bridge.However,temporary fencing was utilized to control access and the project was restricted to the bridge structure.Consequently,Bid Item No.44,High Visibility Fence,was not necessary or used. The reason(s)for the change in unit price for specific bid item(s)is as follows: Bid Item No.36,Testing and Disposal of Containment Waste Work performed under this bid item is paid by Force Account.The unit price listed in the Schedule of Prices is an estimate.The actual costs associated with the Work exceeded the amount specified in the Schedule of Prices by 388%.Therefore,an adjustment in the contract budget is necessary to account for this overrun. Extension of Time: The Time for Completion is extended 0 Working Days. Sections 104.4 and 104.5 of the 2022 WSDOT/APWA Standard Specifications shall govern this Change Order.The Work of the referenced Contract is modified to include the changes detailed herein.The payment provided for herein shall constitute the complete and final settlement for all costs of labor,equipment,materials,overhead,profit,permit fees,and all other claims that may be made by the Contractor as a result of this change.
