Hello Alex/Krista,
I hope you both enjoyed the holidays. Attached is a draft contract proposal to continue the
mental wellness check-ins for 2024. I used the 2023 contract and updated the costs using
the service provider's 2024 costs. We also changed the number of check-ins per quarter. For
2023, we were allotted 14 check-ins per quarter whereas 2024 we are doing 10 per quarter.
One additional note regarding the number of check-ins was that if we came across a
scenario where we needed more than 10 check-ins a particular quarter, the service provider
would just bill us for the number of additional visits used. I did not add that language to the
contract so if you need me to add it I can. Please let me know if this works.
